Biomechanical analysis of a novel clavicular hook plate for the treatment of acromioclavicular joint dislocation: A finite element analysis.

Background: Clavicular hook plates are frequently used in clinical orthopedics to treat acromioclavicular joint dislocation. However, patients often exhibit acromial osteolysis and peri-implant fracture after hook plate fixation. To solve the above problems, we developed a novel double-hook clavicular plate and used finite element analysis (FEA) to investigate its biomechanical properties.

Methods: A finite element (FE) model was constructed and validated. Then, a double-hook clavicular plate, a single-hook clavicular plate, and an anatomical double-hook clavicular plate was implanted into the acromioclavicular joint and fixed with screws in groups 1, 2 and 3, respectively. Finally, a load was applied, and some indicators were recorded and analyzed.

Results: For both the proximal clavicular rotation angle and the distal clavicular displacement, the range of motion in groups 1 and 3 was more than 90% lower than that in group 2. The maximum von Mises stress of the clavicle in groups 1 and 3 was more than 45% lower than that in group 2. The maximum stress of the acromion in group 2 was significantly higher than that in groups 1 and 3, and that in group 3 was less than that in group 1, for both cortical and cancellous bone.

Conclusions: The double-hook clavicular plate could immediately reconstruct the stability of the acromioclavicular joint, effectively reducing the stress of the bone around the clavicle and screws. Additionally, the double-hook clavicular plate could reduce the peak stress of the acromion and produce a more uniform stress distribution.

Comprehensive transcriptome analysis of hypothalamus reveals genes associated with disorders of sex development in pigs.

XX sex reversal, also called XX disorders of sex development (XX-DSD), is a condition affecting the development of the gonads or genitalia, and is relatively common in pigs. However, its genetic etiology and transcriptional regulation mechanism in the hypothalamic-pituitary-gonadal axis (HPGA) remain mostly unknown. XX-DSD (SRY-negative) pigs and normal sows were selected by external genitalia observation. The hypothalamus, which is the integrated center of the HPGA was sampled for whole-transcriptome RNA-seq. The role of DEmiRNA was validated by its overexpression and knockdown in vitro. A total of 1,258 lncRNAs, 1,086 mRNAs, and 61 microRNAs differentially expressed in XX-DSD pigs compared with normal female pigs. Genes in the hormone biosynthesis and secretion pathway significantly up-regulated, and the up-regulation of GNRH1, KISS1 and AVP may associate with the abnormal secretion of GnRH. We also predicted the lncRNA-miRNA-mRNA co-expression triplets and constructed three competing endogenous RNA (ceRNA) potentially associated with XX-DSD. Functional enrichment studies suggested that TCONS_00340886, TCONS_00000204 and miR-181a related to GnRH secretion. Further, miR-181a inhibitor up-regulated GNRH1, PAK6, and CAMK4 in the GT1-7 cells. Conversely, transfection of miR-181a mimics obtained the opposite trends. The expression levels of FSHR, LHR, ESR1 and ESR2 were significantly higher in XX-DSD gondas than those in normal sows. Taken together, we proposed that the balance of endocrine had broken in XX-DSD pigs. The current study is the first to examine the transcriptomic profile in the hypothalamus of XX-DSD pigs. It provides new insight into coding and non-coding RNAs that may be associated with DSD in pigs.

SNP rs12794714 of CYP2R1 is associated with serum vitamin D levels and recurrent spontaneous abortion (RSA): a case-control study.

Purpose: Vitamin D (VD) deficiency seems to be associated with the risk of recurrent spontaneous abortion (RSA). Vitamin D receptor (VDR) and cytochrome P450 family 2 subfamily R member 1 (CYP2R1) are two genes which are vital for VD metabolism and actions. However, whether single-nucleotide polymorphisms (SNPs) in these genes are correlated with the risk of RSA are poorly understood. Therefore, we aimed to characterize the relationships among VDR SNPs, CYP2R1 SNPs and RSA.

Methods: This case-control study enrolled 75 RSA patients and 83 controls. Serum VD and some cytokines were detected with LC-MS/MS and flow cytometry, respectively. Genotyping for three SNPs of CYP2R1 (rs10741657, rs10766197 and rs12794714) and five SNPs of VDR (rs7975232, rs1544410, rs2189480, rs2228570 and rs2239179) was done with polymerase chain reaction (PCR) and high-throughput sequencing. All the data were analyzed with appropriate methods and in different models.

Results: The results revealed a significant correlation between the AG genotype of CYP2R1 rs12794714 and VD levels (OR 0.686; 95% CI 0.49-0.96; p = 0.028). Besides, the AG and GG genotypes of CYP2R1 rs12794714 were markedly related to the risk of RSA (OR 52.394, 59.497; 95% CI 2.683-1023.265, 3.110-1138.367; p = 0.009, 0.007, respectively).

Conclusion: Our results indicate that CYP2R1 rs12794714 might be a risk factor for RSA. Hence, early screening of pregnant women for CYP2R1 rs12794714 is necessary to warrant proactive counseling and treatment against RSA.

An efficient hydrodynamic-biokinetic model for the optimization of operational strategy applied in a full-scale oxidation ditch by CFD integrated with ASM2.

