Diclofenac use and cardiovascular risks: series of nationwide cohort studies.

Diclofenac use and cardiovascular risks: series of nationwide cohort studies was published in The British Medical Journal in October 2018, finding that Painkillers like Voltarol and Voltaren may increase the risk of cardiovascular disease by as much as 50 percent.  The article caused quite a reaction, across dozens of news and social media outlets.

  • "Common painkiller may substantially increase the risk of heart disease" - ScienceNordic
  • "Diclofenac may be linked with an increased risk of heart problems"  - NewsHour

Do you think painkillers such as these should be more regulated?  Or is there something more to this?

Diclofenac is an NSAID and should be used with caution in older patients, and in patients with known CV risk factors. This is established medical guidance. Certain drugs in this class have been related to MIs in the past.  It would be interesting to know, in the BMJ article, the age range of the patients surveyed.  Perhaps a conclusion would be to use the drug with caution in certain populations, not to ban the drug from use completely.

Without reservations, all kinds of painkillers in each nation should be more controlled by the regulatory authorities. Besides, ISO too should get appended.

I have a degenerative disc disease since 2012. This disease is extremely painful, and I frequently use Diclofenac tablets and Voltaren to ease my pain. I used these pain killers 4-8 times within 24 hours. But through time I experienced cardiovascular problem when diagnosed. Since then I had a difficulty breathing, sleep problem, fast beat, continuous nightmare and feel tiredness. From my own experience, painkillers should controlled and physician must inform their patients clearly their side effects.

The NSAIDS have same problems but there are many options to choose from depending on their side effects.

However, in my practice prefer to substitute with Ibuprofen because is cheaper for my poor patients and has less cardiac effects compared to Diclofenac.

Painkillers are necessary. Especially in the musculo-skeleton conditions. From personal experience, I can say that paracetamol based interventions are often prescribed in the case of fractures and other kinds of chronic inflammations. In the cohort of NSAID drugs, nimesulide based medicines may be used. This kind of medications go ahead with their side effects. As far as diclofenac is concerned, it is no exception to the rule of having serious side effects. At least in long-term use, even paracetamol may prove to be injurious to liver, while nimesulide has already been marked as boax in America. Diclofenac has general side effects ranging from nausea and vertigo to chronic heart conditions when used for a long period of time. Consequently, findings of the research under discussion would help in understanding the limited use of painkillers in the context of diclofenac at least. Policy-understanding should be expounded increasingly at the level of medical associations so that doctors may use painkillers like diclofenac more cautiously.

  • Diclofenac is the most commonly used non-steroidal anti-inflammatory drug (NSAID) in low, middle, and high income countries. It should be used with caution especially for those at high risk of cardiovascular diseases

  • Its cardiovascular risks compared with other traditional NSAIDs have never been examined in a randomised controlled trial, and current concerns about these risks make such a trial unethical to conduct especially in resource limited settings like Cameroon from where i dwell
