Background: Cancer survivors represent a growing population with complex symptoms, inter-related medical/psychosocial profiles, and unique health care needs. Aims: Posttreatment follow-up represents a crucial aspect of quality survivor care; however, data are sparse regarding content and experiences of follow-up care, impact of provider involvement, and perceived quality of care amongst diverse long-term survivors diagnosed as adults. This study aimed to address these gaps. Methods: Questionnaires were mailed to breast, prostate, colorectal, endometrial, and ovarian cancer survivors, between 5 and 14 years postdiagnosis, in a population-based study sampled from the California SEER cancer registries. Items were tested cognitively and piloted prior to initiating the final survey. Results: Survivors reported five medical conditions (average) ever diagnosed (95% CI, 4.8-5.1) and 1.9 conditions (95% CI, 1.8-2.0) post-cancer. Most (n = 1490) reported cancer-related follow-up care in past 2 years (68.7%) by oncologist only (47.4%) or shared oncologist plus primary care providers (PCPs) (27.6%). Most survivors were advised to receive follow-up care (79.9%), but few were advised about late effects (41.7%) or received treatment summaries (19.9%). PCP-only care was rated lower in quality (OR: 0.34; 95% CI, 0.13-0.91) compared to oncologist-only care. Conclusions: Most long-term survivors received care from oncology specialists. Recent guidelines that PCPs alone should lead/provide survivorship care may need reassessing, or specific PCP training. For many long-term survivors care lacks critical aspects, e.g. long-term/late effects. Identifying one main provider was key. These are important findings and should shape future survivor care.

Background: Most individuals diagnosed with early stage cancers can expect to survive beyond 5 years due to successful treatment/early detection strategies. They are also at risk of long-term/late effects of cancer/its treatment, or comorbidities, especially as time increases from initial cancer diagnosis. Lifestyle health behavior interventions (LHBI) are important in targeting adverse health outcomes. Unclear is how effective they are among long-term survivors or in ameliorating late adverse health outcomes. Aims: To review the literature over the past 10 years and assess whether there is a paucity of LHBIs addressing adverse health outcomes among long-term survivors, or their ability to ameliorate survivors adverse health outcomes. Methods: A systematic review of LHBI's among long-term survivors (5 years beyond cancer diagnosis, 2006-2017). All LHBIs purporting to address adverse health outcomes/comorbidities among survivors were included. Identified were 162 published papers of which only 9 met inclusion criteria. Article references yielded 7 additional papers (secondary analyses of the 9 studies). Results: LHBI's addressing long-term survivors are rare (9/162 or 5.55%). Long-term or late-effects among survivors are rarely addressed. LHBIs were successful but only among survivors with good function, good overall health, or physically active. Conclusions: Critical gaps in research include the serious paucity of research among long-term survivors; sparse data on ability of LHBIs to ameliorate late/long-term adverse health outcomes, and their impact on survivors in poor health.
