Comparison of venous couplers versus hand-sewn technique in free flap breast reconstruction.

Ann Chir Plast Esthet 2018 Dec 5. Epub 2018 Dec 5.

Department of Plastic and Reconstructive Surgery, Centre Hospitalier de l'Université de Montréal (CHUM), 850, Rue Saint-Denis, Montréal, QC H2X 0A9, Canada. Electronic address:

Introduction: Despite positive outcomes reported in the literature, some surgeons remain reluctant to the systematic use of venous couplers in free flap tissue transfer. In our institution, the use of the coupler devices for venous anastomosis in free flap breast reconstruction has yet to be adopted by all surgeons. The purpose of the study was to compare postoperative outcomes of coupler-assisted venous anastomoses with hand-sewn techniques in free flap breast reconstruction. Read More

[Tongue reconstruction by thoracodorsal perforator flap: A new harvesting technique to reduce morbidity].

Ann Chir Plast Esthet 2019 Feb 28. Epub 2019 Feb 28.

Electronic address:

The thoracodorsal artery perforator flap is increasingly used in head and neck reconstructions. One of its multiple advantages is the low donor site morbidity compared to the other free flaps usually used for this type of surgery, such as the radial forearm free flap and the anterolateral flap of the thigh. However, the current harvesting technique of the thoracodorsal artery free perforator flap needs a vertical incision rising high in the axillary hollow for the dissection of the pedicle, thus impeding optimal discretion of the donor site, especially for women. Surgical treatment of large and multiple lesions of irradiated scalp may require subtotal scalp exeresis and reconstruction in two-stage free flap surgery: harvesting and placing the flap first, then scalp removing and defect covering in a second step. This strategy raises the question of how to care the flap between the two surgeries. We report an original technique of free latissimus dorsi flap lacing. Read More

[Evolution of surgical indications in upper limb functional surgery for tetraplegic patients: Retrospective study of 428 procedures].

Ann Chir Plast Esthet 2018 Dec 7. Epub 2018 Dec 7.

Introduction: Regarding surgical strategy for upper limb functional rehabilitation in patients with traumatic tetraplegia, there are few publications and the case series are quite small.

Patients And Method: We reviewed all traumatic quadriplegic patients, operated one by one surgeon, professor Marc Revol, for functional surgery of the upper limb in the same department from 1989 to 2018. For each patient, we recorded their gender, their group according to the international classification, their age at the time of the first surgical procedure, the length of time between the accident and the first surgery and between two procedures, the average duration of the whole surgical program, and the surgical technique used for the elbow, the wrist, the long fingers and the thumb. Abdominoplasty with simultaneous laparoscopic umbilical hernia repair: A practical approach to preserve the umbilical vascularization.

Ann Chir Plast Esthet 2018 Dec 4. Epub 2018 Dec 4.

Hôpital de la Croix-Rousse, 69004 Lyon, France.

Background: Umbilical necrosis is a well-known complication of abdominoplasty, the risk of this complication can be increased when an associated umbilical hernia requires further dissection in peri-umbilical region, potentially leading to umbilical devascularisation. Multiple minimally invasive open techniques were described to avoid this problem. The combined approach of abdominoplasty with laparoscopic umbilical hernia repair is one promising solution to avoid devascularising the umbilicus. Read More

[To harvest a SIEA flap instead of a DIEP flap in breast reconstruction: A technical note].

Breast reconstruction by abdominal flap has evolved to ensure minimal donor-site morbidity with the description of Deep Inferior Epigastric artery Perforator flap (DIEP flap). Being of the same thickness and the same surface, the Superficial Inferior Epigastric Artery flap (SIEA flap) does not require, for it harvesting, to open the abdominal fascia or to dissect through the muscles minimizing again donor-site sequelae. However, it is little used because of the variability of its vascularization and a higher failure rate than the DIEP in the literature. Read More

Postoperative complications of male to female sex reassignment surgery: A 10-year French retrospective study.

Ann Chir Plast Esthet 2019 Feb 27;64(1):24-32. Epub 2018 Sep 27.

