Necrobiotic Xanthogranuloma on 18F-FDG PET/CT.

Clin Nucl Med 2020 Dec;45(12):967-969

From the University of North Carolina School of Medicine.

We report the case of a 57-year-old man who presented with a 6-year history of yellowish plaques on his left buttock and right medial knee, as well as ulcerating subcutaneous nodules on his forearms bilaterally. Skin biopsy, bone biopsy, and serum protein electrophoresis confirmed the diagnosis of necrobiotic xanthogranuloma with associated monoclonal gammopathy. Necrobiotic xanthogranuloma is a rare form of non-Langerhans histiocytosis characterized by development of chronic cutaneous nodules that enlarge over time to form plaques. Read More

A Multicenter Cross-Sectional Study and Systematic Review of Necrobiotic Xanthogranuloma With Proposed Diagnostic Criteria.

JAMA Dermatol 2020 03;156(3):270-279

Department of Dermatology, Brigham and Women's Hospital, Harvard Medical School, Boston, Massachusetts.

Importance: Necrobiotic xanthogranuloma (NXG) is a non-Langerhans cell histiocytosis classically associated with paraproteinemia attributable to plasma-cell dyscrasias or lymphoproliferative disorders. Despite the morbidity of NXG, the literature is limited to case reports and small studies, and diagnostic criteria are lacking.

Objective: To evaluate the characteristics of NXG and propose diagnostic criteria. Read More

Necrobiotic xanthogranuloma associated with smoldering multiple myeloma: satisfactory response to cyclophosphamide, dexamethasone, and thalidomide.

An Bras Dermatol 2019 07 29;94(3):337-340. Epub 2019 Jul 29.

Dermatology Service, Hospital das Clínicas, Universidade Federal de Minas Gerais, Belo Horizonte, MG, Brazil.

Necrobiotic xanthogranuloma is a rare chronic condition, belonging to the group C non-Langerhans cell histiocytoses, which is relevant due to the possibility of extracutaneous involvement and association with systemic diseases, particularly hematologic malignancies. The case reported here was only diagnosed after nine years of evolution and was associated with plasma cell dyscrasia. After treatment with cyclophosphamide, dexamethasone, and thalidomide, there was a reduction of cutaneous lesions and serum levels of monoclonal protein. Read More

Necrobiotic Xanthogranuloma as the Presenting Manifestation of Smouldering Myeloma.

J Assoc Physicians India 2019 Mar;67(3):87-88

Department of Dermatology, All India Institute of Medical Sciences, New Delhi.

Necrobiotic xanthogranuloma is a rare dermatological manifestation of underlying hematological malignancies, in particular, when associated with paraproteinemia. These patients who are clinically symptomatic with chronic papules, nodules or plaques which demonstrate a histopathological pattern suggestive of extensive and frequently confluent areas of necrobiosis with granulomatous infiltration, warrant evaluation for an underlying monoclonal gammopathy. Non-infectious granulomatous dermatoses.

J Dtsch Dermatol Ges 2019 May;17(5):518-533

Department of Dermatology, Basel University Hospital, Basel, Switzerland.

Granulomatous dermatoses comprise a wide range of etiologically and clinically distinct skin diseases that share a common histology characterized by the accumulation of histiocytes include macrophages. While the pathogenesis of these disorders is not fully understood, the underlying mechanism is thought to involve a reaction pattern caused by an immunogenic stimulus. Antigen-presenting cells and the effect of various cytokines play a key role. Read More

TIMP-1 Mediates Inflammatory and Immune Response to IL-6 in Adult Orbital Xanthogranulomatous Disease.

Ocul Immunol Inflamm 2020 11;28(2):288-297. Epub 2019 Apr 11.

Department of Ophthalmology, Ninth People's Hospital, Shanghai JiaoTong University School of Medicine, Shanghai, P.R. China.

: To explore the pathogenesis that TIMP-1 mediated in adult orbital xanthogranulomatous disease (AOXGD), a rare type of non-Langerhans histiocytosis that damages the appearance and quality of life of patients: We reviewed 22 patients diagnosed with AOXGD based on clinical manifestations and histological analysis, and then investigated the expression of TIMP-1 and IL-6 with q-PCR and IHC in AOXGD tissues and the possible mechanism involved in the induction of TIMP-1 by IL-6. IL-6 and TIMP-1 were significantly increased in AOXGD tissues. IL-6 promoted TIMP-1 production by M1 macrophages by stimulating the phosphorylation of JAK2 and STAT3. Read More
