Primary biliary non-Hodgkin's lymphoma: A case report.

Rationale: Primary biliary non-Hodgkin's lymphoma (PBNHL) is a rare disease with only 41 cases reported since 1982. The incidence of PBNHL in patients with malignant cholangiocarcinoma was 0.6%, and PBNHL accounted for 0.4% of extranodal non-Hodgkin's lymphoma, and only 0.016% of all non-Hodgkin's lymphoma cases.

Patient Concerns: We present a rare case of PBNHL in a 59-year-old female who had jaundice for 3 days with weight loss and Epstein-Barr virus infection. Initial computed tomography and magnetic resonance imaging showed diffuse thickening wall of bile ducts with corresponding lumen stenosis, blurred fat space around the portal vein, lymphadenopathy, and a normal spleen. These manifestations and images were similar to hilar cholangiocarcinoma. So, the diagnosis of hilar cholangiocarcinoma was initially considered.

Diagnoses: Postoperative pathology confirmed the final diagnosis was PBNHL.

Interventions: The patient and her family requested to clarify the histologic diagnosis by laparotomy biopsy. Because the biopsy result could not be defined during operation, then right hemihepatectomy and choledochojejunostomy were performed. She did not receive any antitumor treatment.

Outcomes: One month after the patient's first examination, both computed tomography and magnetic resonance images showed diminished stenosis of common bile duct and left hepatic duct, but a new mass in segment IV of liver was observed. Unfortunately, the patient died due to disease progression.

Lessons: This case reminds us that although PBNHL is rare, making accurate diagnosis difficult preoperatively, PBNHL should be considered when encountering a case with Epstein-Barr virus infection and those typical imaging findings.

Cesium Lead Bromide Perovskite-Based Lithium-Oxygen Batteries.

The main challenge for lithium-oxygen (Li-O) batteries is their sluggish oxygen evolution reaction (OER) kinetics and high charge overpotentials caused by the poorly conductive discharge products of lithium peroxide (LiO). In this contribution, the cesium lead bromide perovskite (CsPbBr) nanocrystals were first employed as a high-performance cathode for Li-O batteries. The battery with a CsPbBr cathode can exhibit the lowest charge overpotential of 0.5 V and the best cycling performance of 400 cycles among all the reported perovskite-based Li-O cells, which represents a new benchmark. Most importantly, the density functional theory (DFT) calculations further prove that the rate limitation step during OER processes is the decomposition of LiO to form O and Li, and the weak adsorption strength between CsPbBr surfaces and LiO results in a low charge overpotential for the CsPbBr-based Li-O battery. This work first demonstrates the good potential of CsPbBr for application in metal-air batteries.

Frequency splicing code-based Brillouin optical time domain collider for fast dynamic measurement.

We propose a frequency splicing code-based Brillouin optical time domain collider (FSC-BOTDC) for fast dynamic sensing. By delicately designing the frequency splicing code (FSC), multiple collision modes with controllable characteristics are realized for probing multiple target areas with a high sampling rate. Moreover, the sensing system is simpler and more robust than the previous BOTDC. In the experiment, the FSC-BOTDC with 10-time enhanced sampling rate is implemented for single and multiple target areas measurements. Results demonstrate that tailorable measurements can be achieved by the tunable FSC. Furthermore, the FSC-BOTDC is executed to measure periodic mechanical vibrations over 7.9-km sensing range with the sampling rate of 625 Hz.

Gestational high-fat diet impaired demethylation of Pparα and induced obesity of offspring.

Gestational and postpartum high-fat diets (HFDs) have been implicated as causes of obesity in offspring in later life. The present study aimed to investigate the effects of gestational and/or postpartum HFD on obesity in offspring. We established a mouse model of HFD exposure that included gestation, lactation and post-weaning periods. We found that gestation was the most sensitive period, as the administration of a HFD impaired lipid metabolism, especially fatty acid oxidation in both foetal and adult mice, and caused obesity in offspring. Mechanistically, the DNA hypermethylation level of the nuclear receptor, peroxisome proliferator-activated receptor-α (Pparα), and the decreased mRNA levels of ten-eleven translocation 1 (Tet1) and/or ten-eleven translocation 2 (Tet2) were detected in the livers of foetal and adult offspring from mothers given a HFD during gestation, which was also associated with low Pparα expression in hepatic cells. We speculated that the hypermethylation of Pparα resulted from the decreased Tet1/2 expression in mothers given a HFD during gestation, thereby causing lipid metabolism disorders and obesity. In conclusion, this study demonstrates that a HFD during gestation exerts long-term effects on the health of offspring via the DNA demethylation of Pparα, thereby highlighting the importance of the gestational period in regulating epigenetic mechanisms involved in metabolism.

