Efficiency of lysine utilization by growing meat quail.

The objective of present study was to estimate the efficiency of lysine utilization by meat quail of 21 to 35 d of age. A total of 500 meat quails were distributed in a completely randomized design in a 2 × 5 factorial arrangement, with 2 sexes (male and female) and 5 digestible lysine levels (0.714, 0.816, 0.918, 1.020, and 1.122%) and 5 replications of 10 birds each. The variables studied were feather-free body weight (FFBW), feed intake (FI), lysine intake (LysI), feather-free body protein deposition (FFBPD), feather-free body lysine deposition (FFBLysD), feather-free body fat deposition (FFBFatD), feather weight (FW), feather protein deposition, feather lysine deposition, and feather fat deposition. The FFBW, FFBPD, FFBLysD, and FFBFatD were regressed as a function of LysI for each sex to estimate the efficiency of lysine utilization in the feather-free body (ELysFFB), and the individual equations were compared. In addition, a multiple regression without intercept was also used to estimate the ELysFFB and in feathers (ELysF) individually. To compare the ELysFFB obtained by the different methods, the t-statistic was used. There was no effect on sex × lysine level interactions for any variable. The females showed higher FFBW (5.07%) and FFBFatD (26.23%) than males. All variables increased with the level of dietary lysine, with the exception of FI, FW, and the deposition of nutrients within them. The ELysFFB values obtained by simple linear regression and multiple linear regression were 48.0 and 44.6%, respectively. As there was no difference in the efficiencies estimated by the different methodologies, the best estimate of ELysFFB was 46.3%, that is, the average. The best estimate of ELysF was 18.1%, obtained by multiple linear regression.

Cashew apple ( L.) extract from a by-product of juice processing: assessment of its toxicity, antiproliferative and antimicrobial activities.

Cashew apple extract (CAE) is a product with intense yellow color obtained from residual fibers of juice processing. Although CAE is known to be rich in carotenoids and anacardic acids, the biological activities of this potential natural food colorant remain unexplored. The present study is the first to investigate the toxicity, antiproliferative and antimicrobial activities of the lyophilized CAE (L-CAE) and its encapsulated products, using maltodextrin (M-CAE) or cashew gum (CG-CAE) as carriers. In addition to their high carotenoid content, the phenolic contents in all materials was determined using UPLC-QTOF-MS. The acute toxicity was performed using adult zebrafish (); antiproliferative activity was assessed using seven different human tumor cell lines [U-251 (glioblastoma), MCF-7 (breast, adenocarcinoma), NCI-ADR/RES (multidrug-resistant ovarian adenocarcinoma), NCI-H-460 (lung, large cell carcinoma), PC-3 (prostate, adenocarcinoma), OVCAR-3 (ovarian adenocarcinoma), and HT-29 (colon, adenocarcinoma)] and an immortalized human keratinocyte (HaCaT) while the antimicrobioal activity was evaluated on ATCC 25923, ATCC 19115, ATCC 25922 and Typhimurium ATCC 51812 microorganisms. Both lyophilized and encapsulated CAE samples did not exert acute toxicity against zebrafish neither antiproliferative effect against human tumor and non-tumor cell lines. Further, L-CAE showed potential antimicrobial activity against which was confirmed using electron microscopy. The current findings demonstrated that CAE is a potential source of bioactive compounds to use as an additive in the food industry.

Single-Position Prone Transpsoas Lateral Interbody Fusion Including L4L5: Early Postoperative Outcomes.

Background: The lateral lumbar interbody fusion (LLIF) was a revolutionary approach devised by Luiz Pimenta that allowed the surgeon to access the lumbar spine through the major psoas muscle. Although the traditional LLIF had enabled enormous advances, the technique has its drawbacks. A new concept to perform the traditional LLIF has been proposed, with the patient being prone to decubitus with slightly extended legs. Our study aims to analyze the early outcomes of patients who had undergone the prone transpsoas (PTP) for degenerative spine pathologies including the L4/5 level.

Methods: This study was multicentric, retrospective, nonrandomized, noncomparative, and observational. Only participants who received PTP in L4/5, with no more than 3 levels of intersomatics and fixation no further than S1, were included. The primary outcomes were the onset of new neurologic deficits and postoperative complications. Also, surgery details, such as blood loss and surgery duration, were measured. Neurologic deficits were accessed at the postoperative visit, which ranged from 7 to 14 days after surgery.

