Eyelid Reanimation with Free Platysma Graft: Final Stage of Reconstruction after Gunshot Wound to Face.

Plast Reconstr Surg Glob Open 2022 Jun 10;10(6):e4372. Epub 2022 Jun 10.

Department of Plastic Surgery, Beaumont Health Systems, Royal Oak, Mich.

Patients with gunshot wounds to the face have massive soft tissue and bony damage from projectile and blast injuries. They often require multiple, staged reconstructive surgeries with cross-facial nerve grafting and free muscle flap for re-establishment of facial expression. Injury to or total loss of the facial nerve and branches can result in loss of function of the orbicularis oculi muscle, which leads to the loss of protective mechanisms of eyelid function and blink reflex. Simultaneous Double Eyelid Blepharoplasty and Blepharoptosis Correction With Levator Aponeurosis Plication Technique: Clinical Experience of 108 Cases.

Ann Plast Surg 2022 06 18;88(6):606-611. Epub 2022 Feb 18.

From the Department of Plastic and Reconstructive Surgery, Xijing Hospital, Forth Military Medical University, Xi'an, Shaanxi, China.

Background: Fifty percent of Asians are born without a supratarsal fold (also called single eyelid), and double eyelid blepharoplasty is one of the most commonly performed and most popular facial cosmetic surgeries in the Asian population. However, patients with single eyelid frequently present with concomitant mild blepharoptosis (degree of ptosis, ≤2 mm), which often fails to cause the attention of surgeons and misses correction.

Methods: A retrospective study of all patients who underwent double eyelid blepharoplasty and blepharoptosis correction simultaneously with the modified levator aponeurosis plication technique was performed from June of 2017 to June of 2020. Read More

Although eyelid reconstruction by transplanting an autologous free tarsoconjunctival graft (FTG) is a well-established technique, few studies have examined the postoperative course of FTG transplantation for East Asian eyelids, including those of Japanese patients. Therefore, this study investigated complication and reoperation rates after FTG transplantation in the reconstruction of East Asian (Japanese) eyelids. This study included 42 eyelids wherein posterior lobe reconstruction after resection of a malignant tumour of the eyelid was performed by FTG transplantation between 2007 and 2019 at Niigata University Medical and Dental Hospital. The upper lid-brow junction is a complex anatomic zone that undergoes many interconnected age-related changes. Although considerable effort has gone into defining the ideal female eye and brow, no such work has been done for the male. Typically, men develop forehead and glabellar lines in conjunction with either upper lid hooding, brow ptosis, or blepharoptosis, whereas some men develop hollowing of the upper lid sulcus. Minimally invasive temporalis tendon transposition and upper lid lipofilling for immediate and secondary facial reanimation in patients treated for malignant tumors of the parotid gland.

J Craniomaxillofac Surg 2022 May 9;50(5):419-425. Epub 2022 Mar 9.

Maxillo Facial Surgery Department, ASST Santi Paolo e Carlo, Milan, University of Milan, Italy.

Treatment for facial nerve-invading parotid malignancies usually results in complete facial palsy. The authors present a novel technique to treat facial palsy following radical parotid surgery and retrospectively evaluate results in terms of soft tissue symmetry at rest and during smiling and eyelid closure using the eFACE system. 9 patients with facial palsy following parotid malignancies resection or undergoing parotidectomy with planned facial nerve resection for tumor invasion were treated with the association of mini-invasive temporalis flap rotation and upper lid lipofilling to restore symmetry of the middle facial third at rest and during smiling and eyelid closure. Prelamination of the Radial Forearm Free Flap Using Free Full-Thickness Eyelid Skin Grafts: A New Approach for Intraoral Defect Reconstruction.

Ann Plast Surg 2022 Jul 10;89(1):54-58. Epub 2022 Mar 10.

From the Department for Oral and Maxillofacial Surgery, Hannover Medical School, Hannover, Germany.

Abstract: Free flap transfer for reconstruction of intraoral defects is a common procedure in oral and maxillofacial surgery. For tumor-related defects, the radial forearm flap is widely used for soft tissue restoration. However, transfer of free skin grafts to the donor site region is often required for wound closure after free flap harvesting, resulting in esthetic disturbances due to shrinkage of the grafted skin, attendant scarring, or mismatches in skin texture or color. Arcade Suture Upper Blepharoplasty Combined With the Forceps Technique.

Ann Plast Surg 2022 03;88(1s Suppl 1):S68-S72

From the Division of Plastic Surgery, Department of Surgery, Taipei Veterans General Hospital, Taipei, Taiwan.

Background: Double-eyelid blepharoplasty is the most popular cosmetic surgery in Taiwan. The creation of a small palpebral fold along with reduction of eyelid fullness makes the eye look slightly larger, thereby leading to an appearance of youthfulness, alertness, and vitality. Herein, we propose a method of combining the techniques of arcade buried suture and forceps, which is designed to create a more physiologically natural double eyelid. Read More

The New Face of the Switch Flap in the Reconstruction of a Large Upper Eyelid Defect.

Case Rep Ophthalmol Med 2022 12;2022:4159263. Epub 2022 Feb 12.

Faculty of Dental Medicine and Health Care Osijek, University Josip Juraj Strossmayer in Osijek, Croatia.

Reconstruction of a large defect after the removal of a massive malignant upper lid tumor is still a challenge in oculoplastic surgery. Our method of choice is Mustardé switch flap. Due to the lack of Mohs micrographic surgery and frozen section technique as well as waiting time of two weeks for histopathological results, we made modifications enabling us to reexcise in case of positive margins: the width of the pedicle of the flap was 7 mm allowing the length of the flap to be increased if needed, the lids were closed with temporary lateral tarsorrhaphy to protect the eye, and the lower lid is finally reconstructed in the second stage of the procedure. Suneva Medical, San Diego, California, USA.

Introduction: Plasma IQ™, the first FDA-cleared hand-held plasma energy device, is indicated for removal and destruction of skin lesions and coagulation of tissue. Treatment involves use of an electrostatic plasma spark to heat the skin and create discrete microthermal wounds.

Methods: Microthermal wounds on pre-auricular and upper eyelid skin from two individual subjects were created using multiple treatment parameters to assess the impacts of power, pulse duration and needle electrode type on wound depth, width and thermal spread, and analyzed using histology to characterize treatment effects. Background: In East Asians, a common method to ameliorate puffy eyelids is to resect the excess orbital fat and the orbicularis. However, in some patients, a satisfactory improvement cannot be achieved because of the thick submuscular fibroadipose tissue (SMFAT) and minimal orbital fat.