Role of clinicopathological features for the early prediction of prognosis in lupus nephritis.

Ambiguities remain regarding the role of clinicopathological characteristics in the early prediction of the prognosis of lupus nephritis (LN). Systemic lupus erythematosus (SLE) patients who completed routine follow-up were identified and retrospectively reviewed for eligible cases. Poor prognosis was defined as all-cause mortality or a persistent decrease of eGFR greater than half the baseline level or progression to end-stage renal disease (ESRD). An optimal Cox regression model was constructed for the early prediction of a poor prognosis for LN. Among the 2163 SLE patients, 376 eligible LN cases were enrolled in the study, with a median follow-up time of 55 [27.0, 87.0] months. The male-to-female ratio was 1:7.2, and 37 patients (9.8%) progressed to the composite endpoint. The ISN/RPS class was significantly associated with proteinuria levels (P-value < 0.001), and class IV/IV + V patients, but not class V patients, had the most severe proteinuria. Our optimal multivariate Cox regression model indicated that sex, ISN/RPS class, tubular atrophy/interstitial fibrosis, serum albumin, tertiles of proteinuria, and their interaction were independently associated with a poor prognosis. ROC analysis and external validation demonstrated that our model was efficient and robust for distinguishing LN patients with a poor prognosis. Our study constructed a robust and early predictive model for convenience in clinical practice to identify poor prognosis in LN patients. We found a significant interaction effect between proteinuria and serum albumin for the prediction of poor prognosis. LN patients with low-level proteinuria and hypoalbuminemia exhibit an increased hazard of progression to poor outcomes.

Paternal obesity impairs hepatic gluconeogenesis of offspring by altering Igf2/H19 DNA methylation.

Over the past four decades, the global prevalence of obesity has increased rapidly in all age ranges. Emerging evidence suggests that paternal lifestyle and environmental exposure have a crucial role in the health of offspring. Therefore, the current study investigated the impact of paternal obesity on the metabolic profile of offspring in a male mouse model of obesity. Female offspring of obese fathers fed a high-fat diet (HFD) (60% kcal fat) showed hyperglycemia because of enhanced gluconeogenesis and elevated expression of phosphoenolpyruvate carboxykinase (PEPCK), which is a key enzyme involved in the regulation of gluconeogenesis. Methylation of the Igf2/H19 imprinting control region (ICR) was dysregulated in the liver of offspring, and the sperm, of HFD fathers, suggesting that epigenetic changes in germ cells contribute to this father-offspring transmission. In addition, we explored whether H19 might regulate hepatic gluconeogenesis. Our results showed that overexpression of H19 in Hepa1-6 cells enhanced the expression of PEPCK and gluconeogenesis by promoting nuclear retention of forkhead box O1 (FOXO1), which is involved in the transcriptional regulation of Pepck. Thus, the current study suggests that paternal exposure to HFD impairs the gluconeogenesis of offspring via altered Igf2/H19 DNA methylation.

Distributed dynamic strain sensing in coherent Φ-OTDR with a pulse conversion algorithm.

In this Letter, a chirped-pulse conversion algorithm (CPCA) is proposed to convert a normal probe pulse into an equivalent chirped probe pulse by convolving a chirp factor on the received signal in coherent phase-sensitive optical time-domain reflectometry (-OTDR). With this algorithm, the Rayleigh interference pattern (RIP) demodulation once applied to chirped-pulse -OTDR can be adopted to quantify the dynamic strain in the traditional coherent -OTDR. Since the equivalent chirped pulse is generated by digital processing, complex and costly chirp modulation is not required. The proposed scheme is capable of fully quantifying perturbations with spatial resolution of 4 m, a sensing range of 8 km, frequency response of 5 kHz, and strain resolution of 56ε/√.

Protective effect of electrical stimulation of the vagus nerve in lipopolysaccharide‑induced acute lung injury in rats.

