The motto of the German Ophthalmolological Society (DOG) Congress 2019 "Ophthalmology: our medical discipline with a future" raises the question of what "ophthalmological future" should really be, what are its driving forces and what are the foundations of its design. The design of the future, usually equated with "progress", naturally always takes place out of the present, which itself is a product of the past. History, like time and evolution, is always a continuum. Read More

Univ.-Augenklinik Düsseldorf, Heinrich-Heine-Universität, Moorenstr. 5, 40225, Düsseldorf, Deutschland.

Astigmatism is the most frequent refractive error worldwide followed by hyperopia and myopia. Internal astigmatism has to be differentiated from external astigmatism. Furthermore, external astigmatism can be divided into "with the rule", "against the rule" and "oblique". Read More

Ophthalmologe 2019 Feb;116(2):201-212

Universitäts-Augenklinik, Universität Bonn, Ernst-Abbe-Str. 2, 53127, Bonn, Deutschland.

Approximately 500,000 blind and 1 million visually impaired persons live in Germany, which lacks a national blind registry. Therefore data from social welfare agencies and population-based studies are used to estimate prevalence and incidence. Main causes for severe visual impairment and blindness are age-related macular degeneration, glaucoma and diabetic eye diseases. Read More

Neurotrophic Keratopathy: Neurotrophic keratopathy (NK) is a rare corneal disease caused by an impaired corneal innervation. It leads to spontaneous recurrent epithelial defects potentially leading to corneal ulcers and perforation. To avoid severe corneal damage, a prompt and stage-adjusted treatment is necessary. Ophthalmologe 2019 Jan 25. Epub 2019 Jan 25.

Augenklinik, Universitätsklinikum Hamburg-Eppendorf (UKE), Martinistr. 52, 20251, Hamburg, Deutschland.

According to the literature there are a large number of potential systemic contraindications for corneal refractive surgery. In cases with relative contraindications, surgery should be performed only after weighing up the individual risks and under controlled disease conditions; however, the "evidence" for many of these contraindications is based only on case reports or series, expert opinions or known complications in non-laser-assisted eye surgery. Randomized clinical trials are therefore required. Read More

The Kahook Dual Blade (New World Medical, Rancho Cucamonga, CA, USA) is a novel goniotomy instrument for opening Schlemm's canal. Performing the procedure is comparable to the Trabectome (Neomedix, Tustin, CA, USA) under gonioscopic control with the major difference that the trabeculectomy is created by a dual blade instead of electroablation. The currently available clinical data are limited to a small number of retrospective studies with ≤1 year of follow-up. Read More

Universitäts-Augenklinik Bonn, Ernst-Abbe-Str. 2, 53127, Bonn, Deutschland.

A 37-year-old male patient presented with metamorphopsia and unilateral visual impairment with the presence of hundreds of bilateral avascular retinal pigment epithelial detachments (PEDs). The patient suffered from allergic bronchial asthma which was treated with inhaled corticosteroids. Cessation of corticosteroid treatment resulted in flattening of larger PEDs and subsequent transition to atrophic areas over time while smaller PEDs persisted and spread peripherally over an observation period of 14½ years. Service d' Ophtalmologie, Hôpitaux Universitaires de Genève, 22 Rue Alcide-Jentzer, 1205, Genf, Schweiz.

Eye movements are performed in order to either focus the gaze onto a new interesting object or to stabilize the viewed object in the fovea. As every type of eye movement has its own anatomical substrate, it is often possible to localize an eye movement disorder to a specific brain region or to make a diagnosis of a systemic disease in an appropriate clinical setting. Thus, in many patients the assessment of eye movements can effectively help in the diagnosis and appropriate treatment can be initiated in a timely manner. Read More

[Does the micropulse laser have an effect on chronic CSC?]

Ophthalmologe 2018 Dec 21. Epub 2018 Dec 21.

Introduction: Central serous chorioretinopathy (CSC) is the fourth most common cause of vision loss without an evidence-based treatment recommendation. A positive effect of micropulse laser (MPL) treatment has been described in the literature in recent years. This study aimed to reappraise these results in a patient population with chronic CSC. Klinik für Augenheilkunde, Universitätsklinikum Münster, Domagkstr. 15, 48149, Münster, Deutschland.

Background: The foveal avascular zone (FAZ) has often been analyzed under different aspects in the last decades, mainly by fluorescence angiography (FA); however, the novel technology of optical coherence tomography angiography (OCTA) enables a non-invasive examination, visualization and quantitative analysis of the FAZ, which has recently led to many new findings, especially in a multidisciplinary manner. This article provides an overview of the investigation of the FAZ using OCTA and the new findings that have been obtained using OCTA in recent years.

Background And Objective: In this prospective randomized simple blind study, we investigated the long-term results of trabecular micro-bypass stent implantation as part of routine cataract surgery in eyes with primary chronic open-angle glaucoma. The chamber angle implants drain between the anterior chamber and the canal of Schlemm and circumvent the main outflow resistance by the trabecular meshwork.

Zentrum für Augenheilkunde, Uniklinik Köln, Joseph-Stelzmann Str. 9, 50931, Köln, Deutschland.

The term nanophthalmos refers to a clinically small eye that appears morphologically normal. A nanophthalmos is characterized by hyperopia but can also be associated with various secondary pathologies, such as angle-closure glaucoma. In particular, the perioperative risks associated with a nanophthalmic eye necessitate examination of the anatomical characteristics, which can result from the disproportional size of intraocular tissues despite structural normality. Augenklinik und Poliklinik, Universitätsmedizin Mainz, Langenbeckstr. 1, 55116, Mainz, Deutschland.

The treatment of congenital glaucoma requires special expertise and often novel surgical approaches. The combined use of a XEN and a Baerveldt implant is an alternative to conventional tube implants and may be less harmful for long-term corneal endothelial damage. If this technique does not provide sufficient long-term control of intraorbital pressure (IOP), a direct implantation of the Baerveldt tube can be easily performed in a second intervention. Background: Descemet membrane endothelial keratoplasty (DMEK) has become the standard surgical treatment for Fuchs' endothelial corneal dystrophy (FECD) in Germany. In addition to classical FECD, there are endothelial disorders amenable to DMEK associated with complex changes in the anterior segment of the eye, such as anterior synechia of the iris, larger iris defects, after glaucoma surgery, after vitrectomy and after trauma.

[Vitrectomy for endophthalmitis : Incubation time and prognosis following intravitreal injection vs. cataract surgery].

Ophthalmologe 2018 Nov 12. Epub 2018 Nov 12.

Klinik für Augenheilkunde, Akademisches Lehrkrankenhaus der Johannes Gutenberg-Universität Mainz und der Medizinischen Fakultät Mannheim der Universität Heidelberg, Klinikum Ludwigshafen am Rhein, Bremserstr. 79, 67063, Ludwigshafen am Rhein, Deutschland.

Ophthalmologe 2018 Nov 9. Epub 2018 Nov 9.

Klinik für Augenheilkunde, Universitätsklinikum Erlangen, Erlangen, Deutschland.

Background: Optical coherence tomography angiography (OCTA) is a novel noninvasive method which enables a quantitative evaluation of retinal and optic nerve head (ONH) perfusion. In this article, we discuss the principles of the application of OCTA and give a summary of the knowledge gained by using this method in glaucoma patients.

Methods: This article is based on a selective literature search and the analysis of own data. Read More
