Professor Gianluca Franceschini is top-level senior physician at the Breast Unit, relating to the Department of Women’ and Children’s Health, Catholic University, “Agostino Gemelli” Hospital, Rome, Italy since 1 june 2006.
He heads the Unit for integrated therapies for breast cancer at the Department of Women’ and Children’s Health, Catholic University, “Agostino” Gemelli Hospital, Rome since 1st July 2014.
He is an Associate Professor in General Surgery since January 2017 at the Catholic University of Rome.
He is lecturer at the PhD School “Technological Innovations in the Integrated Therapies of Breast Tumours” (2010-2013) and at the School of Specialization in General Surgery, Plastic Surgery and Oncology at the Catholic University of Rome (2011-2019).
He is academic Coordinator for the University Masters Degree Course in “Endocrinology and Breast Pathology” at the Catholic University of Rome (2014-2019).
He is reviewer and member of Editorial Board for several scientific journals.
On the 15th February 2016 he was appointed Permanent Member of the College of Italian Breast Surgeons.
He has authored numerous scientific articles, books, and book chapters. His present interests include therapy for pathologies of the breast and surgical treatment of breast cancer with oncoplastic techniques.

OncotypeDX® predictive nomogram for recurrence score output: A machine learning system based on quantitative immunochemistry analysis - ADAPTED01

Background: OncotypeDX (ODXVR ) can enhance prediction of breast cancer recurrence, guiding adjuvant treatment options. However, the opportunity to access this test is not always possible. The aim of this study is to investigate the correlation between phenotypical tumor characteristics, quantitative classical immunohistochemistry (IHC) and recurrent score (RS) resulting from ODXVR .Methods: All breast cancer patients who underwent ODXVR between 2014 and 2018 were retrospectively included in the study. The data selected for analysis were age, menopausal status, pathological and IHC features. IHC was performed with standardized quantitative methods. Dataset was split into two subsets (70% for training and 30% for internal validation). Logistic models were built with statistically significant features for predicting RS25 or20. An external validation set, provided by another center, was used to test reliability of prediction models.Results: The internal dataset included 407 patients (Table) who underwent ODXVR . Mean age was 53.7 (31-80) and 222 patients (54.55%) were > 50 years old. ODXVR results showed: 67 patients (16.6%) between 0-10, 272 patients between 11-25 (66.8%) and 68 pts> 26 (16.6%). At the logistic regression analysis, RS score was significantly associated with ER (p¼0.004), PgR (p<0.001), and Ki67% (p<0.001) with the threshold equal to 25. Above patients with RS25, the generalized linear regression resulted in a well calibrated model with an AUC of 92.2% (sensitivity 84.2%; specificity 80.1%). External validation set included 180 patients and confirmed the model performance with an AUC of 82.3% and good calibration. A nomogram predicting RS score25 was generated.

Diagnosis and management of breast lymphoma: a single- institution retrospective analysis

Background: Breast lymphoma (BL) is a rare entity. Although mimicking clinical presentation and radiological features of epithelial breast malignancies, prognosis and treatment management are different. Here we report our single institution experience in such population.Methods: The database of our pathology institute was retrospectively searched for breast neoplasms coded as lymphoma from January 2000 to January 2017. The assessed data were: gender, age at diagnosis, laterality, stage (primary or secondary), histopathological diagnosis and grading (high vs. low) treatment, recurrence and survival status. Progression-free survival (PFS), overall survival (OS) and 5-year recurrence rate were estimated. An exploratory analysis comparing survival (PFS, OS and 5-year recurrence rate) between high- and low-grade histotypes and primary and secondary stages was performed.Results: Fifteen patients were included in the analysis. All patients were affected by B-cell type lymphomas; the most frequent subtype was diffuse large B-cell lymphoma (DLBCL). Patients affected by high-grade lymphomas were treated with polychemotherapy followed in about half of cases by consolidative ipsilateral breast radiotherapy. Patients affected by low-grade lymphomas were treated with local treatment only (surgical resection +/− radiotherapy). No patient received radical surgery or axillary dissection. At a median follow up of 9 years, four patients had relapsed (all affected by high-grade lymphomas) with central nervous system (CNS) involved in half cases. Both median PFS and OS were not reached, but a slight trend toward a better survival was noticed in favor of primary breast lymphoma (PBL), while a clearer survival benefit was observed in the low-grade cohort. The estimated 5-year survival rate was 77% within the whole population, 78% vs. 66% for primary vs. secondary and 100% vs. 66% for low-grade vs. high-grade BL.Conclusions: Our data concerning clinical and pathological features are consistent with those previously reported. Management of disease was based on histology. Although being a rare disease it is mandatory to take it into account for differential diagnosis since treatment and prognosis differs widely from primary breast malignancies.

Background: Radiation therapy (RT) represents a standard adjuvant treatment for high risk patients with breast cancer, even in post-mastectomy setting. Mammary expanders and prostheses often do not tolerate RT producing a deterioration of aesthetic profile that can lead to clinical risks (rejection, sepsis) or new surgery. This retrospective series evaluates clinical and aesthetic results in patients who did or did not undergo adjuvant RT after reconstruction for breast cancer surgery. Material a

Purpose or Objective: Post-mastectomy radiation therapy  (RT) is a prophylactic adjuvant treatment for high risk patients with breast cancer. Mammary prostheses or expanders often do not tolerate RT causing a reduction of aesthetic profile and, even more, an exposure to clinical risks or new surgery. In this retrospective study clinical and aesthetic results were quantified in patients who did or did not undergo adjuvant RT after reconstruction for breast cancer.  Material and Methods: Patients who underwent mastectomy with immediate mammary reconstruction for breast cancer and who had a follow-up (FUP) period of at least six months were selected for this retrospective study. Two subgroups were defined between irradiated or not patients. All the patients were submitted to the scheduled surgical and oncoplastic FUP and the irradiated ones underwent also a RT FUP program. For both groups local infection rate (IR), lipofilling rate (LR), reconstruction with DIEP flap rate (DIEPR), local control (LC) and distant failure rate (DFR) were analyzed. All chi square tests were performed on MedCalc. For the irradiated patients acute and late toxicities were also registered according to CTCAE v4.0 scale.   Results: From January 2012 to April 2015, 152 patients underwent mastectomy with prostheses or expanders. Out of them, 76 pts were candidates to standard adjuvant RT for high risk factors, according to NCCN guidelines. Mean age was 48 years (range 32-74). Median FUP was 28 months (range 6-44). IR, LR, DIEPR are reported in Table 1. 21% of not irradiated pts underwent second surgery to replace the expander with the prostheses. LC was equal in both group. High risk pts had higher DFR than low-medium risk ones (5.26% vs 0 with p<0.05). Regarding irradiated patients acute skin toxicities G0, G1, G2 and G3 were 38%, 40%, 12% and 9.8% respectively. We reported late skin toxicities rates of: G0 20%, G1 28%, G2 42% and G3 10% (Figure 1)
