Spontaneous Resolution of Racemic Cage-Catenanes via Diastereomeric Enrichment at the Molecular Level and Subsequent Narcissistic Self-Sorting at the Supramolecular Level.

The spontaneous resolution of racemates, from natural compounds to artificial structures, has long been pursued to shed light on the origin of homochirality in life. Even though diverse synthetic systems have been elegantly devised to elaborate the underlying principles of spontaneous symmetry breaking, their complexity is still unparalleled to the natural masterpieces including DNA helix and proteins, which convey remarkable coalescence at both molecular and supramolecular levels. Here, we report on the spontaneous resolution of a pair of homochiral entities from a racemic mixture of a triply interlocked cage-catenane comprising 720 possible stereoisomers. This cage-catenane comprises six methyldithiane ring-containing linkers (denoted -). As each methyldithiane ring has two chiral centers, it exhibits four possible diastereomers. These otherwise equimolar diastereomers are preferentially differentiated with the conformers over their analogues, leading to the dominant formation of (, )- and (, )-, i.e., . This diastereomeric enrichment is unbiasedly transferred from precursor - to cage-catenane -, from which a pair of homochirals (, )- and (, )- is narcissistically self-sorted upon crystallization. This powerful symmetry breaking is attributed to a of directional π-π stacking with the multivalency of erstwhile weak S···S contacts () that are cooperatively arranged in a helical fashion. This work highlights the attainability of complex homochiral entities by resorting to coalesced covalent and noncovalent contributions and therefore provides additional clues to the symmetry breaking of sophisticated yet well-defined architectures.

Leaf and Flower Extracts of Six Michelia L.: Polyphenolic Composition, Antioxidant, Antibacterial Activities and in Vitro Inhibition of α-Amylase and α-Glucosidase.

Methanolic extracts of the leaf and flower of Michelia L., an evergreen aromatic genus widely used in landscaping, industry and medicine of various countries, were analyzed. The UPLC-ESI-MS/MS analysis led to the identification of 28 polyphenols from six Michelia species that widely distributed and cultivated in southern China, among which quinic acid and chlorogenic acid were the main components. The flower extract of Michelia maudiae had the most abundant polyphenols content, as well as high contents of total phenolic (117.31 ± 7.26 mg GAE/g DW) and total flavonoid (251.60 ± 15.56 mg CE/g DW). Meanwhile, it also showed outstanding performance in three antioxidant indexes of DPPH, ABTS and FRAP. The leaf extracts of Michelia chapensis and Michelia floribunda exhibited excellent inhibition against four pathogenic bacteria. Moreover, certain inhibitory activities were displayed by Michelia macclurei extracts against α-amylase and α-glucosidase This study explored the biological activities of six Michelia species, and provided reference for variety selection with the aim of designing novel phyto-pharmaceuticals.

Associations between Feeding Patterns and Infant Health in China: A Propensity Score Matching Approach.

Breastmilk is the optimal food for infants. Feeding pattern is closely related to physical development and health during infancy. Understanding the associations between feeding patterns and health status can inform related policy interventions and advocacy in China. This study aimed to investigate the relationship between infant feeding patterns and health status in China infants. The China National Nutrition and Health Surveillance 2013 was a national-representative cross-sectional study performed particularly for children aged 0-5 years. A total of 3974 infants aged under 1 year were included in the analysis, of whom 1082 (27.2%) made up the formula feeding group, and 2892 (72.8%) made up the breastfeeding group. The associations between feeding patterns and physical development and health were investigated using propensity score matching and multivariable logistic regression models. Among breastfeeding and formula feeding infants aged 9-11 months old, weight-for-age z score was 1.1 ± 1.1 and 0.9 ± 1.3, respectively, and weight-for-length z score was 1.0 ± 1.3 and 0.7 ± 1.4, respectively. Hemoglobin in 0-2, 3-5, 6-8, and 9-11 months old breastfeeding infants was 121.4 ± 15.2 g/L, 117.1 ± 13.0 g/L, 113. 9 ± 11.9 g/L, and 114.4 ± 14.0 g/L, while in 0-2, 3-5, 6-8, and 9-11 months formula feeding infants was 116.3 ± 14.8 g/L, 120.4 ± 11.3 g/L, 119.8 ± 11.2 g/L, and 120.0 ± 11.5 g/L, respectively. Breastfeeding was associated with lower risk of respiratory disease (OR: 0.79; 95% CI: 0.64, 0.99) and diarrhea (OR: 0.75; 95% CI: 0.57, 0.98). Breastfeeding could slightly improve infant physical development, and had a protective effect on the diarrheal and respiratory diseases. Infants aged 3-11 months who were breastfeeding showed lower hemoglobin than that of formula-fed infants and thus should increase intake of iron rich complementary foods.

[Association between timing of introducing complementary foods and anemia of infants aged 6-11 months in four provinces of China].

Objective: To analyze the anemia status of infants aged 6-11 months in Beijing, Shanxi, Jiangxi and Zhejiang of China, and to explore the association between timing of introducing complementary foods and hemoglobin level, anemia of infants aged 6-11 months.