The present study investigated the role of electrical stimulation of the vagus nerve in the acute lung injury (ALI) inflammatory response induced by lipopolysaccharide (LPS) in rats. A rat model of ALI was established using LPS and by connecting an electrode to the left vagus nerve proximal to the heart in order to provide continuous electrical stimulation (1 mA; 1 msec; 10 Hz). After 120 min, the rat lung tissue was stained with hematoxylin and eosin and the expression of inflammatory factors was evaluated by reverse transcription‑quantitative PCR and western blot analysis. The change in apoptosis rate in cells from bronchoalveolar lavage fluid (BALF) was analyzed using flow cytometry. The results of the present study demonstrated that inflammatory cell infiltration, alveolar wall and interstitial thickening, and lung hyperemia in rats with LPS‑induced ALI were decreased following electrical stimulation of the vagus nerve. Electrical stimulation inhibited the expression levels of IL‑1, IL‑6, IL‑10, IL-8 and TNF‑α at both the mRNA and protein levels and decreased early and late apoptosis rates in inflammatory cells from BALF. The results indicated that vagus nerve stimulation can reverse the inflammatory response in lung injury, thereby exerting a pulmonary protective effect.

Epidural analgesia followed by epidural hydroxyethyl starch prevented post-dural puncture headache: Twenty case reports and a review of the literature.

Background: Accidental dural puncture (ADP) and subsequent post-dural puncture headache (PDPH) remain common complications of epidural procedures for obstetric anesthesia and analgesia. No clear consensus exists on the best way to prevent PDPH after ADP.

Case Summary: We report our findings in twenty parturients who underwent an incorporated strategy of epidural analgesia followed by epidural hydroxyethyl starch (HES) to prevent PDPH after ADP with a 16-gauge Tuohy needle during epidural procedures. ADP with a 16-gauge Tuohy needle occurred in nine parturients undergoing a cesarean section (CS) and in eleven parturients receiving labor analgesia. An epidural catheter was re-sited at the same or adjacent intervertebral space in all patients. After CS, the epidural catheter was used for postoperative pain relief over a 48-h period. After delivery in eleven cases, epidural infusion was maintained for 24 h. Thereafter, 15 mL of 6% HES 130/0.4 was administered the epidural catheter immediately prior to catheter removal. None of the parturients developed PDPH or neurologic deficits over a follow-up period of at least two months to up to one year postpartum.

Conclusion: An incorporated strategy of epidural analgesia followed by epidural hydroxyethyl starch may have great efficacy in preventing PDPH after ADP.

Zoom liquid lens employing a multifocal Fresnel zone plate.

We propose a zoom liquid lens employing a multifocal Fresnel zone plate. The proposed lens has two optical surfaces: liquid-liquid interface and Fresnel zone plate. The Fresnel zone plate is designed to have a multifocal point and an increased depth of focus. Therefore, the proposed lens has two obvious advantages. Due to increased depth of focus, the proposed lens can realize zooming using only one tunable liquid-liquid interface, which is not available for conventional liquid lens. Thus, it is possible to remove conventional zooming mechanisms from cameras. Besides, the focal length tuning range is also increased, and a lens system based on the proposed lens can simultaneously collect two images with different magnifications. We present the design, fabrication and characterization of the proposed lens. The shortest positive and negative focal length are ∼17.5mm and ∼-34.5mm and the diameter is 5mm. The zoom ratio of the proposed lens reaches ∼1.48×. Our results confirm that the proposed lens has widespread applications in imaging system.

Data fusion for the measurement of potentially toxic elements in soil using portable spectrometers.

Soil contamination posed by potentially toxic elements is becoming more serious under continuously development of industrialization and the abuse of fertilizers and pesticides. The investigation of soil potentially toxic elements is therefore urgently needed to ensure human and other organisms' health. In this study, we investigated the feasibility of the separate and combined use of portable X-ray fluorescence (pXRF) and visible near-infrared reflectance (vis-NIR) sensors for measuring eight potentially toxic elements in soil. Low-level fusion was achieved by the direct combination of the pXRF and vis-NIR spectra; middle-level fusion was achieved by the combination of selected bands of the pXRF and vis-NIR spectra using the Boruta feature selection algorithm; and high-level fusion was conducted by outer-product analysis (OPA) and Granger-Ramanathan averaging (GRA). The estimation accuracy for the eight considered elements were in the following order: Zn > Cu > Ni > Cr > As > Cd > Pb > Hg. The measurement for Cu and Zn could be achieved by pXRF spectra alone with Lin's concordance correlation coefficient (LCCC) values of 0.96 and 0.98, and ratio of performance to interquartile distance (RPIQ) values of 2.36 and 2.69, respectively. The measurement of Ni had the highest model performance for high-level fusion GRA with LCCC of 0.89 and RPIQ of 3.42. The measurements of Cr using middle- and high-level fusion were similar, with LCCC of 0.86 and RPIQ of 2.97. The best estimation accuracy for As, Cd, and Pb were obtained by high-level fusion using OPA, with LCCC >0.72 and RPIQ >1.2. However, Hg measurement by these techniques failed, having an unacceptable performance of LCCC <0.20 and RPIQ <0.75. These results confirm the effectiveness of using portable spectrometers to determine the contents of several potentially toxic elements in soils.