Oxidation ditches (ODs) are often used for wastewater treatment. However, limitations of ODs like high energy expenditure and increased sludge sedimentation hinder its wide application. In this study, the computational fluid dynamics (CFD) model integrated with Activated Sludge Model No. 2 (ASM2) was proposed and applied in a full-scale OD. The integrated model provided heterogeneous information on the characteristics of hydrodynamics and biokinetics of OD, especially with respect to the simulation of total phosphorus removal by CFD-ASM2 integration model for the first time. The simulated values of flow velocities, suspended solids (SS), dissolved oxygen, chemical oxygen demand, total nitrogen, ammonium nitrogen, and total phosphorus concentrations were well validated with the measured results, with the standard deviation errors of less than 5.56%, 0.28%, 0.74%, 7.39%, 3.17%, 5.27%, and 7.40%, respectively. Based on the integrated model, four different operational strategies were simulated. The proposed operational strategy of operating 7 surface aerators and 10 submerged impellers not only met the standard discharge requirements (GB 18918-2002) but also consumed less energy by 22.3%, compared with the original strategy of operating 9 surface aerators and 13 submerged impellers. Meanwhile, this proposed operational strategy also reduced the SS concentrations in the second and fourth channels, which was beneficial to elimination of sludge sedimentation. Moreover, the proposed operational strategy was successfully applied and validated in full-scale OD. The foregoing results collectively suggest that the CFD-AMS2 integration model is numerically capable to optimize the operational strategy of ODs.

Prolonged median distal sensory nerve action potential duration in carpal tunnel syndrome.

Objective: Routine nerve conduction study (NCS) parameters are less sensitive in the early stage of carpal tunnel syndrome (CTS). Recently, some studies have shown that prolonged distal sensory nerve action potential (DSNAP) duration may be a more sensitive technique for the diagnosis of demyelinating peripheral neuropathies. We aimed to evaluate the sensitivity of median DSNAP duration in patients with CTS.

Methods: DSNAP duration and routine NCS data of the median nerve were retrospectively collected in 173 CTS patients, 73 controls, and 78 cervical radiculopathy patients.

Results: Prolonged median DSNAP durations were found in 22 patients (22/35, 63%) and 36 patients (36/54, 67%) in the minimal and mild CTS groups respectively, which was more sensitive than routine NCS parameters. The percentage of patients demonstrating abnormalities in median NCS was significantly increased from 80% to 92% with the addition of DSNAP duration.

Conclusion: Our results demonstrate the electrodiagnostic value of median DSNAP duration for the diagnosis of CTS, especially in early cases.

Biomechanical properties of a novel nonfusion artificial vertebral body for anterior lumbar vertebra resection and internal fixation.

The aim of the study was to evaluate the biomechanical properties of a novel nonfused artificial vertebral body in treating lumbar diseases and to compare with those of the fusion artificial vertebral body. An intact finite element model of the L1-L5 lumbar spine was constructed and validated. Then, the finite element models of the fusion group and nonfusion group were constructed by replacing the L3 vertebral body and adjacent intervertebral discs with prostheses. For all finite element models, an axial preload of 500 N and another 10 N m imposed on the superior surface of L1. The range of motion and stress peaks in the adjacent discs, endplates, and facet joints were compared among the three groups. The ranges of motion of the L1-2 and L4-5 discs in flexion, extension, left lateral bending, right lateral bending, left rotation and right rotation were greater in the fusion group than those in the intact group and nonfusion group. The fusion group induced the greatest stress peaks in the adjacent discs and adjacent facet joints compared to the intact group and nonfusion group. The nonfused artificial vertebral body could better retain mobility of the surgical site after implantation (3.6°-8.7°), avoid increased mobility and stress of the adjacent discs and facet joints.

Visual response characteristics of neurons in the second visual area of marmosets.

The physiological characteristics of the marmoset second visual area (V2) are poorly understood compared with those of the primary visual area (V1). In this study, we observed the physiological response characteristics of V2 neurons in four healthy adult marmosets using intracortical tungsten microelectrodes. We recorded 110 neurons in area V2, with receptive fields located between 8° and 15° eccentricity. Most (88.2%) of these neurons were orientation selective, with half-bandwidths typically ranging between 10° and 30°. A significant proportion of neurons (28.2%) with direction selectivity had a direction index greater than 0.5. The vast majority of V2 neurons had separable spatial frequency and temporal frequency curves and, according to this criterion, they were not speed selective. The basic functional response characteristics of neurons in area V2 resemble those found in area V1. Our findings show that area V2 together with V1 are important in primate visual processing, especially in locating objects in space and in detecting an object's direction of motion. The methods used in this study were approved by the Monash University Animal Ethics Committee, Australia (MARP 2009-2011) in 2009.

Lead-Free BiFeO-BaTiO Ceramics with High Curie Temperature: Fine Compositional Tuning across the Phase Boundary for High Piezoelectric Charge and Strain Coefficients.

BiFeO-BaTiO is a promising high-temperature piezoelectric ceramic that possesses both good electromechanical properties and a Curie temperature (). Here, the piezoelectric charge constants () and strain coefficients (*) of (1 - )BiFeO-BaTiO (BF-BT; 0.20 ≤ ≤ 0.50) lead-free piezoelectrics were investigated at room temperature. The results showed a maximum of 225 pC/N in the BF-0.30BT ceramic and a maximum * of 405 pm/V in the BF-0.35BT ceramic, with s of 503 and 415 °C, respectively. To better understand the performance enhancement mechanisms, a phase diagram was established using the results of XRD, piezoresponse force microscopy, TEM, and electrical property measurements. The superb of the BF-0.30BT ceramic arose because of its location in the optimum point in the morphotropic phase boundary, low oxygen vacancy () concentration, and domain heterogeneity. The superior * of the BF-0.35BT ceramic was attributed to a weak relaxor behavior between coexisting macrodomains and polar nanoregions. The presented strategy provides guidelines for designing high-temperature BF-BT ceramics for different applications.

An Efficient Memristor-Based Circuit Implementation of Squeeze-and-Excitation Fully Convolutional Neural Networks.

