[Iliac artery reconstruction in patients with chronic critical limb ischemia: endarterectomy or replacement].

Objective: To evaluate the effectiveness and postoperative morbidity after endarterectomy from the iliac arteries in comparison with ilio-femoral and aorto-femoral replacement in patients with chronic critical lower limb ischemia.

Material And Methods: There were 95 patients with chronic critical lower limb ischemia. Iliac artery replacement was carried out in 43 (45.3%) patients, endarterectomy from the iliac arteries - in 52 (54.7%) cases. Trophic disorders on the lower extremities occurred in 36 (37.9%) patients.

Results: Iliac artery replacement was accompanied by postoperative wound suppuration and infection of prosthesis in 3 (6.9%) patients, postoperative ventral hernia in 2 (4.7%) patients. The total number of complications specific for endarterectomy (iliac artery perforation - 1 (1.9%) patient, iliac artery thrombosis - 3 (5.8%) patients) was similar to the literature data. Successful endarterectomy was performed in 52 (88.1%) patients. Iliac segment patency was similar in both groups throughout a 12-month follow-up period.

Conclusion: Endarterectomy from the iliac arteries has some advantages over replacement and should be preferred for trophic complications on the lower extremities.

[Surgical treatment of a patient with false aneurysm of the ulnar artery].

Today, the number of endovascular procedures is steadily growing. For example, there were 40.005 endovascular interventions in 2018. Most patients are discharged within 1-2 days after intervention due to its effectiveness and minimal invasiveness. However, physicians do not always note local postoperative complications. Multidisciplinary approach in follow-up and treatment of such patients is important. In particular, we talk about awareness of doctors of polyclinics about possible complications after arterial puncture. Effectiveness and favorable result of surgical treatment of local postoperative complications after arterial puncture are reported in the manuscript.

[Persistence of Epstein-Barr virus antigens in gastric cancer: characteristic of inflammatory cellular reactions in tumor].

The Aim Of The Study: Is to establish the relationship between the persistence of viral antigens of the Epstein-Barr virus (EBV) and the cellular composition of the immune microenvironment of tumor tissue and the mucous membrane of peritumoral area in gastric cancer.

Material And Methods: We used samples of surgical material from 55 patients with a verified diagnosis of gastric cancer. The expression of CD4, CD8, CD68, CD1a and LMP-1 was assessed. The results were assessed by the morphometric method. We selected three fields of view (magnification x200) in tumor tissue and in peritumoral areas separately and counted an absolute number of cells with positive staining with further calculation of the average number of cells and the median.

Results: LMP-1-negative tumors with LMP-1 expression in epithelium of peritumoral area were characterized by the highest density of CD4+ lymphocyte infiltration in the central part of the tumor; the highest density of CD8+ lymphocyte infiltration in the mucous membrane of peritumoral area (=0.0190); the highest density of infiltration by macrophages in the mucous membrane of peritumoral area (=0.2492); the highest density of infiltration by CD1a+ cells in the mucous membrane of peritumoral area (=0.1503). The highest density of infiltration with CD1a+ cells was characteristic for LMP-1-positive and LMP-1-negative tumors (=0.0813). The persistence of the LMP-1 viral antigen in the glandular epithelium of the peritumoral area in our sample does not have a statistically significant effect on the prognosis of the disease (RR=1.7718; =0.0885) but there is a tendency towards a negative predictive value.

Conclusion: High density of infiltration of glandular epithelium of peritumoral area with the expression of LMP-1 by CD4+ and CD8+ lymphocytes is most likely associated with the activation of the cellular immune response and may be one of the signs of the persistence of viral antigens. It was shown for the first time that the phenomenon of persistence of the LMP-1 viral antigen is characterized by a trend towards negative predictive value for patients with gastric cancer.

The Structure of Dipolar Polymer Brushes and Their Interaction in the Melt. Impact of Chain Stiffness.

By using the numerical lattice Scheutjens-Fleer self-consistent field (SF-SCF) method we have studied the effect of the restricted flexibility of grafted chains on the structure and mutual interaction of two opposing planar conventional and A-type dipolar brushes. Brushes are immersed in the solvent consisting of chains similar to the grafted ones. The increase of the chain rigidity enhances the segregation of grafted chains in a A-type dipolar brush into two populations: backfolded chains with terminal monomers near the grafting surface and chains with the ends at the brush periphery. The fraction of backfolded chains grows by an increase of the Kuhn segment length. It is shown that two opposite A-type dipolar brushes from semi-rigid chains are attracted to each other at short distances. The attraction becomes more pronounced and begins at larger distances for more rigid chains with the same brush characteristics: polymerization degree, grafting density, and dipole moments of monomer units. This attraction is connected with the dipole-dipole interactions between chains of oncoming brushes with oppositely directed dipoles penetrating deeply into each other upon contact. This effect of the chain rigidity is opposite to that for conventional brushes without dipoles in the chains. For such brushes, an increase in the chain rigidity leads to the enhanced repulsion between them.

One of the main duties of the Russian Federation is to respect and protect the rights and freedoms of its citizens. In addition, the Russian Federation as a social state the activities of which are designed to provide a decent life and a high level of social protection for all categories of the citizens, without exception, by regulating social, economic and other spheres of society provides social support to all segments of the population, paying special attention to certain categories of citizens which, due to life circumstances or health conditions, need social assistance and special support from the state. The main massif in the category of socially unprotected layers of the population are people with physical disabilities. Comprehensive support for the above category of citizens is carried out, among other things, based on the decision of the medical and social examination. However, the current situation has greatly complicated the on-site examination of disabled people and predetermined the correspondence order as the only possible mechanism for medical and social examination during the coronavirus pandemic. Thus, in fact, conditions close to emergency sent medical and social expertise to the path of telemedicine.