Methods: Data was from National Nutrition and Health Systematic Survey for 0-18 Years Old Children in China. A total of 1404 infants aged 6-11 months from Beijing, Shanxi, Jiangxi and Zhejiang were enrolled in this study. Demographic characteristics, socioeconomic status, birth status and complementary feeding information were collected through questionnaire survey. HemoCue Hb201+ hemoglobin analyzer was used to measure hemoglobin value. The exposure variables in this study were timing of introducing complementary foods(≤5 months, 6 months and ≥7 months), and the outcome variables were hemoglobin level and anemia rate. The association between timing of introducing complementary foods and hemoglobin level was analyzed by using multivariate linear regression model, and the association between timing of introducing complementary foods and anemia rate was analyzed by using multivariate Logistic regression model.

Results: The hemoglobin levels of infants aged 6-11 months were(114.8±11.0)g/L, (115.5±10.5)g/L in urban areas and(114.1±11.5) g/L in rural areas. The anemia rate was 28.2%, 24.0% in urban areas and 32.9% in rural areas. The hemoglobin levels of infants introducing complementary foods at ≤5 months, 6 months and ≥7 months were(114.0±11.1), (115.2±10.9) and(114.5±10.7) g/L, respectively. After adjusting for potential confounding factors, there was no significant difference in hemoglobin level between the ≤5 months group and 6 months group(F=2.37, P=0.124), and no significant difference between the ≥ 7 months group and the 6 months group(F=0.09, P=0.770). The anemia rate of infants introducing complementary foods at ≤5 months, 6 months and ≥7 months were 32.3%, 27.9% and 22.7%, respectively. After adjusting for potential confounding factors, there was no significant difference in anemia rate between the ≤5 months group and 6 months group(OR=1.26(95%CI 0.86-1.83)), and no significant difference between the ≥7 months group and the 6 months group(OR=0.65(95%CI 0.35-1.20)).

Conclusion: Anemia remains a serious problem for infants aged 6-11 months in Beijing, Shanxi, Jiangxi and Zhejiang. Timing of introducing complementary foods may not be related with hemoglobin level and anemia rate of infants aged 6-11 months.

[Association between timing for introducing complementary foods and body composition of children aged 3-5 years in five provinces of China].

Objective: To explore the association between different timing of introducing complementary foods during infancy and the body composition of children aged 3-5 years.

Methods: The data was from National Nutrition and Health Systematic Survey for 0-18 Years Old Children in China. A face-to-face interview to caregivers of children was conducted to retrospectively collect timing of introducing complementary foods of children aged 3-5 years. The bioelectrical impedance analysis was used to measure the body composition of children by professionally trained investigators. The exposure variable of the study was the timing of introducing complementary foods(≤5 months old, 6 months old and ≥7 months old). The outcome variables were the fat-free mass, fat-free mass index, body fat mass, body fat mass index, and percentage of body fat. According to age stratification, a multivariate linear regression model was used to calculate the mean, mean difference and 95% CI after adjusting for potential confounders.

Results: After adjusting for potential confounders, there were no statistically significant difference in body fat mass, body fat mass index, percentage of body fat and fat-free mass index among different groups of timing for introducing complementary foods for children aged 3-5 years old. For children aged 3-5 years, after adjusting for potential confounders, the difference of fat free mass was statistically significant among children with different timing for introducing of complementary foods(3 years old: 12.89 kg vs. 12.66 kg vs. 12.94 kg, F=4.42, P=0.013; 5 years old: 16.27 kg vs. 16.23 kg vs. 16.66 kg, F=5.40, P=0.005). The fat free mass of children aged 3 years was lower when introducing complementary foods during 6 months of age than at ≤5 months of age(mean difference=-0.23 kg(95% CI-0.45--0.02 kg)). The fat free mass of children aged 5 years was higher when introducing complementary foods at ≥7 months of age than at≤5 months of age(mean difference=0.39 kg(95% CI 0.08-0.70 kg)).

Conclusion: It is found that the association between timing for introducing complementary foods during infancy and the fat mass indicators of children aged 3 to 5 years was not statistically significant, and the association between timing for introducing complementary foods and fat-free mass may be inconsistent for different fat-free mass indicators.

[Status of infant and young child feeding using World Health Organization 2021 indicators in China].

Objective: To describe the feeding status of infant and young child aged 6-23 months in China.

Methods: Data was from the China Nutrition and Health Surveillance among 0-5 Years Old Children and Lactating Women in 2013.Stratified multistage cluster sampling method was used, and 9983 children aged 6-23 months were involved. We analyzed the infant and young child feeding practices with World Health Organization(WHO) 2021 updated indicators. Data analyses were conducted using the complex weight based on national census from National Bureau of Statistics in 2010.We used Rao-Scott Chi-square test for statistical difference.