Recently, there has been a surge of interest in applying memristors to hardware implementations of deep neural networks due to various desirable properties of the memristor, such as nonvolativity, multivalue, and nanosize. Most existing neural network circuit designs, however, are based on generic frameworks that are not optimized for memristors. Furthermore, to the best of our knowledge, there are no existing efficient memristor-based implementations of complex neural network operators, such as deconvolutions and squeeze-and-excitation (SE) blocks, which are critical for achieving high accuracy in common medical image analysis applications, such as semantic segmentation. This article proposes convolution-kernel first (CKF), an efficient scheme for designing memristor-based fully convolutional neural networks (FCNs). Compared with existing neural network circuits, CKF enables effective parameter pruning, which significantly reduces circuit power consumption. Furthermore, CKF includes the novel, memristor-optimized implementations of deconvolution layers and SE blocks. Simulation results on real medical image segmentation tasks confirm that CKF obtains up to 56.2% reduction in terms of computations and 33.62-W reduction in terms of power consumption in the circuit after weight pruning while retaining high accuracy on the test set. Moreover, the pruning results can be applied directly to existing circuits without any modification for the corresponding system.

The mental health of Chinese women with polycystic ovary syndrome is related to sleep disorders, not disease status.

Background: Mental health disorders are highly prevalent in polycystic ovary syndrome (PCOS) cases. The etiology for anxiety/depression in women with PCOS still remains unclear, due to conflicting results.

Aim: To examine whether an association exists between the mental health of Chinese women with PCOS and various indicators such as their disease characteristics, biochemistry results and sleep status.

Methods: During July 2018 and January 2020, our study included a total of 433 women diagnosed with PCOS at Chongqing Hospital of Traditional Chinese Medicine. Sleep-related variables were evaluated by the Pittsburgh Sleep Quality Index (PSQI), anxiety and depression values were quantified by the Hospital Anxiety and Depression Scale (HADS), and biochemistry results were collected from the medical records of the patients.

Results: 26.6% patients resulted as positive anxiety and 23.6% as positive depression. We found significant associations between anxiety/depression status and sleep conditions of PCOS patients. More specifically, anxiety significantly associated with sleep quality OR (95%CI) = 1.611 (1.147-2.261), sleep disturbance 2.326 (1.468-3.685) and daytime dysfunction 1.457 (1.122-1.891). Similarly, depression significantly associated with sleep quality 1.467 (1.043-2.063), sleep disturbance 1.624 (1.030-2.561) and daytime dysfunction 1.406 (1.077-1.836). There was no association detected between mental health and disease characteristics, as well as reproductive and metabolic indicators in PCOS.

Limitation: Cross-sectional nature of the data prevents causal associations, selection bias of a hospital-based population.

Conclusions: Sleep-related disorders might be involved in the etiology and development of the anxiety/depression observed in PCOS cases. We propose that management of sleep disorders should be an integral part of the disease management of women with PCOS.

Memristor-Based Edge Computing of Blaze Block for Image Recognition.

In this article, a novel edge computing system is proposed for image recognition via memristor-based blaze block circuit, which includes a memristive convolutional neural network (MCNN) layer, two single-memristive blaze blocks (SMBBs), four double-memristive blaze blocks (DMBBs), a global Avg-pooling (GAP) layer, and a memristive full connected (MFC) layer. SMBBs and DMBBs mainly utilize the depthwise separable convolution neural network (DwCNN) that can be implemented with a much smaller memristor crossbar (MC). In the backward propagation, we use batch normalization (BN) layers to accelerate the convergence. In the forward propagation, this circuit combines DwCNN layers/CNN layers with nonseparate BN layers, which means that the required number of operational amplifiers is cut by half as long as the greatly reduced power consumption. A diode is added after the rectified linear unit (ReLU) layer to limit the output of the circuit below the threshold voltage Vt of the memristor; thus, the circuit is more stable. Experiments show that the proposed memristor-based circuit achieves an accuracy of 84.38% on the CIFAR-10 data set with advantages in computing resources, calculation time, and power consumption. Experiments also show that, when the number of multistate conductance is 2⁸ and the quantization bit of the data is 8, the circuit can achieve its best balance between power consumption and production cost.

Prolonged Life Expectancy for Those Dying of Stroke by Achieving the Daily PM Targets.

This time-series study collects data on stroke-related mortality, years of life lost (YLL), air pollution, and meteorological conditions in 96 Chinese cities from 2013 to 2016 and proposes a three-stage strategy to generate the national and regional estimations of avoidable YLL, gains in life expectancy and stroke-related population attributable fraction by postulating that the daily fine particulate matter (PM) has been kept under certain standards. A total of 1 318 911 stroke deaths are analyzed. Each 10 µg m increment in PM at lag is associated with a city-mean increase of 0.31 (95% CI: 0.19, 0.44) years of life lost from stroke. A number of 914.11 (95% CI: 538.28, 1288.94) years of city-mean life lost from stoke could be avoided by attaining the WHO's Air Quality Guidelines (AQG) (25 µg m). Moreover, by applying the AQG standard, 0.11 (0.08, 0.15) years of life lost might be prevented for each death, and about 0.91% (95% CI: 0.62%, 1.19%) of the total years of life lost from stroke might be explained by the daily excess PM exposure. This study indicates that stroke patients can have a longer life expectancy if stricter PM standards are put in place, especially ischemic stroke patients.

Improvement in life expectancy for ischemic heart diseases by achieving daily ambient PM standards in China.

Background: The potential impacts of daily ambient fine particulate pollution (PM) exposure on year of life lost (YLL) due to ischemic heart diseases (IHD) remain uncertain. We aimed to estimate the improvement in IHD-related life expectancy by attaining the daily air quality standards of ambient PM in China.