Results: Twenty-seven patients fulfilled the inclusion and exclusion criteria, with the majority receiving PTP only in L4/5 (66.6%). The mean surgery time was 182, with 29 minutes of mean transpsoas time. Of the patients, only 1 presented the onset of a motor deficit, while 3 patients presented a new sensory deficit. Five complications occurred, none intraoperative and 5 postoperative, with only 1 directly correlated with the access.

Conclusions: The prone transpsoas is safe and feasible for approaching the L4/5 disk, presenting with a low rate of complication and new-onset neurologic deficits.

A Bibliometric Network Analysis of Coronavirus during the First Eight Months of COVID-19 in 2020.

The COVID-19 pandemic has affected all aspects of society. Researchers worldwide have been working to provide new solutions to and better understanding of this coronavirus. In this research, our goal was to perform a Bibliometric Network Analysis (BNA) to investigate the strategic themes, thematic evolution structure and trends of coronavirus during the first eight months of COVID-19 in the Web of Science (WoS) database in 2020. To do this, 14,802 articles were analyzed, with the support of the SciMAT software. This analysis highlights 24 themes, of which 11 of the more important ones were discussed in-depth. The thematic evolution structure shows how the themes are evolving over time, and the most developed and future trends of coronavirus with focus on COVID-19 were visually depicted. The results of the strategic diagram highlight 'CHLOROQUINE', 'ANXIETY', 'PREGNANCY' and 'ACUTE-RESPIRATORY-SYNDROME', among others, as the clusters with the highest number of associated citations. The thematic evolution. structure presented two thematic areas: "Damage prevention and containment of COVID-19" and "Comorbidities and diseases caused by COVID-19", which provides new perspectives and futures trends of the field. These results will form the basis for future research and guide decision-making in coronavirus focused on COVID-19 research and treatments.

Tissue methylation and demethylation influence translesion synthesis DNA polymerases (TLS) contributing to the genesis of chromosomal abnormalities in myelodysplastic syndrome.

Aims: DNA methylation has its distribution influenced by DNA demethylation processes with the catalytic conversion of 5-methylcytosine (5mC) into 5-hydroxymethylcytosine (5hmC). Myelodysplastic syndrome (MDS) has been associated with epigenetic dysregulation of genes related to DNA repair system, chronic immune response and cell cycle.

Methods: We evaluated the tissue DNA methylation/hydroxymethylation in bone marrow trephine biopsies of 73 patients with MDS, trying to correlate with the mRNA expression of 21 genes (, , , and ).

Results: The ) was significantly higher in patients with chromosomal abnormalities than patients with normal karyotype (95% CI -27.127779 to -2.368020; p=0.022). We observed a higher 5mC/5hmC ratio in patients classified as high-risk subtypes compared with low-risk subtypes (95% CI -72.922115 to -1.855662; p=0.040) as well as patients with hypercellular bone marrow compared with patients with normocellular/hypocellular bone marrow (95% CI -69.189259 to -0.511828; p=0.047) and with the presence of dyserythropoiesis (95% CI 17.077703 to 51.331388; p=0.001). DNA pols with translesion activity are significantly influenced by methylation. 5mC immunoexpression increases, the expressions of (r=-0.816; r =0.665; p=0.000) (r=-0.790; r=0.624; p=0.001) (r=-0.635; r=0.403; p=0.020) (r=-0.633; r=0.400; p=0.036 and (r=-0.578; r=0.334; p=0.049) decrease.

Conclusions: Our results confirm that there is an imbalance in the DNA methylation in MDS, influencing the development of chromosomal abnormalities which may be associated with the low expression of DNA polymerases with translesion synthesis polymerases activity.

Brazilian Group of Gastrointestinal Tumours' consensus guidelines for the management of gastric cancer.

Gastric cancer is among the ten most common types of cancer worldwide. Most cases and deaths related to the disease occur in developing countries. Local socio-economic, epidemiologic and healthcare particularities led us to create a Brazilian guideline for the management of gastric carcinomas. The Brazilian Group of Gastrointestinal Tumors (GTG) invited 50 physicians with different backgrounds, including radiology, pathology, endoscopy, nuclear medicine, genetics, oncological surgery, radiotherapy and clinical oncology, to collaborate. This document was prepared based on an extensive review of topics related to heredity, diagnosis, staging, pathology, endoscopy, surgery, radiation, systemic therapy and follow-up, which was followed by presentation, discussion, and voting by the panel members. It provides updated evidence-based recommendations to guide clinical management of gastric carcinomas in several scenarios and clinical settings.