Results: The proportions of children aged 6-23 months meeting minimum dietary diversity(MDD) was 34.5%(95% CI 28.8%-40.2%); it was over 50% in large cities, and only 20.4% in poor rural areas. The proportions of children aged 6-23 months meeting minimum meal frequency(MMF) was 69.2%(95% CI 64.7%-73.7%); it was about 80% in large cities, medium and small cities, and only 45.3% in poor rural areas. The proportions of children aged 6-23 months meeting minimum acceptable diet(MAD)was 23.7%(95% CI 19.2%-28.2%), it was 44.2% in large cities, and less than 10% in poor rural areas. No consistent differences were observed between boys and girls for all 3 dietary indicators(MDD, MMF and MAD), and there was significant difference in different areas and various months of age(P<0.05).

Conclusion: The minimum dietary diversity and minimum acceptable diet among children aged 6-23 month were not optimal in China, especially in poor rural areas.

This review reveals the potential therapeutic role of in human diseases. Patients with inflammatory bowel disease exhibit significant changes in abundance, and they may benefit a lot from modulations targeting . The data reviewed here demonstrate that plays its role in regulating barrier homeostasis, immune cells, and cytokine release through its metabolite butyrate, flagellin and other. Recent advancements in the application of primary culture technology, culture omics, single-cell sequencing, and metabonomics technology have improved research on and revealed the benefits of this bacterium in human health and disease treatment.

Lobectomy with pulmonary artery angioplasty for lung cancer using video-assisted thoracic surgery versus open thoracotomy: a retrospective propensity matched analysis.

Background: Pulmonary artery angioplasty (PAA) is an important surgical procedure to complete radical resection with maximum preservation of functioning pulmonary tissues when the pulmonary artery is involved. In this study, we aimed to evaluate the short- and long-term outcomes of PAA using video-assisted thoracic surgery (VATS) versus open thoracotomy (OT) in lung cancer surgery.

Methods: We retrospectively reviewed 214 patients who underwent PAA between November 2005 and October 2016. Propensity score matching (PSM) was applied to reduce confounding effects. The survival outcomes were assessed using Kaplan-Meier estimates and Cox regression analysis.

Results: The final sample included 203 patients (28 patients in the VATS group and 175 patients in the OT group). There were no differences between the two groups in operative time, blood loss, duration of chest tube drainage, postoperative hospital stays, margin status, postoperative morbidity and mortality, and number of N1 and N2 stations or number of N1 and N2 lymph nodes both before and after matching. At a median follow-up period of 43 (range, 6 to 158) months, the 5-year overall survival (OS) and recurrence-free survival (RFS) of the overall cohort were 47.9% and 42.1%, respectively. The 5-year OS and RFS were comparable between the VATS and OT groups both in the overall cohort and the matched cohort. The VATS procedure was found not to have a prognostic impact on either OS (hazard ratio, 1.17; 95% CI: 0.60 to 2.30, P=0.647) or RFS (hazard ratio, 1.14; 95% CI: 0.62 to 2.10, P=0.666).

Conclusions: VATS PAA is associated with comparable short- and long-term outcomes in selected patients with lung cancer compared with OT.

Relationship between Serum 25OH-Vitamin D2 Level and Vitamin D Status of Children Aged 3-5 Years in China.

Background: Vitamin D deficiency is prevalent globally and there is lack of evidence as to how 25(OH)D2 contributes to vitamin D status. The aim of this study was to describe vitamin D status and to assess the role of vitamin D2, a dietary vitamin D source, against the vitamin D status of children aged 3-5 years in China.

Methods: Data were extracted from the Chinese National Nutrition and Health Surveillance (CNNHS) in 2013. The concentration of serum 25(OH)D2 and 25(OH)D3 was measured by using LC-MS/MS.

Results: A total of 1435 subjects were enrolled and serum 25(OH)D were analyzed. The prevalence of total serum 25(OH)D < 30 nmol/L was 8.9%. Serum 25(OH)D2 was detected in 10.9% of the studied children. After adjusting for confounding factors, total 25(OH)D concentration was 8.48 nmol/L lower and odds ratio of vitamin D deficiency was 4.20 times (OR (95%CI): 4.20 (1.64, 10.77)) in children without 25(OH)D2 than those with 25(OH)D2 detected.

Conclusions: Vitamin D deficiency was common among children aged 3-5 years in China. Vitamin D2 may play a role in preventing vitamin D deficiency in Chinese children aged 3-5 years.

Research on Curing Water Demand of Cementing Material System Based on Hydration Characteristics.

The performance of cover concrete is acknowledged as a major factor governing the degradation of concrete structures. Curing plays a vital role in the development of concrete durability. The effects of different water-binder ratios and mineral admixtures on the curing water demand of concrete were studied by the surface water absorption test. Combined with the characteristics of the hydration heat and chemically bound water of the composition cementing material system, the law of variation for curing water demand was analyzed. The results show that the addition of mineral admixtures can reduce the early hydration rate and hydration exothermic characteristics, and the hydration degree decreases with the increase of mineral admixtures. Due to the filling effect and active effect, the addition of fly ash (FA) and ground granulated blast slag (GGBS) reduces the curing water demand. The curing water demand of cover concrete decreases with the increase of mineral admixture content, and the curing water demand of pure water is the maximum and that of mix FA and GGBS is the minimum. Moreover, there is a strong correlation between the cumulative curing water demand and the chemically bound water content, indicating that the power of water migration mainly comes from the hydration activity of the cementing material system. The results provide a theoretical basis for the fine control of a concrete curing system.