Methods And Results: This study was based on daily mortality data covering 96 Chinese cities from 2013 to 2016. Regional- and national-associations between IHD-related YLLs and daily PM were estimated by generalized additive models. We further evaluated the IHD-related avoidable YLLs with an assumption that the daily PM was below the ambient air quality standards of World Health Organization (WHO) and China, and calculated the improvement of life expectancy by dividing the avoidable YLLs by the overall number of IHD mortality. We totally recorded 1,485,140 IHD deaths from 2013 to 2016. At the national level, we found a positive association between IHD-related YLLs and daily PM. Per 10 μg/m increment of four-day averaged ambient PM related to an increase of 0.40 IHD-related YLLs (95% CI: 0.28, 0.51). By achieving the WHO's air quality guideline, we estimated that an averaged number of 1346.94 (95% CI: 932.61, 1761.27) YLLs can be avoided for the IHD deaths in each city. On average, the life expectancy can be improved by 0.15 years (95% CI: 0.11, 0.19) for each death.

Conclusions: Our study provides a nationwide picture of the life expectancy improvements by reaching the daily PM standards in China, indicating that people can live longer in an environment with higher air quality.

Mental health and related influencing factors among rural elderly in 14 poverty state counties of Chongqing, Southwest China: a cross-sectional study.

Background: China has the largest elderly population in the world; little attention has been paid to the mental health of elderly in areas of extreme poverty. This is the first study to investigate the mental health of the rural elderly in poverty state counties in Chongqing and was part of the Chongqing 2018 health literacy promotion project.

Methods: In 2019, a cross-sectional study was conducted to investigate the mental health status of the rural elderly in fourteen poverty state counties of Chongqing, in which a total of 1400 elderly aged ≥ 65 years were interviewed, where mental health status was measured by the ten-item Kessler10 (K10) scale. Ordered multivariate logistic regression was performed to evaluate the influencing factors related to mental health of the elderly in these areas.

Results: The average score of K10 in 14 poverty state counties was 17.40 ± 6.31, 47.6% was labeled as good, 30.2% was moderate, 17.0% was poor, and lastly 5.1% was bad, and the mental health status of the elderly in the northeastern wing of Chongqing was better than the one in the southeastern wing of Chongqing. A worse self-rated health was the risk factor for mental health both in the northeastern and southeastern wings of Chongqing (all P < 0.001). Lower education level (OR (95% CI) = 1.45 (1.12-1.87), P = 0.004) was a risk factor in the northeastern wing, whereas older age (OR (95% CI) = 1.33 (1.13-1.56), P = 0.001) was a risk factors in the southeastern wing.

Conclusions: The results showed that mental health of the elderly in poverty state counties was poor, especially in the southeastern wing of Chongqing. Particular attention needs to be paid to the males who were less educated, older, and single; female with lower annual per capital income; and especially the elderly with poor self-rated health.

Comparison between atlas and convolutional neural network based automatic segmentation of multiple organs at risk in non-small cell lung cancer.

Delineation of organs at risk (OARs) is important but time consuming for radiotherapy planning. Automatic segmentation of OARs based on convolutional neural network (CNN) has been established for lung cancer patients at our institution. The aim of this study is to compare automatic segmentation based on CNN (AS-CNN) with automatic segmentation based on atlas (AS-Atlas) in terms of the efficiency and accuracy of OARs contouring.The OARs, including the lungs, esophagus, heart, liver, and spinal cord, of 19 non-small cell lung cancer patients were delineated using three methods: AS-CNN, AS-Atlas in the Pinnacle-software, and manual delineation (MD) by a senior radiation oncologist. MD was used as the ground-truth reference, and the segmentation efficiency was evaluated by the time spent per patient. The accuracy was evaluated using the Mean surface distance (MSD) and Dice similarity coefficient (DSC). The paired t-test or Wilcoxon signed-rank test was used to compare these indexes between the 2 automatic segmentation models.In the 19 testing cases, both AS-CNN and AS-Atlas saved substantial time compared with MD. AS-CNN was more efficient than AS-Atlas (1.6 min vs 2.4 min, P < .001). In terms of the accuracy, AS-CNN performed well in the esophagus, with a DSC of 73.2%. AS-CNN was better than AS-Atlas in segmenting the left lung (DSC: 94.8% vs 93.2%, P = .01; MSD: 1.10 cm vs 1.73 cm, P < .001) and heart (DSC: 89.3% vs 85.8%, P = .05; MSD: 1.65 cm vs 3.66 cm, P < .001). Furthermore, AS-CNN exhibited superior performance in segmenting the liver (DSC: 93.7% vs 93.6%, P = .81; MSD: 2.03 cm VS 2.11 cm, P = .66). The results obtained from AS-CNN and AS-Atlas were similar in segmenting the right lung. However, the performance of AS-CNN in the spinal cord was inferior to that of AS-Atlas (DSC: 82.1% vs 86.8%, P = .01; MSD: 0.87 cm vs 0.66 cm, P = .01).Our study demonstrated that AS-CNN significantly reduced the contouring time and outperformed AS-Atlas in most cases. AS-CNN can potentially be used for OARs segmentation in patients with pathological N2 (pN2) non-small cell lung cancer.

Antibiotic eye drops prescription patterns by orthokeratology practitioners in China and the development of antibiotic usage guidelines.

Purpose: To investigate antibiotic eye drops prescribing patterns of contact lens practitioners in Mainland China for orthokeratology (ortho-k) patients and to develop guidelines for their use.

Methods: A questionnaire on their antibiotic prescribing habits was administered to eye care practitioners (ECP) involved in ortho-k lens fitting nationwide via an official online account. Multiple logistic regression models were used to assess the relationships between prescribing habits and demographics.

