Reply to HEP-21-0216 "Acute Liver Injury due to Therapeutic Doses of Acetaminophen - Confounders must be ruled out!"

Authors:
Alexandre Louvet Philippe Mathurin

Hepatology 2021 Feb 27. Epub 2021 Feb 27.

Hôpital Claude Huriez, Service d'Hépato-gastroentérologie, France.

We disagree with the conclusion of Vojjala et al., which states that clinicians should pay attention to metabolic risk factors, fatty liver and concurrent drug use. The authors questioned the presence of NAFLD in our study. However, the median BMI was normal in the patients admitted with ALITD and was not different from that of patients admitted with acetaminophen overdose: 21.9 vs. 22 kg/m , p=0.5. Therefore, the prevalence of metabolic syndrome was a rare event. Moreover, previous studies have not identified any relationship between BMI and the outcome or pattern of intoxication(1). Obesity was not found to have any impact on acetaminophen toxicity in the study by Radosevich et al.(2).

Aim: To summarize our experience on the implementation of a telemedicine service dedicated to adult congenital heart disease (ACHD) patients during the lockdown for the first wave of COVID-19.

Methods: This is a prospective study enrolling all ACHD patients who answered a questionnaire dedicated telematic cardiovascular examination.

Results: A total of 289 patients were enrolled, 133 (47%) were male, 25 (9%) were affected by a genetic syndrome. Read More

Fluid Responsiveness Predictability in Immediate Postoperative Pediatric Cardiac Surgery. Is the Old Slandered Central Venous Pressure Back Again?

Authors:

Shock 2021 Apr 20. Epub 2021 Apr 20.

Pediatric Cardiac Intensive Care Unit, Schneider Children's Medical Center of Israel, Petach Tikva, Israel, 4920235 Sackler Faculty of Medicine, Tel Aviv University, Tel Aviv, Israel, 6997801.

Objective: Acute low cardiac output (CO) is a frequent scenario in pediatric cardiac intensive care units (PCICU). While fluid responsiveness has been studied extensively, literature is scarce for the immediate postoperative congenital heart surgery population admitted to PCICUs. This study analyzed the utility of hemodynamic, bedside ultrasound and Doppler parameters for prediction of fluid responsiveness in infants and neonates in the immediate postoperative cardiac surgery period. Nephron 2021 Apr 21:1-10. Epub 2021 Apr 21.

Division of Nephrology, Kidney Institute of CPLA, Changzheng Hospital, Second Military Medical University, Shanghai, China.

Objectives: The determinants leading to different renal outcomes in community-acquired acute kidney injury (CA-AKI) and the influence of renal histological damage on the prognosis and recovery of CA-AKI are scarcely reported.

Methods: Adult patients with CA-AKI admitted to Shanghai Changzheng Hospital with renal biopsy profiles from January 1, 2010, to December 31, 2018, were enrolled in our cohort. After 3 months of follow-up, clinical outcomes, including patient survival, dialysis requirement during hospitalization and at 3 months, CKD stage 3-5, and renal functional recovery at 3 months, were analyzed, and risk factors were identified. R. China.

Objectives: Currently there is no validated method to predict renal reversal and recovery after acute kidney injury (AKI). As exosomes have the potential for AKI prognosis and CD26 is involved in the mechanisms in AKI, this study aims to investigate whether urinary exosomal CD26 is associated with renal-related outcomes and explore its prospect as a novel prognosis biomarker.

Methods: This was a single-center, prospective cohort study. Read More

Objectives: COVID-19 pandemic significantly impacted the diagnosis of type 1 diabetes and its acute complications. Thus, the study aimed to evaluate the characteristics of pediatric patients with type 1 diabetes hospitalized during the first wave of the pandemic and the prevalence of new onset diabetes among patients with evidence of COVID-19 infection.

Methods: A single-center surveillance study included all patients with diabetes admitted to Children's Hospital, Ain Shams University, in Egypt between May to August 2020. Read More