Interfacial characteristics and in vitro digestion of emulsion coated by single or mixed natural emulsifiers: lecithin and/or rice glutelin hydrolysates.

Background: The interfacial characteristics and in vitro digestion of emulsion were related to emulsifier type. The mean droplet diameter, ζ-potential, microstructure, interfacial tension, Quartz crystal microbalance with dissipation (QCM-D) and in vitro gastrointestinal fate of emulsions stabilized by soybean lecithin, hydrolyzed rice glutelin (HRG) and their mixture were researched.

Results: The value of interfacial tension was much more dramatically declined for the sample containing 20 g kg of HRG. For QCM-D, a rigid layer was formed for all the samples after rinsing. The layer thickness was 0.87 ± 0.20, 2.11 ± 0.31 and 2.63 ± 0.22 nm, and adsorbed mass was 87.17 ± 10.31, 210.56 ± 20.12 and 263.09 ± 23.23 ng cm , for HRG, lecithin and HRG/lecithin, respectively, indicating both HRG and lecithin were adsorbed at the oil-water interface. Structural rearrangements at the interface occurred for HRG/lecithin. The kinetics and final amount of lipid digestion depended on emulsifier type: lecithin > HRG/lecithin > HRG. These differences in digestion rate were primarily due to differences in the aggregation state of the emulsifiers.

Conclusion: The incorporation of lecithin into HRG emulsions had better interfacial properties comparing with HRG emulsion and facilitated lipid digestibility. These results provide important information for the rational design of plant-based functional food. © 2021 Society of Chemical Industry.

[Prevalence and risk factors for anemia of Chinese lactating women in 2013].

Objective: To investigate the prevalence of anemia and its risk factors for lactating women in China.

Methods: Data was extracted from Chinese National Nutrition and Health Surveillance-Mothers for children under 2 years of age(2013). Totally, 11 178 mothers of children under 2 years old were selected from 55 counties of 30 provinces in China by using the multi-stage stratified cluster randomization sampling method. Among these women 7129 are lactating. The basic information of lactating women was collected through the questionnaire survey. Hemoglobin concentration was measured by photometric determination of haemiglobincyanide(HiCN) method. Participants without blood sample and basic information were excluded. Finally, 6255 lactating women were included in the analysis. Complex sampling method with weighted analysis was used.

Results: Totally, with a complex sampling method with weighted analysis, the prevalence of anemia for lactating women was 9.3%. The mean hemoglobin level was(136.1±0.9) g/L. The mild and moderate anemia accounted for 98.8% of all anemic cases. Compared with those lactating time<6 months, lactating time between 6 to 12 months had a lower risk of anemia(OR=0.74, 95%CI 0.61-0.90). Compared with those living in large cities, the maternal living in poor rural areas had a higher risk of anemia(OR=1.69, 95%CI 1.29-2.20).

Conclusion: In 2013, the prevalence of anemia was mild for the lactating women in China. Lactating women in poor rural areas, within 6 months after deliveryhad the higher risk for anemia.

Comparison of severe and critical COVID-19 patients imported from Russia with and without influenza A infection in Heilongjiang Province: a retrospective study.

Background: The rapid spread of coronavirus disease-19 (COVID-19) poses a global health emergency, and cases entering China from Russia are quite diverse. This study explored and compared the clinical characteristics and outcomes of severe and critically ill COVID-19 patients from Russia with and without influenza A infection, treated in a northern Chinese hospital (Russia imported patients).

Methods: A total of 32 severe and critically ill Russia-imported COVID-19 patients treated in the Heilongjiang Imported Severe and Critical COVID-19 Treatment Center from April 6 to May 11, 2020 were included, including 8 cases (group A) with and 24 cases (group B) without influenza A infection. The clinical characteristics of each group were compared, including prolonged hospital stay, duration of oxygen therapy, time from onset to a negative SARS-CoV-2 qRT-PCR RNA (T) result, and duration of bacterial infection.

Results: The results showed that blood group, PaO/FiO, prothrombin time (PT), prothrombin activity (PTA), computed tomography (CT) score, hospital stay, duration of oxygenation therapy, T, and duration of bacterial infection were statistically different between the two groups (P<0.05). Multivariant regression analysis showed that the Sequential Organ Failure Assessment (SOFA) score, C-reactive protein (CRP), and influenza A infection were factors influencing hospital stay; SOFA score, CRP, and CT score were factors influencing the duration of oxygenation therapy; PaO/FiO, platelet count (PLT), and CRP were factors influencing T; and gender, SOFA score, and influenza A infection were factors influencing the duration of bacterial infection.

