An Imaging Protocol To Discriminate Specialized Conduction Tissue During Congenital Heart Surgery.

Semin Thorac Cardiovasc Surg 2019 Feb 6. Epub 2019 Feb 6.

Department of Cardiac Surgery, Boston Children's Hospital, Boston, Massachusetts; Department of Biomedical Engineering, University of Utah, Salt Lake City, Utah.

Objectives: We performed preclinical validation of intraoperative fiber-optic confocal microscopy (FCM) and assessed its safety and efficacy in an ovine model of the pediatric heart.

Methods: Intraoperative imaging was performed using an FCM system (Cellvizio, Mauna Kea Technology, Paris, France) with specialized imaging miniprobe (GastroFlex UHD, Mauna Kea Technologies). Before imaging, we applied an extracellular fluorophore, sodium fluorescein, to fluorescently label extracellular space. Read More

'External Plication of the Atrialised Right Ventricle in the Repair of Ebstein's Anomaly'.

Semin Thorac Cardiovasc Surg 2019 Feb 5. Epub 2019 Feb 5.

Department of Pediatrics, Artemis Hospital, Delhi, India.

We describe a change in our technique for plication of the atrialised right ventricle during a Cone repair. The plication was performed from the 'Outside' -on a beating heart. Advantages that we expect are- all major coronaries are better seen when full of blood and can be avoided, if a major vessel is compromised - EKG changes should point it out immediately, there is a small but significant saving in terms of arrest time and if the plication needs to be taken down because of an adverse coronary event- this can be done without the need of another cardioplegia. Department of Cardiovascular Surgery, Osaka University Graduate School of Medicine, Suita, Osaka, Japan. Electronic address:

Objectives: We investigated long-term outcomes following aortic valve replacement (AVR) in asymptomatic patients with severe aortic regurgitation (AR) and normal left ventricular (LV) function.

Methods: We reviewed 268 patients who underwent isolated AVR for chronic severe AR from 1991-2010 and enrolled 162 asymptomatic patients with normal LV ejection fraction (LVEF) (≥50%) preoperatively. They were divided into 2 groups according to LV dimension at surgery, the early stage C group (indexed LV end-systolic diameter ≤25 mm/m and LV end-diastolic diameter ≤65 mm, n=61), and late stage C group (indexed LV end-systolic diameter >25 mm/m and/or LV end-diastolic diameter >65 mm, n=101). Optimal Cerebral Protection Strategies in Aortic Surgery.

Semin Thorac Cardiovasc Surg 2019 Jan 8. Epub 2019 Jan 8.

Division of Cardiothoracic Surgery, Emory University School of Medicine, Atlanta, Georgia. Electronic address:

Cerebral protection strategies in aortic surgery have undergone significant evolution over the years, but its tenets remain rooted in maintenance of hypothermia and cerebral perfusion to limit adverse neurologic outcomes. While deep hypothermic circulatory arrest alone remains a viable approach in many instances, the need for prolonged duration of circulatory arrest and increasing case complexity have driven the utilization of adjunctive cerebral perfusion strategies. In this review, we present the most recent studies published on this topic over the last few years investigating the efficacy of deep hypothermic circulatory arrest, retrograde cerebral perfusion, and unilateral and bilateral antegrade cerebral perfusion, as well as the emerging trend toward mild and moderate HCA temperatures. Mathematical Blueprint of a Mitral Valve.

Semin Thorac Cardiovasc Surg 2019 Jan 8. Epub 2019 Jan 8.

The Mitral Laboratory, Yale School of Medicine, New Haven, Connecticut; Section of Cardiac Surgery, Department of Surgery, Yale School of Medicine, New Haven, Connecticut. Electronic address:

Mathematical modeling tries to simplify understanding and proposes a fundamental mechanism that governs the motion and function of a complex biological system such as a mitral valve (MV) motion which represents a dynamic interplay between papillary muscle (PM) position in the context of left ventricular (LV) shape dynamics. Current therapeutic strategies to intervene on the MV may not have exploited these relationships due to lack of understanding of the interactions. We present a MV 3D mathematical model characterized by LV shape dynamics to understand fundamental working principles of ventriculo-papillary-mitral complex. Read More

Semin Thorac Cardiovasc Surg 2019 Jan 4. Epub 2019 Jan 4.

Duke University Medical Center, Department of Surgery, Durham, North Carolina. Electronic address:

Utilization of extracorporeal membrane oxygenation (ECMO) has increased dramatically over the last decade. Despite this trend, many medical centers have limited, if any, access to this technology or the resources necessary to manage these complex patients. In an effort to improve the current infrastructure of regional ECMO care, ECMO centers of excellence have an obligation to partner with facilities within their communities and regions to increase access to this potentially life-saving technology. A Randomized Study Comparing the Incidence of Postoperative Pain After Phrenic Nerve Infiltration Vs Nonphrenic Nerve Infiltration During Thoracotomy.

Semin Thorac Cardiovasc Surg 2018 Dec 7. Epub 2018 Dec 7.

Department of Cardiothoracic Surgery, University Hospital of South Manchester NHS Foundation Trust, Manchester, UK.

Thoracotomy is a common surgical procedure performed worldwide for lung disease. Despite major advances in analgesia, patients still experience severe shoulder, central back and surgical incision site pain in the postoperative period. This study aimed to assess whether intraoperative phrenic nerve infiltration reduces the incidence of postoperative pain and improves peak flow volume measurements during incentive spirometry. Five Maneuvers to Facilitate Faster Robotic Mitral Valve Repair.

Semin Thorac Cardiovasc Surg 2018 Nov 12. Epub 2018 Nov 12.

Division of Thoracic and Cardiovascular Surgery, Cleveland Clinic, Cleveland, Ohio.

Robotically-assisted mitral valve reconstruction has become a cornerstone in the management of degenerative mitral valve regurgitation. High-volume centers report exceptional outcomes with minimal morbidity and mortality in select patient populations. As compared to conventional sternotomy, robotically-assisted surgery has yielded comparable efficacy and safety, with potential benefits of improved cosmesis and a faster recovery period. Read More