Results: A total of 555 ECP completed the survey, of whom 50.5 % were optometrists (non-medical degree). About 30 % of the respondents routinely prescribed antibiotic eye drops for prophylactic use before and/or after commencement of ortho-k treatment. Forty one percent of the respondents dispensed antibiotics to patients for emergency use without giving written instructions, 48.5 % used antibiotic eye drops to wet fluorescein strips during ortho-k lens fitting, and 44.9 % considered treating corneal infiltrate(s) with antibiotics to be appropriate. A set of guidelines, including when to use antibiotic eye drops during ortho-k lens treatment and their proper use, stratified by different corneal conditions, was developed.

Conclusions: Inappropriate use of antibiotic eye drops appeared to be common among ortho-k practitioners in Mainland China. Taking into account deficiencies and errors identified from the survey, a set of guidelines based on best practice was developed and can serve to provide advice for proper use of antibiotics in ortho-k practice.

In-cell destabilization of a homodimeric protein complex detected by DEER spectroscopy.

The complexity of the cellular medium can affect proteins' properties, and, therefore, in-cell characterization of proteins is essential. We explored the stability and conformation of the first baculoviral IAP repeat (BIR) domain of X chromosome-linked inhibitor of apoptosis (XIAP), BIR1, as a model for a homodimer protein in human HeLa cells. We employed double electron-electron resonance (DEER) spectroscopy and labeling with redox stable and rigid Gd spin labels at three representative protein residues, C12 (flexible region), E22C, and N28C (part of helical residues 26 to 31) in the N-terminal region. In contrast to predictions by excluded-volume crowding theory, the dimer-monomer dissociation constant was markedly higher in cells than in solution and dilute cell lysate. As expected, this increase was partially recapitulated under conditions of high salt concentrations, given that conserved salt bridges at the dimer interface are critically required for association. Unexpectedly, however, also the addition of the crowding agent Ficoll destabilized the dimer while the addition of bovine serum albumin (BSA) and lysozyme, often used to represent interaction with charged macromolecules, had no effect. Our results highlight the potential of DEER for in-cell study of proteins as well as the complexities of the effects of the cellular milieu on protein structures and stability.

Development and validation of two artificial intelligence models for diagnosing benign, pigmented facial skin lesions.

Objective: This study used deep learning for diagnosing common, benign hyperpigmentation.

Method: In this study, two convolutional neural networks were used to identify six pigmentary diseases, and a disease diagnosis model was established. Because the distribution of lesions in the original training picture is very complex, we cropped the image around the lesions, trained the network on the extracted lesion images, and fused the verification results of the overall picture and the extracted picture to assess the model performance in identifying hyperpigmented dermatitis pictures. Finally, we evaluated the image recognition performance of the two convolutional neural networks and the converged networks in the test set through a comparison of the converged network and the physicians' assessments.

Results: The AUC of DenseNet-96 for the overall picture was 0.98, whereas the AUC of ResNet-152 was 0.96; therefore, we concluded that DenseNet-96 performed better than ResNet-152. From the AUC, the converged network has the best performance. The converged network model achieved a comprehensive classification performance comparable to that of the doctors.

Conclusions: The diagnostic model for benign, pigmented skin lesions based on convolutional neural networks had a slightly higher overall performance than the skin specialists.

Ambient air pollution exposure associated with glucose homeostasis during pregnancy and gestational diabetes mellitus.

Background: To investigate the effects of air pollution exposure during pregnancy on the indicators of glucose homeostasis and gestational diabetes mellitus (GDM).

Methods: We conducted a birth cohort study in Foshan, China during 2015-2019. Oral glucose tolerance test (OGTT) was administered to each participant during pregnancy. GDM was defined according to the International Association of Diabetes and Pregnancy Study Groups criteria (IADPSG). Air pollutant (fine particulate matter (PM), particulate matter with an aerodynamic diameter of 10 μm or less (PM), sulfate dioxide (SO), nitrogen dioxide (NO) and ozone (O)) concentrations from the air monitoring stations in Foshan were used to estimate individual air pollutant exposure during the first two trimesters. Linear and logistic regression models were employed to estimate the associations between air pollution exposure during the first two trimesters and OGTT glucose levels and GDM.

Results: Of 12,842 pregnant women, 3055 (23.8%) had GDM. A 10 μg/m increase in PM, PM and SO during trimester 1, trimester 2 and two trimesters were associated with 0.07 mmol/L to 0.29 mmol/L increment in OGTT-fasting glucose levels in single-pollutant model. A 10 μg/m increase in NO and O during two trimesters were associated with 0.15 mmol/L and 0.12 mmol/L decrease in OGTT-fasting glucose in single-pollutant model. However, no significant or weaker effects of O during two trimesters on OGTT-fasting glucose were observed in two-pollutant models. Moreover, exposure to PM, PM and SO were associated with increased risk of GDM in both single- and two-pollutant models.

Conclusions: Our study suggests PM, PM and SO exposure during the first two trimesters might increase the risk of GDM.

The effect of temperature on cause-specific mental disorders in three subtropical cities: A case-crossover study in China.

Background: Little is known about the association between ambient temperature and cause-specific mental disorders, especially in subtropical areas.

Objective: To investigate the effect of ambient temperature on mental disorders in subtropical cities.

Method: Daily morbidity data for mental disorders in three Chinese cities (Shenzhen, Zhaoqing, and Huizhou) were collected from medical record systems of local psychiatric specialist hospitals, covering patients of all ages. Case-crossover design combined with a distributed lag nonlinear model (DLNM) was used to assess the nonlinear and delayed effects of temperatures on five specific mental disorders (affective disorders, anxiety, depressive disorders, schizophrenia, and organic mental disorders), with analyses stratified by gender and age. The temperature of minimum effect was used as the reference value to calculate estimates.