Conclusions: Influenza A infection is common in Russia-imported COVID-19 patients, which can prolong the hospital stay and duration of bacterial infection. Routinely screening and treating influenza A should be conducted early in such patients.

Co-doping optimized hydrogel-elastomer micro-actuators for versatile biomimetic motions.

Hydrogels can respond to changes in humidity or temperature, while elastomers can resist structural collapse due to dehydration or external force application. A hybrid bilayer of hydrogel-elastomers while retaining the merits of both the hydrogels and elastomers has emerged as a promising stimuli-responsive micro-actuator. However, the preparation of a hydrogel-elastomer micro-actuator requires multiple steps, mainly due to the differences in the surface properties of these two materials. Among them, the steps to surface-treat the elastomer and functionalize the material of each layer involve intricate processes and excessive consumption of resources. In this work, we introduce a co-doping method to optimize the preparation of a stimuli-responsive hydrogel-elastomer micro-actuator. The surface treatment and functionalization processes are combined into one step by directly doping the polymerization initiator and functional nanomaterials into the hybrid bilayer. The thermo-responsive hydrogel is combined with a photothermal elastomer to fabricate a soft micro-actuator that can bend and unbend in response to changes in humidity and light. Based on this actuator, a set of biomimetic soft micro-robots were developed, demonstrating a series of motions, such as grabbing, crawling, and jumping. This strategy of stimuli-responsive micro-actuator preparation can benefit the hydrogel-elastomer hybrid micro-robot designs for applications ranging from self-locomotive robots in environmental monitoring to drug delivery in biomedical engineering.

Analysis and verification of -methyladenosine-modified genes as novel biomarkers for clear cell renal cell carcinoma.

-methyladenosine (mA) has been involved in diverse biological processes in cancer, but its function and clinical value in clear cell renal cell carcinoma (ccRCC) remain largely unknown. In this study, we found that 1453 mA-modified differentially expressed genes (DEGs) of ccRCC were mainly enriched in cell cycle, PI3K-AKT, and p53 signaling pathways. Then we constructed a co-expression network of the 1453 mA-modified DEGs and identified a most clinically relevant module, where NUF2, CDCA3, CKAP2L, KIF14, and ASPM were hub genes. NUF2, CDCA3, and KIF14 could combine with a major RNA mA methyltransferase METTL14, serving as biomarkers for ccRCC. Real-time quantitative PCR assay confirmed that NUF2, CDCA3, and KIF14 were highly expressed in ccRCC cell lines and ccRCC tissues. Furthermore, these three genes were modified by mA and negatively regulated by METTL14. RTJTN: Relational Triplet Joint Tagging Network for Joint Entity and Relation Extraction.

Extracting entities and relations from unstructured sentences is one of the most concerned tasks in the field of natural language processing. However, most existing works process entity and relation information in a certain order and suffer from the error iteration. In this paper, we introduce a relational triplet joint tagging network (RTJTN), which is divided into joint entities and relations tagging layer and relational triplet judgment layer. In the joint tagging layer, instead of extracting entity and relation separately, we propose a tagging method that allows the model to simultaneously extract entities and relations in unstructured sentences to prevent the error iteration; and, in order to solve the relation overlapping problem, we propose a relational triplet judgment network to judge the correct triples among the group of triples with the same relation in a sentence. In the experiment, we evaluate our network on the English public dataset NYT and the Chinese public datasets DuIE 2.0 and CMED. The F1 score of our model is improved by 1.1, 6.0, and 5.1 compared to the best baseline model on NYT, DuIE 2.0, and CMED datasets, respectively. In-depth analysis of the model's performance on overlapping problems and sentence complexity problems shows that our model has different gains in all cases.

A class of organic cages featuring twin cavities.

A variety of organic cages with different geometries have been developed during the last decade, most of them exhibiting a single cavity. In contrast, the number of organic cages featuring a pair of cavities remains scarce. These structures may pave the way towards novel porous materials with emergent properties and functions.We herein report on rational design of a three-dimensional hexaformyl precursor 1, which exhibits two types of conformers, i.e. Conformer-1 and -2, with different cleft positions and sizes. Aided by molecular dynamics simulations, we select two triamino conformation capturers (denoted CC). Small-sized CC-1 selectively capture Conformer-1 by matching its cleft size, while the large-sized CC-2 is able to match and capture both conformers. This strategy allows the formation of three compounds with twin cavities, which we coin diphane. The self-assembly of diphane units results in superstructures with tunable proton conductivity, which reaches up to 1.37×10 S cm.

Lipid goal attainment in post-acute coronary syndrome patients in China: Results from the 6-month real-world dyslipidemia international study II.

Background: Dyslipidemia International Study II (DYSIS II)-China was conducted to determine the low-density lipoprotein cholesterol (LDL-C) goal (<1.8 mmol/L) attainment rate in patients with post-acute coronary syndrome (ACS).

Hypothesis: Compliance with treatment guideline recommendations improves the LDL-C goal attainment rate in post-ACS patients.