A Proposal for Classification of Retinal Degeneration in Spinocerebellar Ataxia Type 7.

The aim of this study is to propose a classification system for the spinocerebellar ataxia type 7 retinal degeneration (SCA7-RD). Twenty patients with molecularly confirmed SCA7 underwent slit lamp examination, fundus photography, and optical coherence tomography (Spectralis®). Scale for the Assessment and Rating of Ataxia (SARA) and International Cooperative Ataxia Rating Scale (ICARS) were applied, and age, sex, age at symptom onset, and number of CAG expansions were recorded. After analyzing the ophthalmological findings in each participant, a panel of retinal disease experts created a qualitative classification system for SCA7-RD comprising four stages. We assessed the correlations of retinal degeneration severity with SARA and ICARS scores, number of CAG repeats in ATXN7 allele, and age at symptom onset. We graded retinal degeneration as stage 1 in nine participants, as stage 2 in five, and as stage 3 in six. No differences in age and visual symptoms duration were found between groups. SARA and ICARS scores correlated with the severity of SCA7-RD on the classification system (p = 0.024 and p = 0.014, respectively). After adjusting for disease duration, retinal disease stage association with SARA and ICARS scores remained significant (ANCOVA, p < 0.05). The classification system for SCA7-RD was able to characterize different disease stages representing the landmarks in the cone-rod dystrophy natural history. Neurodegeneration appears to occur in parallel in the cerebellum and in the visual pathway. We conclude that retinal degeneration in SCA7 is a potential biomarker of the neurological phenotype severity.

Esophagectomy in a patient with pulmonary histoplasmosis: A case report of surgical aspects and complications (with video).

Introduction: Esophagectomy is a challenging procedure associated with considerable morbidity. Previous pulmonary diseases, such as histoplasmosis fungal infection, may interfere in operative and postoperative outcomes after esophagectomy. Anastomotic leakage is one of the most feared complications after esophagectomy. However, new therapies developed such as vacuum procedure and esophageal prosthesis have been provenly beneficial.

Presentation Of Case: We present a case with squamous cell carcinoma of the mid esophagus portion on a young patient with a pulmonary histoplasmosis history. After a multidisciplinary board, the patient underwent transhiatal esophagectomy with gastric-pull up and cervical anastomosis due to pulmonary disease. The patient later developed an anastomotic leak with mediastinal abscess. We describe this complication's management via an endoscopic vacuum system, esophageal prosthesis, and exhibit a video illustrating the technique.

Discussion: We illustrate the management of esophageal cancer associated with previous pulmonary disease. Histoplasmosis may misunderstand the esophageal cancer staging, and it can contribute to anastomotic leakage occurrence. An endoscopic vacuum system is an excellent tool for treating esophagogastric anastomosis fistula after esophagectomy, even when the drainage is accumulated in the mediastinum. The esophageal prosthesis may be used after mediastinal abscess resolution.

Conclusion: Treatment of the association of esophageal cancer and histoplasmosis is feasible. However, care should be taken to avoid highly potential postoperative complications.

Tucum Fiber from Amazon Palm Tree: Novel Reinforcement for Polymer Composites.

The replacement of synthetic fibers by natural fibers has, in recent decades, been the subject of intense research, particularly as reinforcement of composites. In this work, the lesser known tucum fiber, extracted from the leaves of the Amazon palm tree, is investigated as a possible novel reinforcement of epoxy composites. The tucum fiber was characterized by pullout test for interfacial adhesion with epoxy matrix. The fiber presented a critical length of 6.30 mm, with interfacial shear strength of 2.73 MPa. Composites prepared with different volume fractions of 20 and 40% tucum fiber were characterized by tensile and Izod impact tests, as well as by ballistic impact energy absorption using .22 ammunition. A cost analysis compared the tucum fiber epoxy composites with other natural and synthetic fiber reinforced epoxy composites. The results showed that 40 vol% tucum fiber epoxy composites increased the tensile strength by 104% and the absorbed Izod impact energy by 157% in comparison to the plain epoxy, while the ballistic performance of the 20 vol% tucum fiber composites increased 150%. These results confirmed for the first time a reinforcement effect of the tucum fiber to polymer composites. Moreover, these composites exhibit superior cost effectiveness, taking into account a comparison made with others epoxy polymer composites.

Brain aspergilloma in an immunocompetent individual: A case report.

