Short- and Long-Term Effects of a Multi-Component, Artificial Tear on Preocular Tear Film Stability, Tear Evaporation and Tear Film Optical Dynamic: A Prospective Randomized Double-Phase Study.

Authors:
Giancarlo Montani Linda Landini Maurizio Martino

Curr Eye Res 2022 Jul 19:1-7. Epub 2022 Jul 19.

Mathematics and Physics Department "Ennio De Giorgi", Salento University, Lecce, Italy.

Purpose: To evaluate the short- (within 90 min) and long-term (after 21 days of treatment) effects of an artificial tear containing carbomer, hyaluronic acid, glycerol, and medium-chain triglycerides in patients with mild-to-moderate dry eye symptoms.

Methods: Subjects received the tested artificial tears in the right eye and control artificial tears in the left eye and were assessed 10, 30, 60, and 90 min after instillation in the short-term study phase. They received the study product in both eyes in the long-term phase. Non-invasive break-up time (NIBUT), tear lipid layer pattern, tear evaporation, and tear film objective optical dynamics (TFOOD) were measured in controlled environmental conditions.

Results: In total, 32 patients (10 males, mean age 42 ± 11 years) were enrolled. During the short-term phase, NIBUT increased significantly in the right eyes at all time points compared with baseline (all  < 0.05). Conversely, in the left eyes, NIBUT increased significantly at 10 and 30 min ( < 0.05). The tear evaporation values increased at 10 min and decreased at 30 and 60 min ( < 0.05) in the right eyes, while they increased only at 10 min ( < 0.05) in the left eyes. Compared with baseline, the TFOOD was significantly more stable at all time points in the right eyes ( < 0.05), while it was more stable at 10 and 30 min in the left eyes ( < 0.05). In the long-term phase, treated eyes showed a significant increase in NIBUT values, a significant reduction of tear evaporation, a significant improvement of tear lipid layer pattern, and a more stable TFOOD compared with baseline (all  < 0.05). Symptoms, measured with the Ocular Surface Disease Index questionnaire, were also significantly reduced compared with baseline ( < 0.05).

Conclusion: The tested product increased tear film stability and reduced tear evaporation, normalizing lipid layer pattern and reducing symptoms in the short- and long-term observation.

[Chinese expert consensus on dry eye: lifestyle-related dry eye (2022)].

Authors:

Zhonghua Yan Ke Za Zhi 2022 Aug;58(8):573-583

Epidemiological studies have confirmed that the incidence of dry eye is closely related to lifestyles. The Expert Consensus on Dry Eye in China: Definition and Classification (2020) incorporated lifestyle-related factors into the classification of causes and risk factors for dry eye. In order to improve the diagnosis, treatment and prevention of this type of dry eye, the experts from the Chinese Branch of the Asian Dry eye Society and other scientific organizations have formed a consensus on the definition, classification, incidence, epidemiology, pathological mechanism, clinical characteristics, diagnosis, treatment and prevention of lifestyle-related dry eye. Meibomian gland dysfunction (MGD) is characterized by the obstruction and/or inflammation of the meibomian glands that result in decreased and altered meibum secretion. This results in deficiencies in the tear film lipid layer which contributes to increased evaporation and destabilization of the tear film. One of the mainstay therapies for MGD is medical devices that apply heat and/or pressure to the eyelids and promote the liquification and outflow of meibum into the tear film. Read More

Glove tear or perforation is a common occurrence during various activities that require gloves to be worn, posing a significant risk to the wearer and possibly others. This is vitally important in a clinical environment and particularly during surgical procedures. When a glove perforation occurs (and is noticed), the glove must be replaced as soon as possible; however, it is not always noticeable. Read More

Due to their antioxidant, anti-inflammatory, neuroprotective, and anti-angiogenic effects, polyphenols are first-rate candidates to prevent or treat chronic diseases in which oxidative stress-induced inflammation plays a role in disease pathogenesis. Dry eye disease (DED) is a common pathology, on which novel phenolic compound formulations have been tested as an adjuvant therapeutic approach. However, polyphenols are characterized by limited stability and solubility, insolubility in water, very rapid metabolism, and a very short half-life. Read More

Meibomian gland dysfunction (MGD), a chronic abnormality of meibomian glands, causes various dry eye symptoms. Principal treatments for MGD are warm compression and mechanical squeezing of the eyelids. In this study, the immediate impact of this treatment on tear film lipid layer thickness (TFLLT) and the meibomian gland (MG) structure in MGD and normal groups was investigated to establish its efficacy and potential side effects.
