Assessment of diabetes related distress among subjects with type 2 diabetes in South India.

Diabetes is a complex chronic disease that affects not only an individual’s physical health, but can also

have a profound impact on mental wellbeing. The aim of this study was to assess the prevalence of DRD

among subjects with type 2 diabetes (T2DM) using Diabetes Distress Scale-17 (DDS-17) and to see its

correlation with glycemic control and treatment modalities. A cross-sectional study of 400 T2DM

subjects (200 men and 200 women) aged between 25 to 65 years who visited the tertiary care centre for

diabetes in South India between April 2017 and May 2018 were included in this study. Subjects with

T1DM, gestational diabetes mellitus (GDM) and psychiatric illness were excluded. The total score of

DDS-17 was calculated by taking a sum of the 17 items’ results and then dividing the total by 17. Clinical

validation of the DDS suggests that the following thresholds of severity should be applied when

interpreting the scores: little or no distress < 2.0, moderate distress = 2.0 - 2.9, and high distress ≥ 3.0.

The mean age of men and women was 52.0 ± 8.4 and 51.7 ± 8.1 years, respectively. The mean score in

women was 2.79 ± 1.52 as compared to men (1.62 ± 0.83) (p<0.001). The study findings highlighted that

women had high levels of distress in managing diabetes as compared to men. Diabetes distress should

be considered as a significant health problem and steps should be taken for effective management by

lifestyle modifications, coping with their stress and diabetes.

Effect of Structured Behavior Intervention Program on Childhood Obesity

Background: Obesity is also attributed to environmental factors. Feasible and cost effective strategies to prevent childhood obesity are least explored in India. The purpose of the intervention is to evaluate the effectiveness of Comprehensive School Health Manual (CSHM) among children through a Training of Trainers programme for Central Board of Secondary Education (CBSE) school teachers. Methods: The study was designed as a school-based intervention study adopting a cluster randomized design to recruit students of 3rd, 7th and 11th classes from 7 schools. The intervention and control arm had three and four schools respectively. Pre and post interventional anthropometric and behavioral measure were collected using standard methods. Results: The overall prevalence of obesity was 12.6%. There was no difference in the incidence of obesity (3.7% vs. 3.8%) at the end of study period. Among the participants who had a favorable shift in nutritional grade, 65.4% were in intervention arm. There was a significant reduction in BMI z score (-0.16) and body fat percentage (-2.02) in the experiment group. Both the group reduced their calorie intake but it was significantly lower in intervention arm -381.9 (95% CI -428.2 to -335.4) Conclusion: A structured behavior intervention delivered through a Training of Trainers (ToT) empowered children to adopt healthy behaviors towards weight management. It is crucial to sustain these behaviors for positive impact.

Clinical significance of urinary liver-type fatty acid binding protein at various stages of nephropathy

This cross-sectional study was to evaluate the levels of urinary liver-type fatty acid binding protein (u-LFABP pg/mg urine creatinine ratio) at different stages of diabetic nephropathy and to see its correlation with other clinical parameters in South Indian patients with type 2 diabetes mellitus (T2DM). A total of 65 (M: F; 42:23) T2DM subjects were divided into three groups, and were compared with 13 (M: F; 3:10) nondiabetic controls. The study groups were as follows: normoalbuminuric (n = 22), microalbuminuric (n = 22) and macroalbuminuric (n = 21). Estimated glomerular filtration rate (eGFR) was calculated using Cockcroft and Gault formula. u-LFABP levels in spot urine samples were measured with a solid phase enzyme linked immunosorbent assay. This study showed that u-LFABP levels were undetectable in healthy controls and was very low in the normoalbuminuric subjects. Elevated levels of u-LFABP are evident from the microalbuminuric stage indicating tubular damage. The levels of u-LFABP increased gradually with declining renal function. Geometric mean (95% confidence interval) for normoalbuminuria was 0.65 (0.47-0.97), microalbuminuria was 0.99 (0.55-1.97) and macroalbuminuria was 5.16 (1.8-14.5), (P = 0.005). In conclusion, u-LFABP levels were elevated in patients with reduced eGFR and showed a positive correlation with systolic blood pressure and protein to creatinine ratio in the total study subjects