Background: Aspergillosis is caused by fungi from the species, mainly . Patients with aspergillosis typically have an underlying immunosuppression, and it is rare within the central nervous system (CNS) in an immunocompetent host. The mortality rate is as high as 90% if untreated, and the diagnosis is usually delayed due to nonspecific clinical presentation. This study described a case of CNS aspergillosis in an immunocompetent patient, through which we sought to discuss the current knowledge regarding brain focusing in the immunocompetent host.

Case Description: A 45-year-old male presented with 2 years of low-intensity headache and history of chronic sinusitis with epistaxis in the left nostril. Fifteen days before admission, he had high-intensity headache, episodes of transient aphasia, and seizure. Imaging showed a contrast-enhancing mass within the left maxillary sinus and another lesion in the left frontal lobe. The left frontal craniotomy was conducted, and complete resection was achieved. Biopsy identified , and antifungal therapy was initiated. After 2 weeks, a new lesion was detected in the right frontal lobe, and the patient underwent a new procedure with complete lesion resection. Follow-up at 3 weeks did not reveal any evidence of residual or recurrent disease. The patient did not develop neurological complaints and was referred for resection of the remaining lesion by an otolaryngology team.

Conclusion: Being one of the few cases reporting a successful outcome for brain aspergilloma in an immunocompetent patient after complete surgical resection and amphotericin B and itraconazole therapy, we sought to reveal novel insight into brain aspergillosis.

Genetic polymorphisms in and were associated with dental caries and gingivitis.

Objective: Evaluate the association between single nucleotide polymorphisms (SNPs) in () gene (rs1800795) and in () gene (rs1143627 and rs1143629) with dental caries and gingivitis in Brazilian children.

Material And Methods: Three hundred and fifty-three children aged 8-11 years were included. Visible biofilm and gingival bleeding were evaluated by Community Periodontal Index. The International System for Detection and Assessment of Carious Lesions (ICDAS) was used to investigate dental caries. Real-time PCR evaluated SNPs in the DNA. Chi-square test, haplotype analysis and logistic regression were applied (alpha of 5%).

Results: The GG genotype in rs1800795 () decreases the risk of gingivitis in a co-dominant model ( = .05; OR = 0.64). The GG genotype in rs1143627 () reduces the risk of dental caries (Co-dominant model: ICDAS versus ICDAS:  = .05; OR = 0.55. ICDAS versus ICDAS:  = .02; OR = 0.49. Recessive model: ICDAS versus ICDAS:  = .005; OR = 0.48. ICDAS versus ICDAS:  = .004; OR = 0.45. Logistic regression: ICDAS versus ICDAS:  = .05; OR = 0.24; CI 95%= 0.05-1.00). The GG genotype in rs1143629 was more frequent in ICDAS ( = .05; OR: 0.60). In the haplotype analysis, was associated with gingivitis.

Conclusion: The rs1800795 in gene was associated with gingivitis. The rs1143627 and rs1143629 in were associated with dental caries and gingivitis.

Use of checkpoint inhibitors in gray zone lymphoma.

Checkpoint inhibitors, cancer immunotherapies, are the new forms of treatment for gray zone lymphoma, a rare subtype that combines the characteristics of both Hodgkin and non-Hodgkin disease forms. Programmed cell death protein 1/programmed cell death ligand 1 (PD-L1/PD-1) and cytotoxic T-lymphocyte-associated protein 4 (CTLA-4) modulate the immune system function. Immunological checkpoints can be stimulatory or inhibitory, and tumors can use these checkpoints to protect against immune system attacks. This is a case report of a difficult diagnosis and describes the most current treatment using checkpoint inhibitors, through the review of the clinical record of a patient diagnosed with gray area lymphoma in August 2019, using a descriptive and cross-sectional analysis of the clinical history and disease evolution. The case showed that pembrolizumab therapy is an effective treatment option for patients with rare gray zone lymphoma refractory to different lines of treatment. Both the diagnosis and treatment of gray area lymphoma remain a challenge for the medical and multiprofessional teams, and collaboration between them ensured effective treatment for the patient.

Tuberculosis in Health Care Workers and the Impact of Implementation of Hospital Infection-Control Measures.

: Most studies that have evaluated the impact of infection-control measures (ICM) reported a decrease in latent tuberculosis (TB) and not in TB. The objective of this study was to evaluate the impact of ICM on TB incidence among Health Care Workers (HCW's). We conducted a retrospective record review study in a general, tertiary care, university-affiliated hospital. All TB case reports among HCWs in the hospital from 2005 to 2018 were reviewed. The TB incidence was measured before and after 2012 to evaluate the impact of ICM implemented. In total, there were 53 TB cases. The number of TB cases before and after the implementation of ICM was 42 (incidence: 100.0 cases/100,000 HCWs/year) and 11 (incidence: 26.2 cases/100,000 HCWs/year), respectively ( < .0001). : TB incidence among HCWs reduced significantly after the implementation of ICM. The establishment of ICM, such as written TB infection control plan, monitoring, screening, training, and education, can reduce TB incidence.

