Auditory Processing Disorder Test Battery in European Portuguese-Development and Normative Data for Pediatric Population.

There is an increasing need for state-of-the-art Central Auditory Processing assessment for Portuguese native speakers, applicable as early as possible. As a contribution to answering this need, this paper presents a new battery for Central Auditory Processing assessment for European Portuguese applicable to children aged 5 and above, named BAPA-PE, providing information regarding test selection and development. The battery consists of six behavioral tests: Staggered Spondaic Words (SSW) for European Portuguese, Filtered Speech, Speech in Noise, Detection Interval in Noise, Duration, and Frequency Pattern. The normative data for children aged 5 to 12 are also reported. A sample was obtained of 217 subjects without ear pathology and with typical development. Each age group was composed of at least 30 children. All children were evaluated using pure tone audiometry, speech audiometry, impedance, and otoacoustic emissions. Normative scores are reported for each of the six auditory processing tests. The assessment is applicable to young children (aged 5 and 6). The statistical analyses showed significant effects in scores of Age for all tests and of Ear for several tests. The main result from the work presented, the Auditory Processing Assessment Battery-European Portuguese (BAPA-PE), is available for clinical use with normative data. This battery is a new tool for behaviorism assessment of European Portuguese speakers with suspected central auditory pathology and for monitoring the results of auditory training.

Estimated effect of increased diagnosis, treatment, and control of diabetes and its associated cardiovascular risk factors among low-income and middle-income countries: a microsimulation model.

Background: Given the increasing prevalence of diabetes in low-income and middle-income countries (LMICs), we aimed to estimate the health and cost implications of achieving different targets for diagnosis, treatment, and control of diabetes and its associated cardiovascular risk factors among LMICs.

Methods: We constructed a microsimulation model to estimate disability-adjusted life-years (DALYs) lost and health-care costs of diagnosis, treatment, and control of blood pressure, dyslipidaemia, and glycaemia among people with diabetes in LMICs. We used individual participant data-specifically from the subset of people who were defined as having any type of diabetes by WHO standards-from nationally representative, cross-sectional surveys (2006-18) spanning 15 world regions to estimate the baseline 10-year risk of atherosclerotic cardiovascular disease (defined as fatal and non-fatal myocardial infarction and stroke), heart failure (ejection fraction of <40%, with New York Heart Association class III or IV functional limitations), end-stage renal disease (defined as an estimated glomerular filtration rate <15 mL/min per 1·73 m or needing dialysis or transplant), retinopathy with severe vision loss (<20/200 visual acuity as measured by the Snellen chart), and neuropathy with pressure sensation loss (assessed by the Semmes-Weinstein 5·07/10 g monofilament exam). We then used data from meta-analyses of randomised controlled trials to estimate the reduction in risk and the WHO OneHealth tool to estimate costs in reaching either 60% or 80% of diagnosis, treatment initiation, and control targets for blood pressure, dyslipidaemia, and glycaemia recommended by WHO guidelines. Costs were updated to 2020 International Dollars, and both costs and DALYs were computed over a 10-year policy planning time horizon at a 3% annual discount rate.

Findings: We obtained data from 23 678 people with diabetes from 67 countries. The median estimated 10-year risk was 10·0% (IQR 4·0-18·0) for cardiovascular events, 7·8% (5·1-11·8) for neuropathy with pressure sensation loss, 7·2% (5·6-9·4) for end-stage renal disease, 6·0% (4·2-8·6) for retinopathy with severe vision loss, and 2·6% (1·2-5·3) for congestive heart failure. A target of 80% diagnosis, 80% treatment, and 80% control would be expected to reduce DALYs lost from diabetes complications from a median population-weighted loss to 1097 DALYs per 1000 population over 10 years (IQR 1051-1155), relative to a baseline of 1161 DALYs, primarily from reduced cardiovascular events (down from a median of 143 to 117 DALYs per 1000 population) due to blood pressure and statin treatment, with comparatively little effect from glycaemic control. The target of 80% diagnosis, 80% treatment, and 80% control would be expected to produce an overall incremental cost-effectiveness ratio of US$1362 per DALY averted (IQR 1304-1409), with the majority of decreased costs from reduced cardiovascular event management, counterbalanced by increased costs for blood pressure and statin treatment, producing an overall incremental cost-effectiveness ratio of $1362 per DALY averted (IQR 1304-1409).