Catalyst-free activation of permanganate under visible light irradiation for sulfamethazine degradation: Experiments and theoretical calculation.

In this study, visible light (VL) was adopted for permanganate (PM) activation without additional catalyst, where sulfamethazine (SMT) was selected as the probe compound. Experiment results showed that the VL/PM system can effectively degrade SMT through pseudo-first-order reaction kinetics. Influencing factors including PM dosage, solution pH, humid acid (HA) and coexisting anions (CO, SO, Cl and NO) which affect SMT photo-degradation were also examined. Pyrophosphate (PP) had an inhibitory effect on SMT degradation due to the complexation of PP with Mn (III). Electron spin resonance (ESR) spectrometry and UV-Vis spectrophotometer proved that VL can activate PM to generate ·O and Mn (III) reactive species. Furthermore, based on the active site prediction, intermediates identification and Density Functional Theory (DFT) calculation, two main degradation pathways involving SMT molecular rearrangement and cleavage of S-N bond were proposed. Moreover, the energy barriers of the two degradation pathways were also calculated. This study offers a novel approach for aqueous SMT removal and deepens our understanding of the degradation mechanism of SMT through DFT calculation, which hopes to shed light on the future development of VL/PM treatment.

Phase fluctuation cancellation for coherent-detection BOTDA fiber sensors based on optical subcarrier multiplexing.

The employment of coherent detection in a Brillouin optical time domain analysis (BOTDA) fiber sensor brings benefits, including signal-to-noise ratio enhancement, non-local effect reduction, and sensing speed improvement. Recently, it was found that the performance of a coherent-detection BOTDA fiber sensor suffers from phase fluctuations introduced by the fiber group delay jitter. Here, we propose a phase fluctuation cancellation approach based on optical subcarrier multiplexing. In a proof-of-concept experiment, the phase stability for in-phase/quadrature demodulation reaches a standard deviation value of as small as 0.4 mrad. The variations in the Brillouin gain and phase spectra caused by the phase fluctuation are then effectively alleviated, resulting in an enhancement of the Brillouin frequency shift measurement certainty along the whole sensing fiber.

Long-term joint outcomes of regular low-dose prophylaxis in Chinese children with severe haemophilia A.

Objectives: To explore the long-term joint outcomes of low-dose prophylaxis in Chinese children with severe haemophilia A and to analyse their related factors.

Methods: We retrospectively analysed follow-up data from 21 severe haemophilia A children on regular low-dose prophylaxis for 6-10 years. We used International Prophylaxis Study Group magnetic resonance imaging score (IPSG MRI score), Hemophilia Joint Health Score (HJHS), number of target joints, and Hemophilia-Specific Quality of Life Index (Haemo-QoL) to evaluate joint outcomes. Factors associated with these outcomes were evaluated by statistical analysis.

Results: (1) The children were 1.75 to 17 years age at prophylaxis initiation. Median prophylactic factor VIII dose was 22.9 IU/kg per week. (2) At the end of follow-up: (a) The total IPSG MRI scores were 2-24 with 90.5% children exhibiting moderate to severe joint involvement (score 7-24); (b) The HJHS ranged 2-27, with 0-10 for 46.7% children and >10 for 53.3% children. There was a positive correlation between the MRI score and HJHS (p < .05); (c) Compared to their on-demand treatment period before prophylaxis, target joints numbers decreased, and no child needed auxiliary devices to walk; (d) Joint outcomes were positively correlated with the age at initiation of low-dose prophylaxis (p < .05) and negatively correlated with the treatment dose.

Conclusion: Long-term low-dose prophylaxis had positive effect on joint outcomes compared with on-demand treatment. However, a certain degree of joint damage remained in all children indicating the need for improving the current strategy of low-dose prophylaxis.

Prolonged Diagnostic Intervals as Marker of Missed Diagnostic Opportunities in Bladder and Kidney Cancer Patients with Alarm Features: A Longitudinal Linked Data Study.

Background: In England, patients who meet National Institute for Health and Care Excellence (NICE) guideline criteria for suspected cancer should receive a specialist assessment within 14 days. We examined how quickly bladder and kidney cancer patients who met fast-track referral criteria were actually diagnosed.

