Acute Kidney Injury Following Dermatomyositis.

Clinical medicine & research

Renal involvement in patients with inflammatory myopathies, like dermatomyositis, is rare. We present the case of a 66-year-old lady that arrived at the hospital with acute renal failure. She had a diffuse erythematous rash, severe muscle weakness and decreased motor capacity. Inflammatory features of the skin/muscle biopsy along with the dramatic improvement in symptomatology upon the intake of steroids pointed towards dermatomyositis. Recognition of this pattern of events, adopting a multidisciplinary approach, early diagnosis and steroid treatment are necessary for better patient outcomes.

Musculoskeletal lower limb injuries in Major League Baseball.

Physical therapy in sport : official journal of the Association of Chartered Physiotherapists in Sports Medicine

OBJECTIVES:To explore the epidemiological patterns and musculoskeletal characteristics of lower limb injuries in Major League Baseball (MLB). DESIGN:Descriptive epidemiological study. SETTING:The MLB was screened for injuries between 2010 and 2016, inclusive. PARTICIPANTS:Players that were placed on the Disabled List (DL) and had lower limb injuries. MAIN OUTCOME MEASURES:Our database entailed the player's position, injury location, injury cause, and time spent on the DL. Analysis of characteristics and mean incidence rates (MIR) were conducted, and P values<0.05 were considered significant (CI 95%). RESULTS:A total of 3546 injuries were recorded; 1053 injuries (29.7%) affected the lower limbs (n = 1053). On average, 150.4 players per season were placed on the DL for a mean of 45.7 days. Thighs were the most common site of injury, whereas hip injuries had the highest average days on DL (62 days). Fielders had a significantly higher MIR of lower leg injuries when compared to pitchers (P = 0.007) and catchers (P = 0.024), and a significantly higher MIR of foot injuries than that of pitchers (P = 0.035). April was the month with the highest number of injuries (224 injuries, 21.3%). Only 43(4%) injuries required surgical intervention. CONCLUSION:Lower limb injuries are increasing in the MLB. Understanding the neuromuscular patterns of injuries will help in extrapolating better prevention policies.

Prevalence and patterns of shoulder injuries in Major League Baseball.

The Physician and sportsmedicine

Background: This study aims to explore the epidemiological patterns of shoulder injuries in professional baseball. Methods: The transaction lists of the 'Major League Baseball' website were screened for injuries from 2011 to 2016, inclusive. Only players that were placed on the 'Disabled List' were included in the study. Our database included the player's position, injury location, injury cause, and time spent on the Disabled List. Results: A total of 3090 injuries were recorded; 511 (17%) were shoulder injuries, making it the most vulnerable anatomic location in baseball (N = 511). Of the 511 shoulder injuries, 132 (25.8%) were due to inflammation, making it the most common cited cause of shoulder injury. Most shoulder injuries occurred in April with 125 (24%) injuries, while June and May came in second and third with 89 (17%) and 81 (16%) injuries, respectively. The pitcher was the player most prone to injury, as it constituted 78% of the injuries. The average time spent off by a shoulder injury was 69 days, which implicated substantial economic losses for the injured athlete's club. Conclusion: Shoulder injuries are prevalent in professional baseball. The overhead pitching motion puts the shoulder at risk and accounts for the high incidence of its injuries.

Diagnostic Clinical Trials in Breast Cancer Brain Metastases: Barriers and Innovations.

Clinical breast cancer

Optimal treatment of breast cancer brain metastases (BCBM) is often hampered by limitations in diagnostic abilities. Developing innovative tools for BCBM diagnosis is vital for early detection and effective treatment. In this study we explored the advances in trial for the diagnosis of BCBM, with review of the literature. On May 8, 2019, we searched ClinicalTrials.gov for interventional and diagnostic clinical trials involving BCBM, without limiting for date or location. Information on trial characteristics, experimental interventions, results, and publications were collected and analyzed. In addition, a systematic review of the literature was conducted to explore published studies related to BCBM diagnosis. Only 9 diagnostic trials explored BCBM. Of these, 1 trial was withdrawn because of low accrual numbers. Three trials were completed; however, none had published results. Modalities in trial for BCBM diagnosis entailed magnetic resonance imaging (MRI), computed tomography (CT), positron emission tomography (PET), PET-CT, nanobodies, and circulating tumor cells (CTCs), along with a collection of novel tracers and imaging biomarkers. MRI continues to be the diagnostic modality of choice, whereas CT is best suited for acute settings. Advances in PET and PET-CT allow the collection of metabolic and functional information related to BCBM. CTC characterization can help reflect on the molecular foundations of BCBM, whereas cell-free DNA offers new genetic material for further exploration in trials. The integration of machine learning in BCBM diagnosis seems inevitable as we continue to aim for rapid and accurate detection and better patient outcomes.

Cluster bombs are detrimental weapons of extensive destruction on environmental, biological and psychosocial levels. However, the broader effects of sub-munitions’ presence on postwar recovery remain largely unexplored. A prospective study was performed in Lebanon to analyze and interpret the effect of sub-munitions on multiple environmental and psychosocial stages for the devastation wrought by them on local populations is severe. The study showed that 42% of South Lebanon was infected with sub-munitions. 92% of these areas are green areas covered with plantations and/or vegetations. These infected areas included cultural and historic monuments of great importance. A theory of “land detachment” is proposed to explain the way sub-munitions affect the biological and social qualities of the infected land. Cluster bombs are a main cause in the desensitization from one’s homeland. Laws and legislations should be passed to prohibit the use of such damaging weapons.