Department of plastic, reconstructive and aesthetic surgery, Saint-Louis hospital, avenue Claude-Vellefaux, 75475 Paris cedex 10, France; Plastic and reconstructive surgery, François-Mitterand hospital, 14, rue Paul-Gaffarel, 21079 Dijon, France. Electronic address:

In primary male to female (MTF) sex reassignment surgery (SRS), the most frequent postoperative functional complications using the penoscrotal skin technique remain neovaginal stenosis, urinary meatal stenosis and secondary revision surgery. We aimed to retrospectively analyze postoperative functional and anatomical complications, as well as secondary procedures required after MTF SRS by penile skin inversion. All patients operated on for MTF SRS, using the inverted technique, from June 2006 to July 2016, were retrospectively reviewed. Read More

Primary clear cell sarcoma of the tongue and surgical reconstruction: About a rare case report.

Ann Chir Plast Esthet 2019 Feb 24;64(1):98-105. Epub 2018 Sep 24.

Institut Gustave-Roussy, department of plastic surgery, Gustave-Roussy, 114 rue Édouard-Vaillant, 94805 Villejuif, France.

Clear cell sarcomas (SCC), also called "soft-tissue melanoma", are rare and aggressive tumors that preferentially affect the lower limbs (tendons and fasciae) and which have also been described in head and neck localizations. Their clinical and immunohistochemical mimicry with melanoma makes it difficult to diagnose sarcomas. SCC treatment is mainly focused on large-scale resection surgery with adjuvant radiotherapy because of their low chemo-sensitivity and extreme lymphophilia. Read More

[Indications for breast reconstruction after mastectomy according to the oncological situation].

Ann Chir Plast Esthet 2018 Nov 20;63(5-6):580-584. Epub 2018 Sep 20.

Breast reconstruction is an integral part of breast cancer treatment. It must be systematically mentioned when the disease is announced and proposed at an optimal time during cancer management. In case of ductal carcinoma in situ, reconstruction should be proposed at the same time as mastectomy, which generally preserves the skin. Read More

A full range of breast reconstructions procedures exists and had to be proposed within a surgical team, from the simplest to the most complex. The choice of the technique is simply based on the discussion between the surgeon and the patient during a consultation. A computed tomographic angiography might be required in the case of DIEP flap reconstruction. Read More

The choice of an implant by a plastic surgeon is often based on marketing arguments, believes, and subjective reasons more than real high level evidence basis. Furthermore, we rarely compare manufacturers with each other objectively. Therefore our aim was to put together information coming from the different companies available in France in a single article. Read More

Centre du sein, 18, rue Pierre-et-Marie-Curie, 75005 Paris, France. Electronic address:

Breast reconstruction by prosthesis remains the most used in the world and even tends to increase again at the expense of musculo-cutaneous flaps since the systematic use, in recent years, of adipocytes grafts (lipofilling) before and/or after in place of the implant. This simpler technique is often preferred by patients who want to avoid scars and pain away from the chest area. The use of different implant forms, fat injection, abdominal advancement flaps, biological or synthetic matrices can significantly improve the results of these reconstructions in secondary or immediate. Perforator free flaps allow breast reconstructions « like with like » with skin and fat, excluding mammary gland, with a low morbidity. Those autologous reconstructions prevent material associated infections, capsular contracture and implant replacements, associated to breast reconstructions including implants. DIEP flap was described in 1994 to reduce the morbidity faced with TRAM flaps harvest. Read More

[Breast reconstruction with the autologous latissimus dorsi flap].

Ann Chir Plast Esthet 2018 Nov 28;63(5-6):422-436. Epub 2018 Aug 28.

Département de chirurgie plastique, centre Léon-Bérard, 28, rue Laennec, 69008 Lyon, France.

Amongst various techniques of breast reconstruction, Autologous Latissimus Dorsi (ALD) flap without breast implant is the newest technique that took advantage of recent improvement during the last decade. Surgical procedure is well standardized, and allows to harvest various fat areas attached to the muscle, to obtain an autologous reconstruction. Thoracodorsal pedicle is steady and makes ALD the most reliable flap that can be used in several indications, especially when DIEP or TRAM flap are inappropriate. Read More

Breast Lipomodeling, or Breast Lipofilling, consists in performing a graft of fatty tissue, from an area of the lower body, and moving it to the breast. This method, initially decried, has progressively taken a major place in breast reconstruction. Preoperative information is important: information must be given verbally and also by information forms from the French for French Society of Plastic, Reconstructive and Aesthetic Surgery (SOFCPRE) that can be downloaded from www.