Results: We observed inversed J-shaped exposure-response curves between temperature and mental morbidity and observed that low temperatures had a significant and prolonged effect on most types of mental disorders in the three cities. For example, the effect of the cold (2.5th percentile) on anxiety was consistently observed in the three cities with an odds ratio (OR) of 1.29 (95% CI: 1.06-1.57) in Zhaoqing, 1.26 (95% CI: 1.18-1.34) in Shenzhen, and 1.45 (95% CI: 1.17-1.81) in Huizhou. Low temperature was also associated with an increased risk of depressive disorders and schizophrenia. For the high temperature exposure (97.5th percentile), we only observed a significant, harmful effect on anxiety [OR = 1.30 (95% CI: 1.08, 1.58) in Shenzhen, OR = 1.16 (95% CI: 1.00, 1.34) in Zhaoqing], affective disorders [OR = 1.32 (95% CI: 1.08, 1.62) in Shenzhen], and schizophrenia [OR = 1.24 (95% CI: 1.03, 1.48) in Zhaoqing, OR = 1.03 (95% CI: 1.00, 1.06) in Huizhou].

Conclusions: Our study suggests that both low and high temperatures might be important drivers of morbidity from mental disorders, and low temperature may have a more general and wide-spread effect on this cause-specific morbidity than high temperature.

The mediation effect of maternal glucose on the association between ambient air pollution and birth weight in Foshan, China.

Maternal blood glucose level is associated with fetal growth, therefore, its role in the associations between air pollution and birth weight deserves investigation. We examined the mediation effect of maternal blood glucose on the associations between maternal air pollution exposure and birth weight. A total of 10,904 pregnant women in Foshan, China during 2015-2019 were recruited. Oral glucose tolerance test (OGTT) was administered to each participant after late trimester 2. Air pollution data at the monitoring stations in residential districts was used to estimate exposures of each participant during trimester 1 and trimester 2. Mixed-effects linear models were used to estimate the associations between air pollution and birth weight. After controlling for ten covariates, the direct effect of PM and SO (each 10 μg/m increment) on birth weight was -15.7 g (95% CI: -29.4, -4.8 g) and -83.6 g (95% CI: -134.8, -33.0 g) during trimester 1. The indirect effect of PM and SO (each 10 μg/m increment) on birth weight by increasing maternal fasting glucose level was 6.6 g (95% CI: 4.6, 9.1 g) and 22.0 g (95% CI: 4.1, 44.0 g) during trimester 1. Our findings suggest that air pollution might affect the birth weight through direct and indirect pathway, and the indirect effect might be mediated by maternal blood glucose.

Ubiquitin-specific protease as the underlying gene biomarker for aortic stenosis.

Background: Aortic stenosis is a common heart valvular disease whose pathological processes include an inflammatory reaction and lipid accumulation. However, its detailed pathogenesis is yet to be completely elucidated. Therefore, it is of great significance to further explore the molecular mechanisms of aortic stenosis.

Methods: Four datasets were downloaded from the Gene Expression Omnibus (GEO) database. Firstly, the differently expressed genes (DEGs) were screened between control and aortic stenosis samples. Secondly, weighted gene co-expression network analysis (WGCNA) was performed to find the highly relevant gene modules. Enrichment analysis and protein-protein interaction (PPI) networking were also performed, then Cytoscape was used to identify hub genes. Finally, the six participants (3 control participants and 3 patients with aortic stenosis) were recruited at the Tianjin Chest Hospital. In order to verify the expression level of USP14, several molecular experiments were performed, including hematoxylin-eosin (HE) staining, immunohistochemistry, immunofluorescence technology, real time-quantitative polymerase chain reaction (RT-qPCR), and western blotting.

Results: A total of 9636 DEGs were found between the control and aortic stenosis samples. The DEGs were mainly enriched in the autophagy-animal, cellular lipid catabolic process, apoptosis, and glycoside metabolic process categories. Eleven hub genes were identified via four different algorithms. Following verification of the patient samples, Ubiquitin-specific protease 14 (USP14) was found to be displayed at higher levels in the aortic stenosis samples.

Conclusion: USP14 might be involved in the occurrence and development of aortic stenosis, so it would be a molecular target for early diagnosis and specific treatment of aortic stenosis. There is a significant association between the high expression of USP14 and aortic stenosis, indicating that this gene may be a genetic risk factor for aortic stenosis.

Hematoporphyrin monomethyl ether photodynamic therapy for the treatment of facial port-wine stains resistant to pulsed dye laser.

Background: Pulsed dye laser is the first treatment choice for port-wine stains. However, as some facial port-wine stains are resistant to this modality, we evaluated the efficacy and safety of hematoporphyrin monomethyl ether (hemoporfin) photodynamic therapy for the treatment of such resistant port-wine stains.

Methods: Patients were treated with two sessions of hemoporfin photodynamic therapy in our department. Patients received an intravenous injection of hematoporphyrin monomethyl ether (5 mg/kg) followed by 532 nm LED green light therapy. Three physicians graded the improvement in the port-wine stain, using a 4-level scale. Patients' satisfaction, reaction to treatment, and adverse effects were evaluated.

Results: Thirty-one patients (mean age, 23.9 ± 11.9 years, range, 3-48 years) were enrolled in this study. Hypertrophic lesions accounted for 48.4% of port-wine stain, with 80.6% of lesions being larger than 40 cm. With regard to location, 41.9% were located on the central face and 32.3% involved a mix of the central and peripheral face. After one session, a treatment response was identified in 87.1% of cases, with the response deemed 'significant' in 29.0%. After two sessions, these rates increased to 100.0% and 61.3%, respectively. The clinical effect after two sessions was significantly greater than that after one session. Treatment reactions and adverse effects were well tolerated, and included pruritus, burning sensation, pain, edema, purpura-like change, blister, crust, and hyperpigmentation.

Conclusions: Hemoporfin photodynamic therapy is a promising treatment for port-wine stains resistant to pulsed dye laser therapy.

How longer can people live by achieving the daily ambient fine particulate pollution standards in the Pearl River Delta region, China?