Methods: We used linked primary care and cancer registration data on bladder and kidney cancer patients who met fast-track referral criteria and examined the time from their first presentation with alarm features to diagnosis. Using logistic regression we examined factors most likely to be associated with non-timely diagnosis (defined as intervals exceeding 90 days), adjusting for age, sex and cancer type, positing that such occurrences represent missed opportunity for timely referral, possibly due to sub-optimal guideline adherence.

Results: 28%, 42% and 31% of all urological cancer patients reported no, one or two or more relevant symptoms respectively in the year before diagnosis. Of the 2105 patients with alarm features warranting fast-track assessment, 1373 (65%) presented with unexplained haematuria, 382 (18%) with recurrent urinary tract infections (UTIs), 303 (14%) with visible haematuria, and 45 (2%) with an abdominal mass. 27% overall, and 24%, 45%, 18% and 27% of each group respectively, had a non-timely diagnosis. Presentation with recurrent UTI was associated with longest median diagnostic interval (median 83 days, IQR 43-151) and visible haematuria with the shortest (median 50 days, IQR 30-79). After adjustment, presentation with recurrent UTIs, being in the youngest or oldest age group, female sex, and diagnosis of kidney and upper tract urothelial cancer, were associated with greater odds of non-timely diagnosis.

Conclusion: More than a quarter of patients presenting with fast-track referral features did not achieve a timely diagnosis, suggesting inadequate guideline adherence for some patients. The findings highlight a substantial number of opportunities for expediting the diagnosis of patients with bladder or kidney cancers.

Time from presentation to pre-diagnostic chest X-ray in patients with symptomatic lung cancer: a cohort study using electronic patient records from English primary care.

Background: National guidelines in England recommend prompt chest X-ray (within 14 days) in patients presenting in general practice with unexplained symptoms of possible lung cancer, including persistent cough, shortness of breath, or weight loss.

Aim: To examine time to chest X-ray in symptomatic patients in English general practice before lung cancer diagnosis, and explore demographical variation.

Design And Setting: Retrospective cohort study using routinely collected general practice, cancer registry, and imaging data from England.

Method: Patients with lung cancer who presented symptomatically in general practice in the year pre-diagnosis and who had a pre-diagnostic chest X-ray were included. Time from presentation to chest X-ray (presentation-test interval) was determined and intervals classified based on national guideline recommendations as concordant (≤14 days) or non-concordant (>14 days). Variation in intervals was examined by age, sex, smoking status, and deprivation.

Results: In a cohort of 2102 patients with lung cancer, the median presentation-test interval was 49 (interquartile range [IQR] 5-172) days. Of these, 727 (35%) patients had presentation-test intervals of ≤14 days (median 1 [IQR 0-6] day) and 1375 (65%) had presentation-test intervals of >14 days (median 128 [IQR 52-231] days). Intervals were longer among patients who smoke (equivalent to 63% longer than non-smokers; <0.001), older patients (equivalent to 7% longer for every 10 years from age 27; = 0.013), and females (equivalent to 12% longer than males; = 0.016).

Conclusion: In symptomatic primary care patients who underwent chest X-ray before lung cancer diagnosis, only 35% were tested within the timeframe recommended by national guidelines. Patients who smoke, older patients, and females experienced longer intervals. These findings could help guide initiatives aimed at improving timely lung cancer diagnosis.

Application of functionalized graphene in Li-O batteries.

Li-O batteries (LOB) are considered as one of the most promising energy storage devices using renewable electricity to power electric vehicles because of its exceptionally high energy density. Carbon materials have been widely employed in LOB for its light weight and facile availability. In particular, graphene is a suitable candidate due to its unique two-dimensional structure, high conductivities, large specific surface areas, and good stability at high charge potential. However, the intrinsic catalytic activity of graphene is insufficient for the sluggish kinetics of oxygen reduction reaction (ORR) and oxygen evolution reaction (OER) in LOB. Therefore, various surface functionalization schemes for graphene have been developed to tailor the surface chemistry of graphene. In this review, the properties and performances of functionalized graphene cathodes are discussed from theoretical and experimental aspects, including heteroatomic doping, oxygen functional group modifications, and catalyst decoration. Heteroatomic doping breaks electric neutrality of sp carbon of graphene, which forms electron-deficient or electron-rich sites. Oxygen functional groups mainly create defective edges on graphene oxides with C-O, C=O, and -COO-. Catalyst decoration is widely attempted by various transition and precious metal and metal oxides. These induced reactive sites usually improve the ORR and/or OER in LOB by manipulating the adsorption energies of O, LiO, LiO, and promoting electron transportation of cathode. In addition, functionalized graphene is used in anode and separators to prevent shuttle effect of redox mediators and suppress growth of Li dendrite.

