There are some controversies about the surgical treatment strategy of mirror aneurysms. Whether to take one-stage or two-stages surgery, bilateral or unilateral craniotomy, surgical or interventional treatment is the main points in dispute. The present review mainly introduces the different surgery strategies facing with different patients. Different surgical methods based on the patient's own state, the location and size of the aneurysm are adopted. And this article introduces a new imaging method using 3D-slicer, which can clearly recogonize the relationship between aneurysm, brain tissue, skull and nerve. The 3D-slicer can help surgeons do adequate preoperative preparation. In addition, we also introduce some ruptured factors (age, gender, hypertension, morphological, hemodynamic, etc.) about mirror aneurysm. The systematic introduction about the controversies and methods in surgical treatment strategy of mirror aneurysms may provide new perspectives for the prevention and treatment of mirror aneurysms in the future research.