Interpretation: Reducing complications from diabetes in LMICs is likely to require a focus on scaling up blood pressure and statin medication treatment initiation and blood pressure medication titration rather than focusing on increasing screening to increase diabetes diagnosis, or a glycaemic treatment and control among people with diabetes.

Evaluation of Design, Metallurgy, Microhardness and Mechanical Properties of Glide Path Instruments: a Multimethod Approach.

Introduction: This study aims to compare the design, metallurgy, microhardness and mechanical properties of 3 glide path NiTi instruments.

Methods: A total of 132 ProGlider, Edge Glide Path and R-Pilot instruments (44 per group) were selected. Design was assessed throughout stereomicroscopy (blades, helical angle, measuring lines, deformation) and scanning electron microscopy (symmetry, cross-section, tip, surface finishing). NiTi ratios were measured by energy-dispersive X-ray spectroscopy and phase transformation temperatures by differential scanning calorimetry. Microhardness and mechanical performance (torsion, bending and buckling resistance tests) were also evaluated. Statistical analyses were performed with Mood's median test with a significance set at 5%.

Results: Edge Glide Path had the lowest number of blades and R-Pilot the greatest helical angle. All instruments had an almost equiatomic NiTi ratio, while showing different cross-sections and tip geometries. Edge Glide Path had the smoother surface finishing. R-Pilot showed martensitic characteristics at room temperature, while mixed austenite plus R-phase was observed in the others instruments. R-Pilot had higher results on the microhardness (436.8 HVN), maximum torsion (0.9 N.cm) and buckling load (0.7 N) tests (P<.05), while Edge Glide Path had superior angle of rotation (683.5º) and ProGlider was more flexible (144.1 gf) (P<.05).

Conclusions: Differences in the instruments' design and phase transformation temperatures accounted for their mechanical behavior. R-Pilot showed the highest torque, buckling and microhardness, while ProGlider instrument was the most flexible.

Constraints on the Initial State of Pb-Pb Collisions via Measurements of Z-Boson Yields and Azimuthal Anisotropy at sqrt[s_{NN}]=5.02  TeV.

The CMS experiment at the LHC has measured the differential cross sections of Z bosons decaying to pairs of leptons, as functions of transverse momentum and rapidity, in lead-lead collisions at a nucleon-nucleon center-of-mass energy of 5.02  TeV. The measured Z boson elliptic azimuthal anisotropy coefficient is compatible with zero, showing that Z bosons do not experience significant final-state interactions in the medium produced in the collision. Yields of Z bosons are compared to Glauber model predictions and are found to deviate from these expectations in peripheral collisions, indicating the presence of initial collision geometry and centrality selection effects. The precision of the measurement allows, for the first time, for a data-driven determination of the nucleon-nucleon integrated luminosity as a function of lead-lead centrality, thereby eliminating the need for its estimation based on a Glauber model.

Oral lesions associated with COVID-19: A systematic review.

Background: Oral lesions has been reported in coronavirus disease 2019 (COVID-19). However, there are gaps about the occurrence. The aim of this study was to describe the clinical characteristics of oral lesions in patients with COVID-19.

Methods: A systematic review was conducted in four electronic databases (PubMed, Lilacs, Cochrane and Google Scholar) with the search terms: "coronavírus", "COVID-19", "SARS-CoV-2", "novel coronavírus", "2019nCoV", "oral manifestation", "mucosal lesions", "oral lesions", "mucosa viral lesions" and "oral conditions". Articles were limited to those published between january and july 2020, and in English.

Results: Initially, 182 articles were found, after reading the titles and abstracts a total of 24 records were included for full-text reading. Finally, 22 articles were included for data extraction and assessement. We identified and subsequently discussed clinical characteristics and differential diagnosis.

Conclusions: The etiology of many oral mucosal lesions is intimately related to infectious, immunological, and psychosocial factors. Therefore, amid the SARS-CoV-2 pandemic in which many doubts about the disease still remain, caution is needed when evaluating patients and establishing correlations with other diseases that could be associated.
