Spontaneous resolution of myopic retinoschisis.

AME Case Rep 2019 13;3. Epub 2019 Feb 13.

Ophthalmology Department, Hong Kong Adventist Hospital, Hong Kong, China.

Spontaneous resolution of myopic retinoschisis is an unusual event due to the fact that most eyes of patients with this condition have already undergone certain morphological changes and vision will progressively worsen over time. We present a case of spontaneous anatomic and visual improvement in a myopic eye with a macular retinoschisis. The patient was monocular oculus dextrus (OD) with a history of left eye enucleation for recurrent retinal detachment and glaucoma for 30 years prior. Read More

Papillary thyroid carcinoma with bilateral axillary lymph node involvement: a case report outlining hypothesis for locoregional spread and therapeutic implications.

AME Case Rep 2019 24;3. Epub 2019 Jan 24.

Department of Surgery, University of Massachusetts Memorial Medical Center, Worcester, MA, USA.

Axillary lymph node involvement (ALNI) in well differentiated papillary thyroid carcinoma (WDPTC) is a rare sequela of disease presentation. We report a case of PTC with extensive cervical lymph node involvement, local extension to the skin of the neck and bilateral ALNI without evidence of distant disease. We posit that ALNI represents local extension of disease rather than distant metastasis. Read More

Late implant migration with neurologic compromise as a complication of scoliosis surgery.

AME Case Rep 2019 16;3. Epub 2019 Jan 16.

Orthopaedic Department, University Hospital of Santa Maria, Lisbon, Portugal.

Late neurological deficit following scoliosis surgery is a rare event and any change in the patient's neurological status deserves immediate attention. We report on two clinical cases (at 2 and 6 years of follow-up) where two different types of proximal instrumentation resulted in lateral drift into the spinal canal causing cord compression with neurological deficit. Late neurological compromise in a posterior spinal-fusion setting deserves a prompt and complete spine investigative search for non-union, infection, or implant migration into the spinal canal. Use of image-guided bone scalpel for resection of spine tumors: technical note.

AME Case Rep 2018 29;2:48. Epub 2018 Nov 29.

Department of Neurosurgery, University of Rochester Medical Center, Rochester, NY, USA.

In the literature, the use of navigation for spine tumor surgery has largely centered on implant placement. We describe the cases of two patients with spinal tumors on whom we utilized our resection technique of registering an ultrasonic bone scalpel (UBS) to a navigation system. In both cases, we achieved a satisfactory tumor resection with negative margins and excellent neurologic outcomes. Read More

A case report of sphenoid sinusitis causing opsoclonus myoclonus syndrome.

AME Case Rep 2018 19;2:47. Epub 2018 Nov 19.

Department of ENT, John Radcliffe Hospital, Oxford, UK.

Opsoclonus myoclonus syndrome (OMS) is a rare neurological condition causing rapid jerking involuntary eye movements and myoclonus. The combination of opsoclonus and myoclonus have led to condition being coined the "dancing eyes, dancing feet syndrome". There are a wide variety of paraneoplastic and para-infectious aetiologies for OMS and therefore a detailed workup is needed as OMS symptoms can precede the commencement of symptoms from the underlying triggering disease process. Granulomatosis with polyangiitis causing subglottic stenosis-two cases and their management.

AME Case Rep 2018 30;2:17. Epub 2018 Apr 30.

Department of Otolaryngology, Pinderfields General Hospital, Wakefield, UK.

Granulomatosis with polyangiitis (GPA) is characterised by vasculitis of small and medium sized blood vessels and granulomatous lesions of the respiratory tract. The aetiology is unclear, however it is thought to be due to an autoimmune process with about 92% of patients with the disease being antineutrophilic cytoplasmic antibodies (ANCA) positive. Patients normally present in early adulthood, more commonly in the winter months. Read More

Open window thoracostomy as an alternative approach to secondarily infected malignant pleural effusion and failure of intrapleural catheter drainage: a case report.

AME Case Rep 2018 13;2:12. Epub 2018 Apr 13.

Department of General Surgery, MedStar-Georgetown University Hospital, Washington DC, USA.

Malignant pleural effusion (MPE) is a common and complex manifestation of advanced stage cancer. Treatment options have trended towards less invasive approaches such as intrapleural catheter drainage, however this technique is not without morbidity and not suitable for every patient. A troublesome scenario arises when an MPE is secondarily infected in the setting of an indwelling catheter, given both the high frequency of recurrence of such fluid and the presence of a foreign body. Rare isolated synchronous splenic metastasis in a patient with type II papillary renal cell carcinoma.

AME Case Rep 2018 14;2. Epub 2018 Mar 14.

Department of Urology, Tan Tock Seng Hospital, Singapore.

Metastatic disease is common in renal cell carcinoma (RCC), with a third of cases being synchronous. RCC is known to metastasize to any organ in the body; however, isolated splenic metastasis is extremely rare. We report a case of synchronous splenic metastasis from type II papillary RCC with 80% sarcomatoid change. Read More

Know the name: acute epiploic appendagitis-CT findings and review of literature.

AME Case Rep 2018 7;2. Epub 2018 Mar 7.

Department of Radiology, Yale New Haven Health System at Bridgeport Hospital, Bridgeport, CT, USA.

Epiploic appendages are normal pedunculated peritoneal fat containing outpouchings bordering tenia coli on the anti-mesenteric surface of the colon, extending from caecum to the rectosigmoid. Functions are currently unknown, though some postulate them a blood reservoir. The epiploic appendages can become inflamed, with clinical presentations mimicking that of diverticulitis or acute appendicitis. Read More

Inguinal hernia with ureter in patient with single kidney.

AME Case Rep 2018 9;2. Epub 2018 Feb 9.

Department of Urology, Medical School of São José do Rio Preto, São José do Rio Preto, São Paulo, Brazil.

Inguinal hernias are a common disease in adults, but involvement of extraperitoneal structures is rare. This condition is usually asymptomatic and an incidental finding during the surgical correction of inguinal hernias, often resulting in iatrogenic damage. We present a rare case of left renal agenesis and right inguinal hernia, with the ureter being the only organ protruding as an inguinal hernia, diagnosed in the preoperative period. Read More