Carcinoembryonic antigen levels are increased with pulmonary output in pulmonary hypertension due to congenital heart disease.

Objective: Pulmonary artery hypertension (PAH) is a severe complication of congenital heart disease (CHD). Monitoring of pulmonary arterial pressure (PAP) and pulmonary vascular resistance (PVR) is essential during follow-up. This retrospective study aimed to examine carcinoembryonic antigen (CEA) as an additional marker for evaluation by investigating the correlation between CEA levels and hemodynamics in CHD-PAH.

Methods: Seventy-six patients with CHD-PAH (mean PAP [mPAP] >25 mmHg and PVR >3 Wood units, group A), 71 patients with CHD and pulmonary hypertension (CHD-PH, mPAP >25 mmHg and PVR ≤3 Wood units, group B), and 102 patients with CHD without PH (mPAP ≤25 mmHg, group C) were enrolled. Serum CEA levels and the relationships between CEA levels and hemodynamic data were assessed.

Results: Mean serum CEA levels were 1.99±1.61, 2.44±1.82, and 1.58±1.07 ng/mL, mPAP was 58.66±20.21, 30.2±4.83, and 17.31±4.51 mmHg, and PVR was 10.12±7.01, 2.19±0.56, and 2.2±1.1 Wood units in groups A, B, and C, respectively. Mean pulmonary output (PO) was 7.24±3.07, 15.79±5.49, 10.18±4.72 L/minute, respectively. CEA levels were positively correlated with PO and negatively correlated with PVR in all of the patients.

Conclusion: CEA levels are increased with PO and decreased with PVR in CHD-PH.

Developing and validating an integrated gross tumor volume (GTV)-TNM stratification system for supplementing unresectable locally advanced non-small cell lung cancer treated with concurrent chemoradiotherapy.

Purpose: The gross tumor volume (GTV) could be an independent prognostic factor for unresectable locally advanced non-small cell lung cancer (LANSCLC). We aimed to develop and validate a novel integrated GTV-TNM stratification system to supplement LANSCLC sub-staging in patients treated with concurrent chemoradiotherapy (CCRT).

Methods: We performed a retrospective review of 340 patients with unresectable LANSCLC receiving definitive CCRT. All included patients were divided into two randomized cohorts. Then the Kaplan-Meier method and Cox regression were calculated to access the prognostic value of the integrated GTV-TNM stratification system, which was further validated by the area under the receiver operating characteristic curve (AUC) score and F1-score.

Results: The optimal outcome-based GTV cut-off values (70 and 180 cm) of the modeling cohort were used to determine each patient's integrated GTV-TNM stratum in the whole cohort. Our results indicated that a lower integrated GTV-TNM stratum could had better overall survival and progression-free survival (all P < 0.001), which was recognized as an independent prognostic factor. Also, its prognostic value was robust in both the modeling and validation cohorts. Furthermore, the prognostic validity of the integrated GTV-TNM stratification system was validated by significantly improved AUC score (0.636 vs. 0.570, P = 0.027) and F1-score (0.655 vs. 0.615, P < 0.001), compared with TNM stage.

Conclusions: We proposed a novel integrated GTV-TNM stratification system to supplement unresectable LANSCLC sub-staging due to its prognostic value independent of TNM stage and other clinical characteristics, suggesting that it could be considered in individual treatment decision-making process.

Association between mutation of interleukin 36 receptor antagonist and generalized pustular psoriasis: A PRISMA-compliant systematic review and meta-analysis.

Background: Generalized pustular psoriasis (GPP) is a systemic inflammatory disease with poor outcomes, and several studies have suggested that the mutation of the interleukin 36 receptor antagonist gene (IL36RN) is related to GPP, where the polymorphism c.115+6T>C is reported to be the most common mutation of IL36RN. This study was performed to clarify and comprehensively evaluate the relationship between IL36RN gene polymorphism and the susceptibility of GPP subtypes.

Methods: To conduct a thorough literature review, studies were obtained using databases such as Pubmed, EMBASE, Cochrane, China National Knowledge Infrastructure, and the Wanfang database. Only studies published up to December 2019 were included. The quality of the research studies was estimated using the Newcastle-Ottawa scale. The total odds ratios (ORs) and corresponding 95% confidence intervals (95% CIs) were pooled and analysed using STATA 14. The publication bias was evaluated through the Egger test, performed using the aforementioned software. Five common gene models were built and analysed to assess the association between the polymorphism c.115+6T>C and subtypes of GPP.