Methods: This multicenter prospective observational study conducted at 28 tertiary hospitals determined the LDL-C goal attainment rates at admission and 6-month follow-up in patients on lipid-lowering treatment (LLT) for ≥3 months and those not on LLT (LLT-naive or off LLT for ≥3 months) at admission. Predictors of goal attainment at 6 months were identified using multivariate logistic regression.

Results: The LDL-C goal attainment rate at admission in 1102/1103 enrolled patients was 17.1%; it was 41.2% among 752 patients with available lipid results at 6 months. The distance to goal was 0.7 mmol/L at 6 months. Statin monotherapy was the most prescribed LLT. Only 7.7% of patients were receiving statin + ezetimibe and 8.4% of patients were receiving an atorvastatin-equivalent dose of ≥40 mg/day at 6 months. Being male (odds ratio [OR] 1.7, 95% confidence interval [CI] 1.1-2.6) and undergoing percutaneous coronary intervention during index hospitalization (OR 1.5, 95% CI 1.1 to 2.1) were the independent predictors for LDL-C goal attainment.

Conclusions: This real-world DYSIS II study in China reports a low LDL-C goal attainment rate in post-ACS patients even after 6 months of LLT. Lack of intensification of statin therapy and underutilization of combinations suggest gaps between real-world treatment practices and guideline recommendations.

Comparison of Perioperative Outcomes Between Laparoscopic and Open Partial Nephrectomy for Different Complexity Renal Cell Carcinoma Based on the R.E.N.A.L. Nephrometry Score.

Purpose: This study aimed to compare perioperative outcomes of open partial nephrectomy (OPN) and laparoscopic partial nephrectomy (LPN) from a retrospective single-center dataset.

Patients And Methods: A retrospective single-center analysis of 210 patients who underwent open (n=91) or laparoscopic (n =119) partial nephrectomy for RCC was conducted between 2012 and 2015. All patients were grouped into low complexity, moderate complexity, and high complexity according to the R.E.N.A.L. Nephrometry Score, respectively. The rates of intraoperative and postoperative complications estimated blood loss, warm ischemia time, operative time, conversion of laparoscopic procedure to open surgery, and postoperative length of stay were assessed for both procedures.

Results: In low complexity group (n=93), patients undergoing LPN (n=52) under ischemia conditions had significantly longer renal artery clamp time (p < 0.001) and operative time (p = 0.001) compared with OPN (n=41). However, patients undergoing LPN had a significantly less postoperative length of stay (p=0.005) and estimated blood loss (p < 0.001) compared with OPN. There was no statistically significant difference in the rates of complications between LPN and OPN. In the moderate complexity group (n=114), 67 and 47 patients underwent LPN and OPN, respectively. LPN had notably longer warm ischemia time (p < 0.001) and operative time (p < 0.001) compared with OPN. There were no statistically significant differences in the rates of complications, estimated blood loss, and postoperative length of stay between LPN and OPN. In the high complexity group (n=3), all patients underwent OPN.

Conclusion: OPN and LPN procedures performed in patients with low and moderate complexity tumors based on the RENAL Nephrometry score offer acceptable and comparable results. When applied to low complexity tumors, our data suggest that laparoscopic NSS is an effective, minimally invasive therapeutic approach with the advantages of less blood loss, earlier hospital discharge, and more rapid convalescence.

Deep Inguinal Lymph Node Metastases Can Predict Pelvic Lymph Node Metastases and Prognosis in Penile Squamous Cell Carcinoma.

Objectives: To evaluate the relationship between deep inguinal lymph node metastasis (ILNM) and pelvic lymph node metastasis (PLNM) and explore the prognostic value of deep ILNM in penile squamous cell carcinoma (PSCC).

Materials And Methods: The records of 189 patients with ILNM treated for PSCC were analysed retrospectively. Logistic regression models were used to test for predictors of PLNM. Cox regression was performed in univariable and multivariable analyses of cancer-specific survival (CSS). CSS was compared using Kaplan-Meier analyses and log rank tests.

Results: PLNM were observed in 53 cases (28.0%). According to logistic regression models, only deep ILNM (OR 9.72, p<0.001) and number (≥3) of metastatic inguinal lymph nodes (ILNs) (OR 2.36, p=0.03) were independent predictors of PLNM. The incidences of PLNM were 18% and 19% with negative deep ILNM and extranodal extension (ENE); and 76% and 42% with positive deep ILNM and ENE, respectively. The accuracy of deep ILNM, ENE, bilateral involvement and number (≥3) of ILNMs for predicting PLNM were 81.0%, 65.6%, 63.5% and 67.2%, respectively. The CSS was significantly different in patients with positive and negative deep ILNM (median 1.7 years not reached, p<0.01). Patients who presented with deep ILNM had worse CSS (median 3.8 years not reached, p<0.01) in those with negative PLNs.

Conclusions: Deep ILNM is the most accurate factor for predicting PLNM in PSCC according to our data. We recommend that patients with deep ILNM should be referred for pelvic lymph node dissection. Involvement of deep ILNs indicates poor prognosis. We propose that patients with metastases of deep ILNs may be staged as pN3.