Background: Previous research has reported the effects of long-term fine particulate matter (PM) pollution on years of life lost (YLL), but these effects may not represent the full impact. This study aims to estimate potential benefits in life time from adhering to daily ambient PM concentration standards/guidelines.

Methods: This study evaluated the relationship between daily ambient PM level and YLL using a two-stage approach with generalized additive models and meta-analysis. Potential life expectancy gains were then estimated by presuming that daily PM levels were in compliance with the Chinese and WHO standards. In addition, the attributable fraction of YLL due to excess PM exposure was also calculated.

Results: During 2013-2016, 459,468 non-accidental deaths were recorded in the six cities of Pearl River Delta, China. Each 10 μg/m increment in four-day average (lag) level of PM was related to an increment of 13.31 [95% confidence interval (CI): 5.74, 20.87] years of life lost. Implementation of the WHO guidelines might avoid 180,980.83 YLLs (95% CI: 78,116.07, 283,845.60), which corresponded to 0.39 (95% CI: 0.17, 0.62) years of increased life time per death. Additionally, an estimated 0.15% (95% CI: 0.06%, 0.23%) or 2.04% (95% CI: 0.88%, 3.20%) of YLLs could be attributed to PM exposures higher than the Chinese or WHO guidelines, respectively.

Conclusions: This study suggests that people might live longer by controlling daily PM concentration and highlights the need to adopt stricter standards in China.

Tracheoesophageal fistula associated with bevacizumab after thoracic radiotherapy in non-small cell lung cancer: A case report.

Introduction: Tracheoesophageal Fistula (TF) is a rare complication of Bevacizumab. Thoracic radiotherapy may be a contributing factor to TF formation. To the best of our knowledge, we report the first case of Chinese patient with non-small cell lung cancer (NSCLC) who developed TF after completion of chemotherapy with bevacizumab and thoracic radiotherapy.

Patient Concerns: A 54-year-old male patient was diagnosed with NSCLC. He received definitive thoracic radiotherapy with concurrent pemetrexed and cisplatin chemotherapy. Two months after the treatment, the disease progressed with enlargement of right inguinal lymph node and chemotherapy of docetaxel, carboplatin and bevacizumab was administrated. Eighteen days after 4 cycles, the patient presented a sudden onset of acute cough after drinking.

Diagnosis: Esophageal barium swallow revealed a TF. Gastroscopy confirmed a fistula in the esophagus.

Interventions: A jejunal feeding tube was placed for nutrition for a month. After that a covered esophageal stent was placed in the esophagus.

Outcomes: At the 6-month follow-up visit, the patient recovered well and had not developed any complication related to the stent placement.

Conclusion: TF is a rare but life-threatening complication of bevacizumab. Careful observation is imperative for those patients who are administered bevacizumab, particularly in patients treated previously with thoracic radiotherapy.

Identification of Potential Hub Genes of Atherosclerosis Through Bioinformatic Analysis.

Cardiovascular and cerebrovascular diseases, which mainly consist of atherosclerosis (AS), are major causes of death. A great deal of research has been carried out to clarify the molecular mechanisms of AS. However, the etiology of AS remains poorly understood. To screen the potential genes of AS occurrence and development, GSE43292 and GSE57691 were obtained from the Gene Expression Omnibus (GEO) database in this study for bioinformatic analysis. First, GEO2R was used to identify differentially expressed genes (DEGs) and the functional annotation of DEGs was performed by gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis. The Search Tool for the Retrieval of Interacting Genes (STRING) tool was used to construct the protein-protein interaction network and the most important modules and core genes were mined. The results show that a total of 211 DEGs are identified. The functional changes of DEGs are mainly associated with the cellular process, catalytic activity, and protein binding. Eighteen genes were identified as core genes. Bioinformatic analysis showed that the core genes are mainly enriched in numerous processes related to actin. In conclusion, the DEGs and hub genes identified in this study may help us understand the potential etiology of the occurrence and development of AS.

PARP-1-regulated TNF-α expression in the dorsal root ganglia and spinal dorsal horn contributes to the pathogenesis of neuropathic pain in rats.

Emerging evidence has implicated poly-(ADP-ribose) polymerase 1 (PARP-1), a transcriptional coregulator, in a variety of inflammatory diseases. In the current study, the role of PARP-1 in neuropathic pain and the underlying mechanisms were investigated. Neuropathic pain was determined by assessing the paw withdrawal threshold (PWT) and paw withdrawal latency (PWL) following lumbar 5 spinal nerve ligation (SNL) in male rates. Western blotting, qRT-PCR, immunohistochemistry, chromatin immunoprecipitation (ChIP), and Co-IP assays were performed to elucidate the mechanisms. The results showed that SNL resulted in a significant increase in the expression and activation of PARP-1 in the ipsilateral L4/5 dorsal root ganglia (DRG) and spinal dorsal horn, which occurred on day one, reached peak on day 7, and persisted more than 2 weeks after surgery. Double immunofluorescence staining revealed that PARP-1 was expressed exclusively in DRG A-type and C-type neurons. In the spinal cord, PARP-1 mainly colocalized with the neuronal marker NeuN and the astrocytic marker GFAP specifically in the superficial lamina. Prior intrathecal (i.t.) injection of PJ-34, a PARPs inhibitor, or Tiq-A, a specific PARP-1 inhibitor, dose-dependently prevented the reductions in PWT and PWL following SNL. Established neuropathic pain-like hypersensitivity was also attenuated with i.t. injection of PJ-34 and Tiq-A starting on day 7 following SNL, a timepoint at which neuropathic pain was fully established. SNL-induced mechanical allodynia and thermal hyperalgesia were also alleviated by i.t. injection of PARP-1 siRNA following a reduction in PARP-1 expression in the dorsal horn. Moreover, the SNL-induced increases in TNF-α protein and mRNA in the dorsal horn and DRG were dramatically suppressed by i.t. injection of Tiq-A or PARP-1 siRNA. The i.t. lipopolysaccharide (LPS)-induced increase in the production of TNF-α in the dorsal horn was also inhibited by prior to i.t. injection of PARP-1 siRNA. Results of ChIP assay showed that SNL-induced PARP-1 activation promoted the binding of NF-κB p65 with the TNF-α promoter in the dorsal horn and that PARP-1 inhibition reduced this binding and suppressed TNF-α expression. Co-IP assay revealed that SNL caused a significant increase in the level of histone H1 poly(ADP)-ribosylation. Together, these results indicate that PARP-1-regulated TNF-α expression in the DRG and spinal dorsal horn following SNL contributes to the development and maintenance of neuropathic pain. Targeting PARP-1 might be a promising therapeutic strategy for the treatment of the chronic pain.