Results: A total of 10 studies were selected, including 683 cases of GPP patients. Meta-analyses showed that there was a significant statistical correlation of IL36RN mutation between GPP with or without psoriasis vulgaris (OR = 3.82, 95%CI 2.63-5.56) and between adult GPP and paediatric GPP (OR = 0.42, 95%CI 0.23-0.77). No obvious discrepancy between European patients (OR = 4.03, 95%CI 2.23-7.26) and Asian patients was found. The gene models showed clear associations between the polymorphism c.115+6T>C and GPP through the dominant model (CC+ TC vs TT, OR 2.74, 95%CI 2.06-3.64), recessive model (CC vs CT + TT, OR 4.33, 95%CI 2.84-6.60), homozygote model (CC vs TT, OR 4.37, 95%CI 2.88-6.62), heterozygote model (CT vs TT, OR 2.26, 95%CI 1.32-3.85) and allelic model (C vs T, OR 3.35, 95%CI 2.63-4.27).

Conclusion: The IL36RN mutation is strongly related to GPP without psoriasis vulgaris and the early onset of GPP. Furthermore, the single-nucleotide polymorphism c.115+6T>C of the IL36RN gene plays a significant role in GPP vulnerability, especially in homozygous mutation. GPP could be a different inflammatory disease, independent of psoriasis.

This unique multiple-fluorine system provides rapid access to difluoromethylated tetrasubstituted stereocenters in isoindolones with wide substrate scope under mild conditions. Further synthetic transformations to enantioenriched CFH-isoindolones and CF-decorated fused isoindolones were also implemented with good efficiency.

A comparison and review of three sets of classification criteria for systemic lupus erythematosus for distinguishing systemic lupus erythematosus from pure mucocutaneous manifestations in the lupus disease spectrum.

Although the original purpose of the systemic lupus erythematosus (SLE) classification criteria was to distinguish SLE from other mimic diseases, and to facilitate sample selection in scientific research, they have become widely used as diagnostic criteria in clinical situations. It is not known yet if regarding classification criteria as diagnostic criteria, what problems might be encountered? This is the first study comparing the three sets of classification criteria for SLE, the 1997 American College of Rheumatology (ACR'97), 2012 Systemic Lupus International Collaborating Clinics (SLICC'12) and 2019 European League Against Rheumatism/American College of Rheumatology (EULAR/ACR'19), for their ability to distinguish patients with SLE from patients with pure mucocutaneous manifestations (isolated cutaneous lupus erythematosus without internal disease, i-CLE) in the lupus disease spectrum. 1,865 patients with SLE and 232 patients with i-CLE were recruited from a multicenter study. We found that, due to low specificity, none of the three criteria are adept at distinguishing patients with SLE from patients with i-CLE. SLICC'12 performed best among the original three criteria, but if a positive ANA was removed as an entry criterion, EULAR/ACR'19 would performed better. A review of previous studies that compared the three sets of criteria was presented in this work.

Modeling osteoinduction in titanium bone scaffold with a representative channel structure.

Optimizing scaffold architecture for perfect osteointegration depends on good understanding of bone ingrowth in the porous space of implants. This study developed an immunoregulatory agent-based model to discover the osteoinduction mechanism in porous scaffolds. Immunoreaction, macrophage polarization, and the corresponding growth factors were combined in the model, and all played critical roles in recruiting osteogenic cells that migrated into the scaffolds. Angiogenesis was also considered in this model. The bone ingrowth predicted by the model coincides with results from published in vivo experiments. Simulation results suggested that the pore architecture affected the diffusion process of chemotactic factors in the scaffolds, subsequently influencing the complex reactions of diverse cells and the osteoinduction location. In flexural pore spaces, bone formation spread from the periphery into the center of scaffolds due to larger M2 phenotype macrophage populations colonizing boundary regions and the distribution of corresponding growth factors concentration. In straight channels, osteogenic cells migrated further inward and osteoinduction initiated in deeper position as a result of the deeper distribution of osteogenic cytokines concentration field.

