Hypolipidemic Therapy and Chronic Kidney Disease: Effects on Cardiovascular Risks and Renal Dysfunction.

Kardiologiia 2019 Mar 8;59(2):79-87. Epub 2019 Mar 8.

Research Institute of Molecular Biology and Medicine at the National Center of Cardiology and Therapy Named After acad. M. Mirrakhimov..

Steady increase in the prevalence of chronic kidney disease (CKD) is a serious public health problem, since CKD potentially leads to the development of end-stage renal disease (ESRD) that requires high-cost replacement therapy and is closely associated with increased risk of developing cardiovascular diseases (CVD), which are the cause of death in most patients. Progression of renal dysfunction and development of CVD are significantly affected by hyper- and dyslipidemia. This review contains results of studies evaluating the effect of hypolipidemic therapy on reduction of cardiovascular risk and slowdown of renal dysfunction in patients with CKD at pre-dialysis and dialysis stages of renal failure, as well as in patients with kidney transplant. Possible genetically determined mechanisms of arterial hypertension in tobacco smoking.

Kardiologiia 2019 Jan 31;59(1S):4-10. Epub 2019 Jan 31.

Novokuznetsk State Institute for Further Training of Physicians - Branch Campus of the FSBEI FPE RМАСРЕ МОН Russia.

Te increase in the prevalence of arterial hypertension (AH) in populations, ineffective treatment, the need for risk stratifcation, prevention, early diagnosis and successful treatment, actualize genomic studies to develop a personalized therapeutic approach to AH. Te review investigates the possible genetically determined mechanisms of the development of hypertension and endothelial dysfunction caused by polymorphism of the genes of endothelial nitric oxide synthase (eNOS) and enzymes of phases I and II of the xenobiotics detoxifcation system. Te probable interaction of both systems under the influence of harmful environmental factors, including tobacco smoking, and in the gestational period is discussed. Read More

The Case of Successful Surgical Treatment of a Patient With Thoracic Endograft Infection in 5 Years After Thoracic Endovascular Aortic Repair.

Kardiologiia 2018 Nov 18;58(4):96-100. Epub 2018 Nov 18.

ФГБУ «Институт хирургии им. А. В. Вишневского» Минздрава России.

We present here a case of successful staged treatment of a patient with para-aortic abscess that arose 5 years after thoracic endovascular aortic repair because of thoracic aortic aneurysm. After stabilization of the patient's condition by intensive antibiotic therapy we performed left-subclavian extra-thoracic debranching as the first stage of the surgical treatment. In 2 weeks via median sternotomy and on-pump we removed the infected endograft and performed extraanatomical ascending-to-descending aortic bypass with good postoperative result. Read More

NONVALVULAR ATRIAL FIBRILLATION AND DIABETIC NEPHROPATHY: EPIDEMIOLOGY, PROGNOSIS, AND CHOICE OF ANTICOAGULANT THERAPY.

Kardiologiia 2018 Nov 18;58(4):81-95. Epub 2018 Nov 18.

ФГБОУ ВО «Пермский государственный медицинский университет им. акад. Е. А. Вагнера» Минздрава России.

In this review we present data on prevalence of atrial fibrillation (AF) among patients with type 2 diabetes (T2D), diabetic nephropathy and chronic kidney disease (CKD). Patients with nonvalvular AF and T2D combined with CKD have elevated risk of both bleeding and thromboembolic complications, as well as of all cause death. Efficacy and safety of novel oral anticoagulants (NOAC) depend on comorbidities and can be determined by the presence of T2D and/or diabetic nephropathy. Read More

Chronic Kidney Disease as Basis of High Thrombotic and Bleeding Risk in Patients With Atrial Fibrillation: Place of Oral Anticoagulants.

Kardiologiia 2018 Nov 18;58(4):60-70. Epub 2018 Nov 18.

ФГБОУ ВО «Санкт-Петербургский государственный университет».

Chronic kidney disease (CKD) aggravates course of practically all diseases by worsening outcomes and hindering adequate treatment. Specificities of renal excretion of various drugs, changes of parameters of their pharmacokinetics and pharmacodynamics, nephrotoxic effects of drugs, tactics of drug therapy in conditions of CKD, terminal stage of kidney failure and dialysis are in the focus of attention of internists. To a greatest degree difficulties of drug therapy in CKD and associated clinical states refer to the group of anticoagulants. Read More

Relationship of Remodeling of Carotid Arteries and Left Ventricular Geometry in Patients With Chronic Glomerulonephritis.

Kardiologiia 2018 Nov 18;58(4):45-52. Epub 2018 Nov 18.

Кыргызская государственная медицинская академия им. И. К. Ахунбаева; Кыргызско-Российский Славянский университет им. первого Президента России Б. Н. Ельцина.

Purpose: to study clinical-functional features of remodeling of carotid arteries and its relation to restructuring of the left ventricle (LV) in patients with chronic glomerulonephritis at pre-dialysis stage.

Materials And Methods: We examined 269 patients (189 men, 80 women) with chronic glomerulonephritis (CGN) aged 17-71 years, at pre-dialysis stages of the disease. We analyzed biochemical parameters of peripheral blood with the determination of daily proteinuria and glomerular filtration rate (GFR). Read More

Contractile Function of Isolated Hearts With Preserved and Reduced Ejection Fraction In Vivo.

Kardiologiia 2018 Nov 18;58(4):36-44. Epub 2018 Nov 18.

ФГБУ «Национальный медицинский исследовательский центр кардиологии» Минздрава России.

The aim of the study was comparison of contractile function of isolated hearts of rats with doxorubicin-induced myocardial injury which were tentatively divided according to the level of ejection fraction determined by echocardiography in vivo. After 4 weeks of doxorubicin administration (2 mg/kg subcutaneously once a week) about half of animals had normal (86±1%) and the other half reduced (61±4%) ejection fraction. The first group was defined as diastolic heart failure (DHF) and the other as systolic (SHF). Read More

Kardiologiia 2018 Nov 18;58(4):22-35. Epub 2018 Nov 18.

ФГУ «Федеральный исследовательский центр «Информатика и управление» РАН».

The states characterized by pronounced hypercoagulable components (deep vein thrombosis, cardio- and cerebro-vascular pathologies) are caused by multiple pathophysiological factors, including insufficient supply of magnesium (Mg) and other micronutrients.

Aim: to present results of analysis of the Institute of Microelements Data Base (IMDB) performed from point of view of interrelationships of Mg deficit and hypercoagulable states in adults treated in medico-preventive facilities of Central, Northwestern, Northern, and Siberian federal districts of Russia.

Methods: The analysis was realized as analysis of data obtained in a cross-sectional study. Read More
