Automatisierte OP-Leuchtenjustierung mittels 3D-Motion-Tracking

Technisches Problem:
Mit der Ausrichtung von OP-Lampen soll das Operationsgebiet möglichst homogen werden. Bisher wurden die Lampen manuell auf das Operationsgebiet zentriert, aufgrund Änderungen dieser Gebiete im OP-Verlauf, ergeben sich zwischenzeitlich jedoch nur suboptimale Ausleuchtungen.

Lösung des Problems:
Das Operationsgebiet soll mittels 3D-Lokalisierung und automatischer Ausrichtung der OP-Leuchten automatisch optimal und ausgeleuchtet werden.
Setup: Lampen an Roboter-Armen/Lampenfeld, Feld- u. Tiefenausleuchtung, s. u. Ausrichtung wird mittels 3D-Lokalisierung (3D- und Farbkamera) errechnet.

Funktion:
1. Der Operationstisch (F1), dessen Ausrichtung (V1) sowie die des Patienten (V2) wird erkannt. Diese werden als Parallellinie (L) bezeichnet. Die niedrigste (L) ist die Mitte des Tisches.
2. Nun Suche des nicht-abgedeckten Bereiches (F2) innerhalb F1 (Surface-Detection). Ausrichten der Lichtkegel auf F2. Es wird eine möglichst niedrige Parallellinie (L) und möglichst niedriger Ring (R) gewählt. Bei Störobjekten wird eine Hemisphäre (H1) um die Zielfläche gebildet und die Lampen entlang der H1-Linien und unter Berücksichtigung der Position der anderen Lampe in den nächsten sinnvollen Kreissektor bewegt.

Vorteile: Reduktion störender Unterbrechungen der OP und Gewährleistung einer durchgehenden Optimalausleuchtung.

Anwendungsgebiet: OP-Leuchtenjustierung

Schmerzmanagement bei Kindern in der Schweiz

MONATSSCHR KINDERH

Objective
Pain in hospitalized children is often undertreated. The aim of the study was to assess the current pain management for children in Switzerland.

Study design
A postal questionnaire was sent to all pediatric units in Switzerland.

Result
A total of 27 of 45 units replied (60% response). Most units used tools for pain assessment (96%) and had a guideline for pain management (78%). Procedural and postoperative pain are always (100%) treated. Premature infants and children in intensive care units often (> 87%) receive analgesics during invasive procedures. However, only 44% of intensive care units have a guideline for this.

Conclusion
Benefits of an effective pain management in children are well established. Many measures have been applied in Switzerland. However, there is still room for improvement, e.g., less than half of all intensive care units rely on guidelines for analgesics during invasive procedures.