Barrier device prototype for open tracheotomy during COVID-19 pandemic.

Objective: To present a low-cost prototype for a barrier enclosure device which can be used during open surgeries such as tracheotomy.

Methods: We provide detailed description of a novel device called COVID-Box, developed by The Surgical Airway Team for COVID-19 Pandemic, a temporary task force formed by Walter Cantídio University Hospital. Safety guidelines for performing tracheotomies in COVID-19 patients are also stated.

Results: Our prototype device provides greater hand mobility than previous barrier devices reported, making it more suitable for airway surgical procedures, such as tracheotomy. The curved shaped format and the customizable access ports provides ergonomics, without compromising safety.

Conclusion: The COVID-Box appears to be an efficient, reproduceable, low-cost barrier enclosure device that can be used for open tracheotomies in ICU patients during the COVID-19 pandemic.

Marine debris on a tropical coastline: Abundance, predominant sources and fate in a region with multiple activities (Fortaleza, Ceará, northeastern Brazil).

The aim of the present study was to perform the first assessment of the abundance and classification of marine debris as well as determine the sources, transport and fate of this debris on an urbanized coast with multiple human activities. More than 80% of the marine debris was composed of synthetic materials. The beached marine debris was classified according to size. Meso-debris accounted for the highest portion of contamination (55%), followed by macro-debris (25.1%) and small debris (19.9%). Contamination by debris, such as cotton swabs (31%) and lollipop sticks (36.8%) accounted for the largest portion of the small debris class. Human recreational activities were the predominant source of debris, followed by navigation/fishing activities, domestic activities and industrial/port activities. The assessment of the predominance of human activities and the results of the model revealed a larger contribution of debris from recreational activities on nearby beaches on the small to larger scale and that rivers exert less of an influence due to the fact that they do not flow the entire year.

Effect of pulsed light on postharvest disease control-related metabolomic variation in melon (Cucumis melo) artificially inoculated with Fusarium pallidoroseum.

Pulsed light, as a postharvest technology, is an alternative to traditional fungicides, and can be used on a wide variety of fruit and vegetables for sanitization or pathogen control. In addition to these applications, other effects also are detected in vegetal cells, including changes in metabolism and secondary metabolite production, which directly affect disease control response mechanisms. This study aimed to evaluate pulsed ultraviolet light in controlling postharvest rot, caused by Fusarium pallidoroseum in 'Spanish' melon, in natura, and its implications in disease control as a function of metabolomic variation to fungicidal or fungistatic effects. The dose of pulsed light (PL) that inhibited F. pallidoroseum growth in melons (Cucumis melo var. Spanish) was 9 KJ m-2. Ultra-performance liquid chromatography (UPLC) coupled to a quadrupole-time-of-flight (QTOF) mass analyzer identified 12 compounds based on tandem mass spectrometry (MS/MS) fragmentation patterns. Chemometric analysis by Principal Components Analysis (PCA) and Orthogonal Partial Least Squared Discriminant Analysis (OPLS-DA) and corresponding S-Plot were used to evaluate the changes in fruit metabolism. PL technology provided protection against postharvest disease in melons, directly inhibiting the growth of F. pallidoroseum through the upregulation of specific fruit biomarkers such as pipecolic acid (11), saponarin (7), and orientin (3), which acted as major markers for the defense system against pathogens. PL can thus be proposed as a postharvest technology to prevent chemical fungicides and may be applied to reduce the decay of melon quality during its export and storage.

Effects of sex and chronic cigarette smoke exposure on the mouse cecal microbiome.

Rationale: Chronic smoke exposure is associated with weight loss in patients with Chronic Obstructive Pulmonary Disease (COPD). However, the biological contribution of chronic smoking and sex on the cecal microbiome has not been previously investigated.

