Inhaled corticosteroids (ICS) are the cornerstone of the management of asthma. Daily use allows to reduce mortality, intensity and frequency of exacerbations, to increase the control of symptoms of asthma and the quality of life of asthmatics patients and to reduce the decline of the lung function. A daily use of a weak dose of ICS allows to control the symptoms of asthma of the vast majority of mild to moderate asthmatics patients who account for about 75-80 % of the French asthmatic patients. Inserm U1168, VIMA (aging and chronic diseases, epidemiological and public health approaches), Villejuif, France; UVSQ, UMR-S 1168, université Versailles, Saint-Quentin-en-Yvelines, France.

The prevalence of asthma has increased rapidly since the early 1970s, and only changes in exposure to environmental factors; which go together with changes in lifestyle, are likely to explain such a rapid increase. Exposure to allergens is a risk factor for allergic sensitization, and allergic sensitization is a risk factor for allergic asthma. However, apart from indoor mold exposure as a risk factor for childhood asthma, there is insufficient evidence to conclude that the associations between allergen exposure and asthma development are causal. AP-HP, Paris V, Paris Descartes faculty of medicine, Cochin hospital, department of digestive, hepatobiliary and pancreatic surgery, 75014 Paris, France.

Pancreatic ductal adenocarcinoma (PDAC) remains a dismal prognosis and surgery is the only chance for cure. However, only few of the patients have localized tumor eligible for curative complete resection. Preoperative management and well-staging of the disease are the cornerstone for appropriate surgery and major issues to define the best therapeutic strategy. Read More

Chemotherapy for pancreatic cancer.

Presse Med 2019 Mar 15;48(3 Pt 2):e159-e174. Epub 2019 Mar 15.

Heidelberg University Hospital, Department of Surgery, Heidelberg, Germany.

Chemotherapy is an important part of multimodality pancreatic cancer treatment. After curative resection, adjuvant chemotherapy can significantly improve disease free survival and overall survival. The current standard of care is six months adjuvant chemotherapy with modified folinic acid, 5-fluorouracil, irinotecan and oxaliplatin (mFOLFIRINOX) in patients fit enough for this protocol, otherwise six months of gemcitabine and capecitabine based on the European Study Group for Pancreatic Cancer (ESPAC)-4 study. Epidemiology and burden of pancreatic cancer.

Presse Med 2019 Mar 14;48(3 Pt 2):e113-e123. Epub 2019 Mar 14.

Unit of Clinical Epidemiology, IEO, European Institute of Oncology IRCCS, Milan, Italy. Electronic address:

Pancreatic cancer, although infrequent, has a very poor prognosis, making it currently the fourth common causes of cancer mortality in most developed countries including the European Union (EU). Its incidence varies across regions, which suggests that lifestyle factors play an important role in its etiology, although part of the variation could be ascribed to difference in diagnostic and coding practices. Because pancreatic cancer is strongly age-dependent, increasing population longevity and ageing will lead to an increase of the global burden of pancreatic cancer. Read More

Palliative and supportive care holds a major place in pancreatic ductal adenocarcinoma (PDAC) management. It aims to prevent and reduce symptoms and hospital admissions, while ensuring optimal health-related quality of life (HRQoL), which has been reported to be correlated with overall survival in PDAC. Best supportive care includes non-specific treatment of pain, anxiety and depression, chemotherapy-related toxicities, as well as thromboembolic disease treatment and prevention in high-risk patients. Pancreas Translational & Clinical Research Center, San Raffaele Scientific Institute, Milan, Italy; "Vita-Salute" University, San Raffaele Scientific Institute, Milan, Italy. Electronic address:

The correct diagnosis of a patient presenting with a solid pancreatic mass requires a careful diagnostic work-up, since many differential diagnoses are possible that completely alter the following treatments. In our chapter, we have discussed the clinical approach to the problem in a sort of diagnostic flow-chart. Firstly, we analysed the different potential presentations of a solid pancreatic mass, which can be both asymptomatic or symptomatic, and the differential diagnosis based on the symptoms of presentation. Asthma is often associated with a Th2-type immune response with well-known cellular and molecular actors such as eosinophils, Th2 lymphocytes and associated cytokines such as interleukin-5 or IL-4. Nevertheless, some of the asthmatic patients show clinical manifestations and characteristics that do not correspond to the current pattern of the pathophysiology of asthma.
