Supraclavicular approach for subclavian vein catheterization in pediatric anesthesia: The reborn of an ancient technique with the ultrasound's assistance.

Rev Esp Anestesiol Reanim 2019 Feb 1. Epub 2019 Feb 1.

Universidad Nacional de Colombia, Bogotá DC, Colombia; Fundación Hospital Pediátrico de la Misericordia, Bogotá DC, Colombia.

The insertion of a central venous catheter is a procedure frequently performed in paediatric anaesthesia. The most frequent approach for this procedure is the internal jugular vein. However, some specific factors in children, such as the short neck and the easy collapse of this vein, which is accentuated in situations such as hypovolaemia and changes in the respiratory pattern, can lead to greater difficulty. Double intracardiac thrombosis in a patient assisted by a venoarterial extracorporeal membrane oxygenation.

Rev Esp Anestesiol Reanim 2019 Feb 1. Epub 2019 Feb 1.

Servicio de Anestesiología, Reanimación y Terapéutica del Dolor, Hospital Clínico Universitario de Santiago de Compostela, Santiago de Compostela, A Coruña, España; Departamento de Cirugía y Especialidades Médico-Quirúrgicas, Universidad de Santiago de Compostela, Santiago de Compostela, A Coruña, España.

Intracardiac thrombosis is a rare complication in patients receiving haemodynamic support with venous-arterial extracorporeal membrane oxygenation (VA-ECMO), but it has a high risk of mortality. This case report describes a patient who suffered cardiogenic shock after a ST-segment elevation myocardial infarction (STEMI) and who presented with intracardiac thrombosis during VA-ECMO support on two occasions after mitral valve replacement. The first one was after the insertion of a mechanical prosthesis, and the second, after replacing it with a biological valve. Read More

Lumbar erector spinae plane block: Successful control of acute pain after lumbar spine surgery - A clinical report.

Departamento de Anestesiología y Terapia del Dolor, Centro Hospitalar de Trás-Os-Montes e Alto Douro, EPE, Vila Real, Portugal.

We report the successful clinical case of a patient scheduled for lumbar spine surgery in which we performed a bilateral single-shot erector spinae plane block as part of a multimodal analgesic strategy for pain control. Performing the block preoperatively dismissed the need for extra intraoperative opioids other than those for intubation, and enabled the use of paracetamol for analgesia only. Further, there was no need for hypotensive techniques, as the block provided satisfactory sympathetic blockade and regional vasodilation with a clear surgical field. Read More

The importance of the safety of our patients in the surgical theatre, has driven many projects. The majority of them aimed at better control and clinical performance; mainly of the variables that intervene or modulate the results of surgical procedures, and have a direct relationship with them. The Spanish Society of Anesthesiology, Critical Care and Therapeutic Pain (SEDAR), maintains a constant concern for a variable that clearly determines the outcomes of our clinical processes, "unintentional hypothermia" that develops in all patients undergoing an anesthetic or surgical procedure. Anaphylactic shock versus Kounis syndrome in cardiac surgery: Differential diagnosis.

Rev Esp Anestesiol Reanim 2019 Jan 2;66(1):53-56. Epub 2018 Oct 2.

Servicio de Anestesiología y Reanimación, Complejo Asistencial Universitario de León, León, España.

Although Kounis syndrome was described almost 3 decades ago, there has been a notable increase in the reports of cases of acute coronary syndromes developed in the context of allergic reactions, also known as Kounis syndrome. This article discusses the diagnostic possibility in the face of an acute biventricular failure in the course of an anaphylactic reaction during the intra-operative period of a cardiac valve surgery. Experience in anaesthetic management of non-cardiac surgery in patients with ventricular assist devices.

Rev Esp Anestesiol Reanim 2019 Jan 25;66(1):37-45. Epub 2018 Aug 25.

Servicio de Anestesia y Reanimación, Hospital Puerta de Hierro, Majadahonda, Madrid, España.

Heart failure (HF) is a syndromic condition with a high incidence in current medicine. When the symptoms of HF progress, and become refractory, cardiac transplant is the best therapeutic option. However, due to the shortage of donors and the long waiting lists, many of those patients are candidates for implantation of ventricular assist devices as a bridge to the cardiac transplant, or when this is not an option, as a definitive therapy. Case report: serratus intercostal plane block/BRILMA and sedation for mastectomy in a high risk patient.

Rev Esp Anestesiol Reanim 2019 Jan 14;66(1):46-48. Epub 2018 Aug 14.

Departamento de Obstetricia y Ginecología, Hospital Universitario de Burgos, Burgos, España.

Breast cancer surgery can benefit from regional anaesthesia techniques, which can be used as a sole anaesthetic procedure in some cases where risk for general anaesthesia is high. Regional anaesthesia allows early recovery, reduces postoperative opioid and non opioid analgesics consumption and helps early home discharge, reducing thus costs. We present a case of an 87-year-old woman with multiple comorbidities who underwent breast cancer surgery. Read More