Methods: Adult male, female and ovariectomized mice were exposed to air (control group) or smoke for six months using a standard nose-only smoke exposure system. DNA was extracted from the cecal content using the QIAGEN QIAamp® DNA Mini Kit. Droplet digital PCR was used to generate total 16S bacterial counts, followed by Illumina MiSeq® analysis to determine microbial community composition. The sequencing data were resolved into Amplicon Sequence Variants and analyzed with the use of QIIME2®. Alpha diversity measures (Richness, Shannon Index, Evenness and Faith's Phylogenetic Diversity) and beta diversity (based on Bray-Curtis distances) were assessed and compared according to smoke exposure and sex.

Results: The microbial community was different between male and female mice, while ovariectomy made the cecal microbiome similar to that of male mice. Chronic smoke exposure led to significant changes in the cecal microbial community in both male and female mice. The organism, Alistipes, was the most consistent bacteria identified at the genus level in the cecal content that was reduced with chronic cigarette exposure and its expression was positively related to the whole-body weight of these mice.

Conclusion: Chronic smoke exposure is associated with changes in the cecal content microbiome; these changes may play a role in the weight changes that are observed in cigarette smokers.

Copper and lead ion removal from wastewater using fava d'anta fodder (Dimorphandra gardneriana Tulasne).

The contamination of bodies of water by potentially hazardous elements has in recent decades become an environmental problem that poses serious risks to humans, fauna, flora and microbiota, compromising the quality of life of the present ecosystem. Therefore, effluents must be properly treated in a legally acceptable manner before their disposal in the environment. With this in mind, adsorption presents itself as an inexpensive efficient technique for the removal of potentially hazardous elements from effluents with excellent adsorption capacities when natural adsorbents are used. In this study, fava d'anta fodder was used in its crude and alkalinized form to remove Cu(II) and Pb(II) ions. Equilibrium studies were carried out using adsorption isotherms in batch systems with mono- and multi-elementary systems containing the two ions. The Langmuir and Freundlich models were applied to the isotherm studies, with the ions being better suited to the Langmuir model, with maximum adsorption capacities of 24.45 mg g and 68.49 mg g (crude form) and 11.12 mg g and 35.34 mg g (alkalinized form) in the mono-elementary system for Cu(II) and Pb(II) ions, respectively.

Medical discernment and dialogical praxis: treatment as healing oneself.

This essay investigates the hermeneutic idea of health and the resulting formative notion of treatment. In its first part, the essay diagnoses, based on some texts of the German philosopher Hans-Georg Gadamer, the increasing technologization of contemporary professions and, specifically in the case of medicine, the risk of disappearance of self-treatment that this technologization causes. In addition to medicine, it also briefly takes psychoanalysis and pedagogy to exemplify the risk of over-specialized professionalization. In the second part, the essay seeks to doubly ground the hermeneutical idea of health: on the one hand, in the heritage of Hippocratic medicine that supports Gadamer's point of view and, on the other hand, in dialogical praxis, considering it the core of philosophical hermeneutics itself. Still in its second part, the essay interprets three aspects of the Gadamerian dialogue, translating them into medical professional practice. Finally, in the third and last part, the essay shows that hermeneutically-understood medical treatment leads to self-treatment, which is an indispensable, but not sufficient, condition of the patient's cure. In summary, when patients are mobilized by the dialogical praxis of medical discernment, they are more able to understand the importance of their taking treatment as self-treatment.

Influence of earthworm presence and hydraulic loading rate on the performance of vertical flow constructed wetlands.

In order to know the behaviour and performance of a vertical flow constructed wetland (VF-CW) operating with earthworm, this study evaluated the earthworm presence linked to hydraulic loading rates applied for domestic wastewater treatment. Two VF-CW units (710 cm², 0.75 m deep, with sand as filter media and planted with ) operated with a variable hydraulic and organic loadings rates (200 mm d/123 g COD m d; 280 mm d/186 g COD m d; 160 mm d/94 g COD m d). Although the efficiency of COD load removal was similar (around 70%) for both CWs, the efficiency of total nitrogen load removal was around 95% throughout the evaluated period. The nitrification-denitrification process was identified with and without the presence of earthworm. VF-CW with earthworms showed higher hydraulic conductivity values (from 0.11 to 0.14 m h) compared with the VF-CW wetland without earthworms (0.07-0.09 m h). This study showed that the use of earthworms in CWs can be associated mainly with a preventive measure of clogging, which requires measures to maintain earthworms inside the bed media. Additionally, the earthworm presence has an ingestion mechanism of organic and inorganic solid particles in wastewater which excretes them as finer particles.