Tumor-Infiltrating PD-1CD8-T-Cell Signature as an Effective Biomarker for Immune Checkpoint Inhibitor Therapy Response Across Multiple Cancers.

Background: Stratification of patients who could benefit from immune checkpoint inhibitor (ICI) therapy is of much importance. PD-1CD8 T cells represent a newly identified and effective biomarker for ICI therapy response biomarker in lung cancer. Accurately quantifying these T cells using commonly available RNA sequencing (RNA-seq) data may extend their applications to more cancer types.

Method: We built a transcriptome signature of PD-1CD8 T cells from bulk RNA-seq and single-cell RNA-seq (scRNA-seq) data of tumor-infiltrating immune cells. The signature was validated by flow cytometry and in independent datasets. The clinical applications of the signature were explored in non-small-cell lung cancer, melanoma, gastric cancer, urothelial cancer, and a mouse model of breast cancer samples treated with ICI, and systematically evaluated across 21 cancer types in The Cancer Genome Atlas (TCGA). Its associations with other biomarkers were also determined.

Results: Signature scores could be used to identify the PD-1CD8 T subset and were correlated with the fraction of PD-1CD8 T cells in tumor tissue (Pearson correlation, R=0.76, =0.0004). Furthermore, in the scRNA-seq dataset, we confirmed the capability of PD-1CD8 T cells to secrete CXCL13, as well as their interactions with other immune cells. In 581 clinical samples and 204 mouse models treated with ICIs, high signature scores were associated with increased survival, and the signature achieved area under the receiver operating characteristic curve scores of 0.755 (ranging from 0.61 to 0.91) in predicting therapy response. In TCGA pan-cancer datasets, our signature scores were consistently correlated with therapy response (R=0.78, <0.0001) and partially explained the diverse response rates among different cancer types. Finally, our signature generally outperformed other mRNA-based predictors and showed improved predictive performance when used in combination with tumor mutational burden (TMB). The signature score is available in the R package "PD1highCD8Tscore" (https://github.com/Liulab/PD1highCD8Tscore).

Conclusion: Through estimating the fraction of the PD-1CD8 T cell, our signature could predict response to ICI therapy across multiple cancers and could serve as a complementary biomarker to TMB.

Serum Vitamin A Nutritional Status of Children and Adolescents Aged 6-17 Years - China, 2016-2017.

Summary: Vitamin A deficiency (VAD) in children is still a global public health problem, which needs continuous monitoring and timely intervention.

Using surveillance data from China Nutrition and Health Surveillance of Children and Lactating Mothers in 2016-2017, the prevalence of VAD and marginal deficiency was 0.96% and 14.71%, respectively. The vitamin A nutritional status of children and adolescents from urban areas and those aged 12-17 years were better than those from rural areas and aged 6-11 years.

Marginal VAD was a major form of VAD in Chinese children. The monitoring of vitamin A status in key populations should be continuously strengthened, and the public should be encouraged to consume foods rich in vitamin A or vitamin A supplements.

What Is Already Known On This Topic?: Low folate status in pregnancy has been associated with multiple adverse pregnancy outcomes, including neural tube defects, congenital heart defects, fetal growth restriction, low birth weight, and preterm delivery. Low folate status is common in China, especially in northern areas.

What Is Added By This Report?: Folate status remains suboptimal among pregnant Chinese women in 2015. Folate concentration was in the widespread in rural area (9.88ng/mL) and especially in the northern of China (9.10ng/mL). Pregnant women in the last trimester had the lowest folic acid concentration (9.18 ng/mL). Taking folic acid supplements every day would achieve adequate serum folate concentrations (10.87 ng/mL 10.11 ng/mL 9.38 ng/mL, <0.001).

What Are The Implications For Public Health Practice?: Folic acid interventions should be strengthened, especially for pregnant women in rural areas and in northern China, those with junior school or below education, those pregnant during spring and winter, or those with late pregnancy.

Polymorphisms Are Associated With Gastric Cancer Risk: The Changes in Protein Spatial Structure May Play a Potential Role.

Background: Gastric cancer (GC) is one of the most significant health problems worldwide. Some studies have reported associations between Phospholipase C epsilon 1 () single-nucleotide polymorphisms (SNPs) and GC susceptibility, but its relationship with GC prognosis lacked exploration, and the specific mechanisms were not elaborated fully yet. This study aimed to further explore the possible mechanism of the association between polymorphisms and GC.

Materials And Methods: A case-control study, including 588 GC patients and 703 healthy controls among the Chinese Han population, was performed to investigate the association between SNPs of and GC risk by logistic regression in multiple genetic models. The prognostic value of in GC was evaluated by the Kaplan-Meier plotter. To explored the potential functions of , various bioinformatics analyses were conducted. Furthermore, we also constructed the spatial structure of PLCE1 protein using the homology modeling method to analyze its mutations.

