3 Department of Ophthalmology, Icahn School of Medicine, Mount Sinai Medical Center and New York City Health and Hospitals, New York, NY, USA.

The use of intravitreal bevacizumab in pediatric retinal and uveitic disease has become more widespread over the past decade. This article serves to outline the rationale underlying the use of intravitreal bevacizumab, and which disease entities it should be appropriately thought of as a primary or solo therapy, as opposed to an adjuvant one. Also presented is the relevant literature regarding each of these retinopathies. Read More

Multimodal imaging of torpedo maculopathy including adaptive optics.

Eur J Ophthalmol 2019 Feb 8:1120672119827772. Epub 2019 Feb 8.

3 Aix Marseille Université, CNRS, Institut Neurosciences Timone (INT), Marseille, France.

Purpose:: The etiology of torpedo maculopathy remains unknown, but it has been recently suggested that it could represent a persistent defect in the development of the retinal pigment epithelium. As retinal pigment epithelium and photoreceptors form a functional unit, an alteration of photoreceptor distribution or function is predictable. The aim of this study is to describe multimodal imaging, including adaptive optics, in three cases of torpedo maculopathy, and discuss its pathogenesis. Read More

Remote hydrodissection.

Eur J Ophthalmol 2019 Feb 5:1120672119827774. Epub 2019 Feb 5.

1 Department of Ophthalmology, Rambam Health Care Campus, Haifa, Israel.

We describe a technique for performing hydrodissection in which the cannula tip is not advanced beyond the capsulorhexis edge, nor embedded into the cortex, but instead is placed within the capsulorhexis boundaries, pointing to the capsulorhexis edge, and angled slightly posterior to it. The fluid stream squirted from the cannula creates a cleavage plane between the capsule and cortex (hydrodissection), with a distinct wave dissecting the posterior capsule off the overlying cortex. By later rotating the angle slightly more posteriorly, hydrodelineation occurs, with the resultant "golden ring," and often, multiple "golden rings. Read More

Silicone oil droplets in repackaged anti-vascular endothelial growth factors for intravitreal injections: In search of the main source of contamination.

Eur J Ophthalmol 2019 Jan 15:1120672118823133. Epub 2019 Jan 15.

Tadeusz Krwawicz, MD: The inventor of cryosurgery in ophthalmology.

Eur J Ophthalmol 2019 Jan 14:1120672118823125. Epub 2019 Jan 14.

3 Private Practice, Gdańsk, Poland.

Tadeusz Krwawicz (1910-1988) pioneered the use of cryosurgery in, as ophthalmology. The idea arose in 1959 while experimenting because on lyophilization to store corneas and lenses for transplantation it was difficult to remove the lens without damage the capsule was usually torn. Subsequent experiments on rabbits revealed that touching the lens with a wire cooled to 203°K (-70°C) resulted in firm attachment of the lens capsule and subcapsular masses. Read More

Ex-PRESS mini-shunt implanted in a pregnant patient with iridocorneal endothelial syndrome.

Eur J Ophthalmol 2019 Jan 8:1120672118820508. Epub 2019 Jan 8.

Ophthalmology Department, Tajo Hospital, Aranjuez, Spain.

Introduction:: The failure rate of both filtration surgery and of aqueous shunt implantation is higher for iridocorneal endothelial syndrome than in other scenarios, due to the continuous proliferation of abnormal endothelial cells over the trabecular meshwork and the filtration area and also due to the more pronounced cicatrizing response shown by these young patients. We present the first case ever described in the literature of a pregnant patient with iridocorneal endothelial syndrome and uncontrolled ocular hypertension who was implanted an Ex-PRESS mini-shunt.

Sequence effect in the treatment of proliferative diabetic retinopathy with intravitreal ranibizumab and panretinal photocoagulation.

Eur J Ophthalmol 2018 Dec 12:1120672118812270. Epub 2018 Dec 12.

In-Patient Department 2, Affiliated Eye Hospital of Nanjing Medical University, Nanjing, China.

Purpose:: To compare the outcome of the sequence in the two treatments (intravitreal ranibizumab and panretinal photocoagulation) in high-risk proliferative diabetic retinopathy.

Methods:: This retrospective study included 35 patients with newly diagnosed high-risk proliferative diabetic retinopathy in 43 eyes; 18 (22 eyes) received intravitreal ranibizumab before panretinal photocoagulation (intravitreal ranibizumab+ group), while the other 17 (21 eyes) received panretinal photocoagulation before intravitreal ranibizumab (panretinal photocoagulation+ group). Each subject received three intravitreal ranibizumabs that were interleaved with three panretinal photocoagulations. Read More

Phakic intraocular lens as a temporary barrier in aphakic Descemet's membrane endothelial keratoplasty.

Eur J Ophthalmol 2018 Nov 13:1120672118811743. Epub 2018 Nov 13.

Moorfields Eye Hospital, London, UK.

Background:: Descemet's membrane endothelial keratoplasty has a lower risk of endothelial rejection than Descemet's stripping automated endothelial keratoplasty. In aphakic eyes requiring endothelial transplantation, approaches aimed at reducing the risk of posterior graft dislocation in Descemet's stripping automated endothelial keratoplasty are not transferable to Descemet's membrane endothelial keratoplasty.

Technique:: Here we describe the use of an implantable collamer lens placed over the iris to provide a temporary intraoperative platform, to facilitate graft unfolding and to avoid posterior graft dislocation in an aphakic, vitrectomised eye with a fixed, dilated pupil in a young patient with chronic uveitis and previous failed Descemet's stripping automated endothelial keratoplasty.