Emerging contaminants are increasingly present in the environment, and their appearance on both the environment and health of living beings are still poorly understood by society. Conventional sewage treatment facilities that are under validity and were designed years ago are not developed to remove pharmaceutical compounds, their main focus is organic and bacteriological removal. Pharmaceutical residues are associated directly with quantitative production aspects as well as inadequate waste management policies. Persistent classes of emerging compounds such as xenobiotics present molecules whose physicochemical properties such as small molecular size, ionizability, water solubility, lipophilicity, polarity and volatility make degradability, identification and quantification of these complex compounds difficult. Based on research results showing that there is a possibility of risk to human and environmental health the presence of these compounds in the environment this article aimed to review the main pharmaceutical and xenobiotic residues present in the environment, as well as to present the most common methodologies used. The most commonly used analytical methods for identifying these compounds were HPLC and Gas Chromatography coupled with mass spectrometry with potential for characterize complex substances in the environment with low concentrations. An alternative and low-cost technology for emerging compound treatment demonstrated in the literature with a satisfactory performance for several types of sewage such as domestic sewage, wastewater and agroindustrial, was the Wetlands Constructed. The study was able to identify the main compounds that are being found in the environment and identify the most used analytical methods to identify and quantify these compounds, bringing some alternatives combining technologies for the treatment of compounds. Environmental contamination is eminent, since the production of emerging compounds aims to increase along with technological development. This demonstrates the need to explore and aggregate sewage treatment technologies to reduce or prevent the deposition of these compounds into the environment.

Management of Type 2 Diabetes in Developing Countries: Balancing Optimal Glycaemic Control and Outcomes with Affordability and Accessibility to Treatment.

With the growing prevalence of type 2 diabetes, particularly in emerging countries, its management in the context of available resources should be considered. International guidelines, while comprehensive and scientifically valid, may not be appropriate for regions such as Asia, Latin America or Africa, where epidemiology, patient phenotypes, cultural conditions and socioeconomic status are different from America and Europe. Although glycaemic control and reduction of micro- and macrovascular outcomes remain essential aspects of treatment, access and cost are major limiting factors; therefore, a pragmatic approach is required in restricted-resource settings. Newer agents, such as sodium-glucose cotransporter 2 inhibitors and glucagon-like peptide 1 receptor agonists in particular, are relatively expensive, with limited availability despite potentially being valuable for patients with insulin resistance and cardiovascular complications. This review makes a case for the role of more accessible second-line treatments with long-established efficacy and affordability, such as sulfonylureas, in the management of type 2 diabetes, particularly in developing or restricted-resource countries.

Allometric coefficients of major chemical components of meat quail raised in different thermal environments.

The objective of the present study was to estimate and compare allometric coefficients of the major chemical components of meat quail raised in different thermal environments, based on protein weight of feather free body (FFB) and feathers. In total, 300 meat quail, males and females, were distributed in a completely randomized design with 2 treatments (climatized environment, 26°C, and non-climatized environment, 29°C) and 6 replicates of 25 birds each. On the first day, 36 birds were selected to form the reference group and from this day on, 2 quail were weekly sampled from each cage. All selected birds were fasted for 24 h, weighed, slaughtered, plucked, and reweighed. The FFB and feathers were ground separately to obtain homogeneous samples, which were freeze-dried to determine the water content, and thereafter, ground again in a micromill before analyzing for protein, lipid, and ash using AOAC procedures. The adjustment of the allometric equations was made using crude protein (CP) weight as the independent variable and water, lipid, and ash weight as the dependent variables. The data of each dependent variable were transformed into natural logarithm (ln), regressed according to lnCP, and subjected to a parallelism test. In the FFB, water showed early development and lipid and ash showed late development in relation to the CP weight. In feathers, water and ash weight showed early development in relation to the feathers protein weight, whereas lipids showed late development. The environments of 26°C and 29°C did not affect the allometric coefficients that described the growth between the chemical components in the body and in the feathers, except for lipids in male FFB, that showed higher allometric coefficient at 29°C than 26°C. Describing the allometric relationships between the major chemical components of meat quail body is an important step in supporting future research comprised modeling of body growth and nutrition for meat quail.

Two-step ESD: an option for en-bloc resection of extensive colorectal laterally spreading tumors.