Results: Rs3765524 C > T, rs2274223 A > G and rs3781264 T > C in were associated with the increased risk of GC. The overall survival and progression-free survival of patients with high expression of PLCE1 were significantly lower than those with low expression [HR (95% CI) = 1.38 (1.1-1.63), < 0.01; HR (95% CI) = 1.4 (1.07-1.84), = 0.01]. Bioinformatic analysis revealed that was associated with protein phosphorylation and played a crucial role in the calcium signal pathway. Two important functional domains, catalytic binding pocket and calcium ion binding pocket, were found by homology modeling of PLCE1 protein; rs3765524 polymorphism could change the efficiency of the former, and rs2274223 polymorphism affected the activity of the latter, which may together play a potentially significant role in the tumorigenesis and prognosis of GC.

Conclusion: Patients with high expression of had a poor prognosis in GC, and SNPs in were associated with GC risk, which might be related to the changes in spatial structure of the protein, especially the variation of the efficiency of PLCE1 in the calcium signal pathway.

Low-Cost Synthesis of Silicon Quantum Dots with Near-Unity Internal Quantum Efficiency.

As a cost-effective batch synthesis method, Si quantum dots (QDs) with near-infrared photoluminescence, high quantum yield (>50% in polymer nanocomposite), and near-unity internal quantum efficiency were fabricated from an inexpensive commercial precursor (triethoxysilane, TES), using optimized annealing and etching processes. The optical properties of such QDs are similar to those prepared from state-of-the-art precursors (hydrogen silsesquioxane, HSQ) yet featuring an order of magnitude lower cost. To understand the effect of synthesis parameters on QD optical properties, we conducted a thorough comparison study between common solid precursors: TES, HSQ, and silicon monoxide (SiO), including chemical, structural, and optical characterizations. We found that the structural nonuniformity and abundance of oxide inherent to SiO limited the resultant QD performance, while for TES-derived QDs this drawback can be avoided. The presented low-cost synthetic approach would significantly favor applications requiring high loading of good-quality Si QDs, such as light conversion for photovoltaics.

Background: To assess the efficacies and potential predictors of a corticosteroid switch in metastatic castration-resistant prostate cancer (mCRPC) patients with biochemical progression on abiraterone acetate plus prednisone (A + P).

Methods: Patients with mCRPC treated between April 2016 and August 2020, who experienced biochemical progression on A + P and then switched to A plus dexamethasone (D), were retrospectively identified. The primary endpoint was progression-free survival (PFS), and the secondary endpoints were PSA response, overall survival (OS), and safety.

Results: One hundred and thirty consecutive cases were enrolled. The median PFS and OS on A + D were 5.0 and 18.7 months, respectively. The best PSA decline of ≥50% (PSA50) and ≥ 30% (PSA30) were observed in 29.2 and 46.2% patients, respectively. Lower PSA at corticosteroid switch (≤ 20 ng/mL; median PFS, HR 0.63, p = 0.019; median OS, HR 0.38, p = 0.001) and longer mCRPC-free survival (≥ 18 months; median PFS, HR 0.61, p = 0.013; median OS, HR 0.51, p = 0.015) were identified as independent prognostic predictors associated with longer PFS and OS. A risk stratification tool was developed to select candidates for corticosteroid switch based on the independent prognostic predictors of PFS and OS.

Conclusions: A corticosteroid switch from prednisone to dexamethasone is effective for mCRPC which progressed on A + P treatment. Patients with lower PSA at corticosteroid switch and/or longer mCRPC-free survival may gain more benefits by the corticosteroid switch.

Broadband achromatic metalens design based on deep neural networks.

For the design of achromatic metalenses, one key challenge is to accurately realize the wavelength dependent phase profile. Because of the demand of tremendous simulations, traditional methods are laborious and time consuming. Here, a novel deep neural network (DNN) is proposed and applied to the achromatic metalens design, which turns complex design processes into regression tasks through fitting the target phase curves. During training, - projection pairs are put forward to solve the phase jump problem, and some additional phase curves are manually generated to optimize the DNN performance. To demonstrate the validity of our DNN, two achromatic metalenses in the near-infrared region are designed and simulated. Their average focal length shifts are 2.6% and 1.7%, while their average relative focusing efficiencies reach 59.18% and 77.88%.

Tunable Transmissive Terahertz Linear Polarizer for Arbitrary Linear Incidence Based on Low-Dimensional Metamaterials.

In this work, we propose a structure consisting of three metamaterial layers and a metallic grating layer to rotate the polarization of arbitrary linearly polarized incidence to the y-direction with high transmissivity by electrically tuning these metamaterials. The transfer matrix method together with a harmonic oscillator model is adopted to theoretically study the proposed structure. Numerical simulation based on the finite difference time-domain method is performed assuming that the metamaterial layers are constituted by graphene ribbon arrays. The calculation and simulation results show that the Drude absorption is responsible for the polarization rotation. Fermi level and scattering rate of graphene are important for the transmissivity. For a polarization rotation of around 90°, the thickness of either the upper or lower dielectric separations influences the transmission window. For a polarization rotation of around 45° and 135°, the lower dielectric separations decide the frequency of the transmission window, while the upper dielectric separations just slightly influence the transmissivity.