Remote functionalization reactions have the power to transform a C-H (or C-C) bond at a distant position from a functional group. This Review summarizes recent advances and key breakthroughs in remote fluorination, trifluoromethylation, difluoromethylation, trifluoromethylthiolation, and fluoroalkenylation reactions. Several powerful strategies have emerged to control the reactivity and distal selectivity such as the undirected radical approach, the 1,5-hydrogen atom transfer, the metal migration, the use of distant directing groups, and the ring-opening reactions. These unconventional and predictable C-H (and C-C) functionalization transformations should allow for the preparation of a wide range of otherwise-difficult-to-access alkyl, aromatic, heteroaromatic, and structurally complex fluorides.

Spatio-temporal variation and source changes of potentially toxic elements in soil on a typical plain of the Yangtze River Delta, China (2002-2012).

The spatio-temporal variation and temporal changes in the sources of Cr, Pb, Cd, Hg, and As in soil on the Hangzhou-Jiaxing-Huzhou (H-J-H) Plain were analysed based on 4,359 soil samples collected in 2002 and 2012. Geostatistical and spatial analysis methods were used to explore the spatio-temporal variation in the pollution levels and 'pollution hotspots' for potentially toxic elements (PTEs), and the positive matrix factor model was used to quantitatively appoint and analyse temporal changes in PTE sources. The results indicated that the PTE content in most parts of the survey area were at a safe level in both 2002 and 2012, but a clearly upward trend was detected for Cr, Pb, and Cd. Moreover the pollution index for Cr, Pb, Cd, and the Nemerow composite pollution index increased in the west but decreased in the east of the H-J-H Plain from 2002 to 2012. The pollution index for Hg and As presented the opposite spatial pattern. It is obvious that there have been changes in the spatial pattern of pollution hotspots for PTEs on the H-J-H Plain from 2002 to 2012. Four sources of PTEs in soil were quantitatively appointed. In 2002, 2012, the dominant sources of Cr, Cd, Hg, and As were soil parent materials, industrial activities, atmospheric deposition and agricultural inputs, respectively. The dominant source of Pb in the soil changed from traffic emissions to soil parent materials, indicating the benefit of banning the use of leaded gasoline in China. This study highlights the importance of monitoring soil environmental quality and highlights the significance of spatio-temporal variation in PTEs in suburban zones or transitional areas undergoing rapid industrialization and urbanization, like the H-J-H Plain.

Interface Engineering with Ultralow Ruthenium Loading for Efficient Water Splitting.

Developing high-performance and cost-effective bifunctional electrocatalysts for water splitting is the key to large-scale hydrogen production. How to achieve higher performance with a lower amount of noble metal is still a major challenge. Herein, using a facile wet-chemistry strategy, we report the ultralow amount loading of ruthenium (Ru) on porous nickel foam (NF) as a highly efficient bifunctional electrocatalyst for water splitting. Theoretical simulations reveal that the coupling effect of Ru and Ni can significantly reduce the d-band center of the composite. The Ru-modified NF exhibits a very high level of HER activity with only 0.3 wt% of Ru, far surpassing commercial Pt/C. It only requires an extremely low overpotential (η) of 10 mV to achieve a current density of 10 mA cm in alkaline solution and a quite low Tafel slope of 34 mV dec. This catalyst also shows remarkable performance for overall water splitting with a low voltage of 1.56 V at 10 mA cm. These findings indicate the potential of this material in water-alkali electrolyzers, providing a new approach for fabrication of low-cost advanced electrocatalysts.

Current status, spatial features, health risks, and potential driving factors of soil heavy metal pollution in China at province level.

In this study we systematically reviewed 1203 research papers published between 2008 and 2018 in China and recorded related data on eight kinds of soil heavy metals (Cr, Pb, Cd, Hg, As, Cu, Zn, and Ni). Based on that, the pollution levels, ecological risk and health risk caused by soil heavy metals were evaluated and the pollution hot spots and potential driving factors of different heavy metals in different provinces were also identified. Results indicated accumulation of heavy metals in soils of most provinces in China compared with background values. Consistent with previous findings, the most prevalent polluted heavy metals were Cd and Hg. Polluted regions are mainly located in central, southern and southwestern China. Hunan, Guangxi, Yunnan, and Guangdong provinces were the most polluted provinces. For the potential health risk caused by heavy metals pollution, children are more likely confront with non-carcinogenic risk than adults and seniors. And children in Hunan and Guangxi province were experiencing relatively larger non-carcinogenic risk. In addition, children in part of provinces were undergoing potentially carcinogenic risks due to soil heavy metals exposure. Furthermore, in our study the 31 provinces in mainland China were divided into six subsets according to corresponding potential driving factors for heavy metal accumulation. Our study provide more comprehensive and updated information for contributing to better soil management, soil remediation, and soil contamination control in China.
