Same-day mastectomy and axillary lymph node dissection is safe for most patients with breast cancer.

Authors:
Anselm Tamminen Tuomo Meretoja Ilkka Koskivuo

J Surg Oncol 2022 Apr 20;125(5):831-838. Epub 2022 Jan 20.

Department of Plastic and General Surgery, Turku University Hospital, University of Turku, Turku, Finland.

Background And Objective: The aim of this study was to evaluate the safety of same-day mastectomy, with or without a sentinel node biopsy (SNB) and/or axillary lymph node dissection (ALND).

Methods: In this retrospective study, we reviewed 913 consecutive women who underwent a simple mastectomy for breast cancer between the years 2014 and 2019 and were treated either with same-day surgery (SDS) or an overnight stay (OS) regime. We reviewed all surgical complications, any unplanned return to care (RTC) and the rehospitalization rate for 30 postoperative days.

Results: A total of 259 patients (28%) were treated with SDS and 654 patients (72%) with an OS regime. There was no difference in RTC (odds ratio: 0.79 [95% confidence interval: 0.53-1.18], p = 0.26) or any major complications between the groups. None of the investigated subgroups, such as patients with previous neoadjuvant therapy, diabetes, obesity (up to a body mass index of 40 kg/m ), the American Society of Anaesthesiologist Class of 3, or elderly patients aged 75-84 years, showed an increased complication rate when treated with the SDS regime.

Conclusion: A same-day simple mastectomy is safe with SNB and/or ALND. It can be performed safely for most patients with stable co-morbidities.

Cureus 2022 Apr 20;14(4):e24317. Epub 2022 Apr 20.

Oncology, Karmanos Cancer Institute, Detroit, USA.

Background:  The American College of Surgeons Oncology Group (ACOSOG) Z0011 trial demonstrated that sentinel lymph node biopsy (SLNB) alone is adequate for axillary control in patients with one to two positive axillary lymph nodes. However, axillary lymph node dissection (ALND) is required in patients with N1 disease diagnosed with a preoperative needle biopsy. In this report, we determined how many patients could potentially have had SNB alone based on finding only one to two positive nodes in the axilla. Read More

In humid and temperate areas, Septoria nodorum blotch (SNB) is a major fungal disease of common wheat (Triticum aestivum L.) in which grain yield is reduced when the pathogen, Parastagonospora nodorum, infects leaves and glumes during grain filling. Foliar SNB susceptibility may be associated with sensitivity to P. Read More

Purpose: To evaluate the clinical effect of combined orthodontic and orthognathic treatment with invisible aligner technique without brackets in skeletal Class Ⅲ patients with facial asymmetry.

Methods: A total of 24 skeletal Class Ⅲ patients with facial asymmetry treated with combined orthodontic and orthognathic treatment during the past 4 years were reviewed. Patients in the experimental group(n=12) were treated with invisible aligner technique without brackets, while patients in the control group(n=12) were treated with traditional fixed orthodontic technique for pre- and post-operative orthodontic treatment respectively. Scale effects on efficiency and profitability in the Swiss banking sector.

Authors:
Marc Blatter Andreas Fuster

Swiss J Econ Stat 2022 9;158(1):12. Epub 2022 May 9.

EPFL, Swiss Finance Institute and CEPR, Lausanne, Switzerland.

This paper analyzes efficiency and profitability in the Swiss banking sector over the period 1997-2019. We find strong evidence for scale economies: for most banks in the sample, efficiency and profitability increase with bank size. Using an instrumental variables strategy for a subset of geographically restrained banks, we find that the effect of size on efficiency and profitability is likely causal. Read More