 Endoscopic submucosal dissection (ESD) is considered feasible and safe for treatment of colorectal laterally spreading tumors (LST), However it remains a challenge in case of extensive lesions even for experts. This study aimed to describe a new method to facilitate ESD of extensive colorectal LSTs. Between July 2010 and January 2018, 140 patients underwent ESD for colorectal LSTs. Four of them were submitted to two-step ESD and were included in this retrospective study. The submucosal dissection of lesions larger than 12 cm started and continued until the medical team decided to pause the procedure and continue it in a second step. The second procedure was performed 2 days after to finish the en-bloc resection.Three patients were male, with mean age of 67.2y (± 2.2). All lesions were located in the rectum, with a mean size of 153.7 mm (± 33.8). En-bloc and curative resection were successfully achieved in all cases. Mean duration of the first step of the procedure was 255 minutes (± 61.8), and mean duration of the second step was 205 minutes (± 205). Overall mean duration of both steps was 460 minutes (± 168). Mean dissected area in the first step of the procedure was approximately 55 % of the lesion. No adverse events were observed. In conclusion, our results suggest that performing ESD in two steps could be a feasible and safe option for exceptional cases in which is not possible to finish the procedure in one step, avoiding the morbidity of surgical treatment.

Sleep Disorders in Hereditary Ataxias.

Purpose Of Review: In this review, we aim to describe the main sleep disorders observed in patients with different forms of hereditary ataxias and discuss the main pathophysiological mechanisms.

Recent Findings: Several pathological studies have demonstrated that the degenerative process in patients with hereditary ataxias may involve not only the cerebellum, but also other areas of the nervous system, and explain noncerebellar symptoms, such as sleep disorders. Hereditary ataxias are neurodegenerative disorders with heterogeneous genetic and clinical presentation. This group of diseases usually affects other areas of the nervous system, besides the cerebellum, and noncerebellar signs and symptoms may occur, such as sleep disorders. The main sleep disorders related to hereditary ataxias include REM sleep behavior disorder, insomnia, excessive daytime sleepiness, obstructive and central sleep apnea, periodic leg movement in sleep, and restless legs syndrome.

The use of conversation maps in the metabolic control of diabetes in Brazilians: a randomized clinical trial.

Objective: To evaluate effects of an educative intervention over the self-monitoring of blood glucose (SMBG) at home for metabolic control.

Method: A total of 91 people with diabetes participated, recruited from the home capillary blood glycemia self-monitoring program. Two groups of participants were formed: one group participated in the SMBG program at home and with usual care (control group), while the other group participated in the SMBG at home and with educative intervention (intervention group). In total there were 12 meetings, three for each conversation map in the control of diabetes, during four months in 2011 and 2012. For all the analysis, a significance statistical level of 5% ( ≤ 0.05) was adopted.

Results: Most part of participants were females, married, with an average age of 62.1 years old and schooling from four to seven years of study. In the intervention group, an improvement was observed in the following measures: body mass index, abdominal circumference, diastolic blood pressure, fasting plasma glucose, HDL cholesterol, LDL cholesterol and triglycerides. The control group showed improvement in measures of systolic and diastolic blood pressure, fasting plasma glucose, HDL cholesterol and LDL cholesterol. Moreover, an increase on the values of glycated hemoglobin was observed in both groups.

Conclusion: It was observed that there was not statistical significant improvement of the metabolic control. However, it was possible to confirm that an educative intervention for SMBG at home presented a clinical significance, which in turn, resonates in a special way on the health of participants.

Corneal Posterior Curvature Changes After Phacoemulsification Cataract Surgery with 2.75 mm Corneal Incision.

The aim of this study was to evaluate the corneal posterior curvature changes after phacoemulsification cataract surgery, with intraocular lens implantation, with a temporal limbal self-sealing 2.75 millimeters (mm) corneal incision, using a Placido-dual rotating Scheimpflug device. In this prospective intervention study, corneal posterior curvature changes were evaluated in fifty-six patients (56 eyes). All patients underwent corneal tomography using the Galilei G2 (Ziemer Ophthalmic System AG, Port, Switzerland) preoperatively (PRE) and with two weeks (RP15), one month (RP30), and three months (RP90) after phacoemulsification cataract surgery with a temporal limbal self-sealing 2.75 mm incision. Tomographic parameters analyzed in the posterior cornea were the steep curvature (K2), flat curvature (K1), mean curvature (average K), and posterior corneal astigmatism. We did not observe any statistically significant change in the K2, K1, average K, and posterior corneal astigmatism in any postoperative follow-up measurements (RP15, RP30, RP90), showing that the postoperative values tend to be the same as the preoperative ones when measured with the Galilei G2 tomography. In conclusion, the 2.75 mm temporal limbal self-sealing corneal incision in phacoemulsification cataract surgery does not induce significant changes in the posterior corneal curvature parameters of K2, K1, average K, and astigmatism.
