I think it depends on dimensions and types of medical product which you are providing. Sometimes ServQual questionnaire would be helpful, otherwise you should use a Delphi Technique for estimating and quantifying components of medical products which you are evaluating. The primary section should be demographic items such as age, gender, etc.
