Sexually transmitted infections in the context of a sexual assault.

Enferm Infecc Microbiol Clin 2019 Jan 23. Epub 2019 Jan 23.

Servicio de Pediatría, Hospital Universitario Basurto, Bilbao, Bizkaia, España.

Sexual violence is a worldwide problem with an impact on health that encompasses medical, legal and psychological aspects. Therefore the management of these patients must be multidisciplinary, and action protocols are compulsory. People who are victims of a sexual assault (SA) are at high risk of acquiring a sexually transmitted infection (STI), which must be prevented, diagnosed, treated and followed up post treatment, according to the established action protocol to prevent potential complications. Vaginosis. Vaginal microbiota.

Enferm Infecc Microbiol Clin 2018 Dec 26. Epub 2018 Dec 26.

Departamento de Biología Funcional, Universidad de Oviedo, Oviedo, España; Fundación de Investigación Oftalmológica, Instituto Oftalmológico Fernández-Vega, Oviedo, España; Fundación para la Investigación y la Innovación Biosanitaria del Principado de Asturias (FINBA), Oviedo, España.

The latest advances in the vaginal microbiome and molecular diagnosis of bacterial vaginosis have allowed for a better knowledge of this entity, characterising aspects of its pathogenesis and the establishment of the vaginal biolayer, the models and new theories of its aetiology, how it is transmitted, with it being considered nowadays as a probable sexually transmitted infection, the separation of other entities such as aerobic vaginosis, its molecular diagnosis and treatment with new molecules to prevent frequent relapses. This entity and the study of the vaginal microbiome have made it possible to consider these infections as a polymicrobial syndrome, putting an end to the dogma: one microorganism, one disease. In addition, a lesser-known entity such as aerobic vaginosis and the methods for its detection are updated. Read More

Symtuza in clinical practice.

Enferm Infecc Microbiol Clin 2018 12;36 Suppl 2:31-36

Departamento de Enfermedades Infecciosas, Hospital Ramón y Cajal, Madrid.

Because lifelong therapy is needed in HIV infection, high long-term adherence is necessary. Darunavir/co-bicistat/emtricitabina/tenofovir alafenamide (Symtuza) is the first triple therapy combining a protease inhibitor (PI) in a single tablet regimen. This drug combines the potency and high genetic barrier of the most effective PI, darunavir, with the renal and bone safety of tenofovir alafenamide. Read More

Because of the development of antiretroviral therapy (ART) in the last few years, we now have safe, effective short- and long-term options and also simple regimens, with most combinations being administered once daily in one or 2 tablets and almost no treatment failures. Nevertheless, there are still reasons to switch an effective ART in patients who have been suppressed, a switch that must always be made without losing virological efficacy. With longer survival and the need for lifelong treatment, it is important to have simple and safe treatment options to improve the quality of life in patients with HIV. Read More

This combination is active against a wide variety of HIV strains and, in turn, avoids bone and renal toxicity associated with the use of tenofovir disoproxil fumarate, combining efficacy, convenience, tolerability and high genetic barrier. Vitamin D binding protein, but not vitamin D or vitamin D-related peptides, is associated with septic shock mortality.

Enferm Infecc Microbiol Clin 2018 Nov 20. Epub 2018 Nov 20.

Health Research Institute Valdecilla-IDIVAL, University of Cantabria, School of Nursing, Avenida de Valdecilla s/n 39008, Santander, Spain.

Background: The aim of this study was to assess the prognostic value of vitamin D, vitamin D binding protein (VDBP) and vitamin D-related peptides in septic shock patients in relation to hospital mortality.

Methods: This is a single-center, prospective, observational study that included all consecutive patients meeting criteria for septic shock who were admitted to the ICU. VDBP, 25-hydroxy vitamin D, 1,25-dihydroxy vitamin D, cathelicidin and beta-defensin levels were determined in blood samples obtained on admission to the ICU. Read More

Long-term prognosis after surgery for infective endocarditis: Distinction between predictors of early and late survival.

Enferm Infecc Microbiol Clin 2018 Nov 20. Epub 2018 Nov 20.

Cardiac Surgery Department, Ramon y Cajal Hospital, Madrid, Spain.

Objectives: Cardiac surgery is a life-saving procedure in patients diagnosed with infective endocarditis (IE). There are several validated risk scores developed to predict early-mortality; nevertheless, long-term survival has been less investigated. The aim of the present study is to analyze the impact of IE-specific risk factors for early and long-term mortality. Read More

Epidemiology and current control of sexually transmitted infections. The role of STI clinics.

Enferm Infecc Microbiol Clin 2019 01 19;37(1):45-49. Epub 2018 Nov 19.

Servicio Enfermedades Infecciosas, OSI Bilbao-Basurto, Osakidetza, Grupo de estudio de ITS (GeITS) de la SEIMC, Instituto de Investigación Biocruces Bizkaia, Bizkaia, España. Electronic address:

The burden of disease from sexually transmitted infections (STI) undermines sexual and reproductive health and the health of newborn infants. The presence of some STI facilitates the transmission of others, such as HIV, and cause cellular changes that precede some types of cancer. For all these reasons STIs are a first order uncontrolled public health problem. Read More

Pharmacokinetic/pharmacodynamic analysis as a tool for surveillance of the activity of antimicrobials against Pseudomonas aeruginosa strains isolated in critically ill patients.

Enferm Infecc Microbiol Clin 2018 Nov 15. Epub 2018 Nov 15.

Pharmacokinetic, Nanotechnology and Gene Therapy Group (PharmaNanoGene), Faculty of Pharmacy, Centro de investigación Lascaray ikergunea, University of the Basque Country UPV/EHU, Paseo de la Universidad 7, 01006 Vitoria-Gasteiz, Spain. Electronic address:

Introduction: To evaluate the changes in the susceptibility of Pseudomonas aeruginosa over time (2000-2017) against antimicrobials used in an intensive care unit of a Spanish tertiary hospital, and to compare them with the antimicrobial activity considering theoretical pharmacokinetic/pharmacodynamic (PK/PD) criteria. The influence of the method for handling duplicate isolates to quantify susceptibility rates was also evaluated.

Methods: The susceptibility was studied considering the Clinical and Laboratory Standards Institute (CLSI) breakpoints. Read More

Executive summary: Consensus document of GEHEP of the Spanish Society of Infectious Diseases and Clinical Microbiology (SEIMC), along with SOCIDROGALCOHOL, SEPD and SOMAPA on hepatitis C virus infection management in drug users.

Enferm Infecc Microbiol Clin 2018 Nov 8. Epub 2018 Nov 8.

Unidad de Patología Infecciosa, Hospital Universitario Álvaro Cunqueiro, Instituto de Investigación Sanitaria Galicia Sur, Vigo, Spain.

The micro-elimination of HCV infection in drug users (DU) in our area is a priority in order to achieve the overall elimination of this disease. Coordinated action between specialists in addiction treatment, microbiologists and physicians who treat HCV infection is required to implement infection screening, to achieve universal access to treatment and to prevent new infections and reinfections. The objective of this document was to come to a consensus on the screening, hospital referral, treatment, follow-up and prevention of HCV infection in DU by an expert panel from GEHEP/SEIMC and three scientific societies of addiction treating physicians: SEPD, SOCIDROGALCOHOL and SOMAPA.