Use of a Picosecond Alexandrite Laser for Treating Acquired Bilateral Nevus of Ota-Like Macules in Chinese Patients.

Background And Objectives: Acquired bilateral nevus of Ota-like macules (ABNOM) is common among the Chinese population. There have been few studies on the use of a picosecond alexandrite laser for the treatment of ABNOMs. The goal of this study was to evaluate the efficacy and safety of a 755 nm picosecond alexandrite laser for the treatment of ABNOMs.

Study Design/materials And Methods: A total of 225 subjects diagnosed with ABNOMs were included in this retrospective study. A 755 nm picosecond alexandrite laser system was used to treat all subjects. The safety and effectiveness of the laser were determined using a clinical photo assessment. The correlating factors of the patients, including the age, Fitzpatrick skin type, lesion location, lesion color, treatment procedures, melasma, and outcomes, were analyzed.

Results: The patients underwent one to four treatment sessions with clinical clearance (>90%) rates of 8.89%, 30.99%, 56.65%, and 60.00%, respectively. A higher number of sessions was positively associated with a better treatment response. The hyperpigmentation incidence rate was 22.67%, and one case of hypopigmentation was observed.

Conclusion: Our results suggest that a picosecond alexandrite laser is an effective and safe approach for treating ABNOMs in Chinese patients. Lasers Surg. Med. © 2020 Wiley Periodicals, Inc.

Clinical observation of soft palate-pharyngoplasty in the treatment of obstructive sleep apnea hypopnea syndrome in children.

Background: Childhood obstructive sleep apnea hypopnea syndrome (OSAHS) is a common clinical disease that can cause serious complications if not treated in time. The preferred treatment for OSAHS in children is surgery.

Aim: To observe the effects of soft palate-pharyngoplasty on postoperative outcome, pharyngeal formation, and possible complications.

Methods: A total of 150 children with snoring, hernia, and mouth breathing were selected. A polysomnography test was performed to confirm the diagnosis of OSAHS. The children were randomly divided into experimental and control groups. The experimental group underwent adenoidectomy, tonsillectomy, and soft palate-pharyngoplasty. The control group underwent adenoidectomy and tonsillectomy. The -test and chi-square test were used to compare conditions such as postoperative fever, postoperative hemorrhage, and pharyngeal reflux. Postoperative efficacy and complications were interrogated and observed in the form of outpatient follow-up and telephone follow-up at 6 mo and 1 year after surgery. The curative effects were divided into two groups: Cure (snoring, snoring symptoms disappeared) and non-cure.

Results: The effective rate of the experimental group was significantly higher than that of the control group, but the difference was not statistically significant ( > 0.05). The incidence of postoperative bleeding was lower in the experimental group. There was no postoperative pharyngeal reflux in either group. In the experimental group, the incidence of hyperthermia (body temperature exceeded 38.5 °C) was lower than that in the control group. The difference in postoperative swallowing pain scores between the experimental and control groups was significant.

Conclusion: Soft palate-pharyngoplasty can more effectively enlarge the anteroposterior diameter and transverse diameter of the isthmus faucium. Compared with surgery alone, it can better treat OSAHS in children, improve the curative effect, reduce the risk of perioperative bleeding, close the surgical cavity, reduce the risk of postoperative infection, reduce the proportion of postoperative fever, and accelerate healing. Although this process takes more time, it is simple, safe, and effective.

Inhibitory effects of msFGFR2c on the epithelial-to-mesenchymal transition of AE2 cells in pulmonary fibrosis.

In interstitial fibrosis, alveolar epithelial type II (AE2) cells fail to repair damaged epithelium. However, whether this dysfunction is related to fibroblast growth factor (FGF) signal pathway and how it affects the fibrotic process remains unclear. In our study, the medium of the human foetal lung fibroblast cell line MRC-5 (Med) can induce epithelial-to-mesenchymal transition (EMT) in AE2 cells, we also found that TGF-β in Med can induce FGF-2 and CTGF expression in AE2 cells. TGF-β or CTGF exposure trigger a FGFR2 subtype b to c transition which can be supressed by siRNA-CTGF. All together, since FGFR2IIIc have the highest affinity with FGF-2 in all of the FGFRs, we indicate the activation of FGF2 signal pathway was induced by TGF-β, which is the key component of Med Here, we also find the inhibitory effect of msFGFR2c (S252W mutant of soluble FGFR2IIIc extracellular domain) on EMT of mouse primary AE2 cells in pulmonary fibrotic process. In a bleomycin-induced mouse pulmonary fibrosis model, msFGFR2c alleviate pulmonary fibrosis and suppress the decrease in pro-SPC levels. Thus, msFGFR2c can inhibit EMT-induced fibrosis of AE2 cells via FGF-2 signal and AE2 cells is suggested to play an important role in the lung fibrotic process.
