Low O6-methylguanine-dna methytransferase (MGMT) and pan-cytokeratin (PAN-CK) expression via immunohistochemistry in pituitary adenomas.

Introduction. Pituitary adenomas (PA) are the third most common intracranial tumors, with an incidence rate of 10-15%. More than half are invasive, infiltrating adjacent structures. The primary objective of this project was to determine whether MGMT expression is associated with the invasiveness of PA. Material and Method. All patients who underwent surgical decompression consecutively between 2007- 2012 were included. All data were obtained from the case records. Formalin-fixed paraffin-embedded (FFPE) tissue specimens were stained with hematoxylin and eosin (HE) and then examined via light microscope. Paraffin blocks that lacked necrosis and hemorrhage were chosen for histologic examination. In addition to an immunoprofile battery that consisted of Ki-67 and p53, MGMT, S-100 and Pan-CK were evaluated as well. Results. The subjects included 25 women and 15 men. The mean age was 48.9 ± 14.5 years. Of these, 63% of cases involved the invasion of adjacent structures. Of the PA, 17 (42%) were non-functioning pituitary adenomas (NFPA). There was a statistically significant relationship between the invasiveness and Ki-67, p53, MGMT expression, and prolactinoma. Gonodotropinomas were mostly non-invasive. FPAs presented invasive features more frequently than NFPAs. Pan-CK was positive in GH-secreting adenomas but negative in FSH- and LH-secreting adenomas. Conclusion. Ki-67 and p53 in lower expression level can be used for evaluating invasiveness but not for recurrence. MGMT expression can be a useful IHC indicator for invasiveness. However, Pan-CK cannot be used for invasiveness or aggressiveness.

Introduction: To date, several cell proliferation markers, apoptosis, vascular markers, oncogenes, tumor suppressor genes, cell cycle mediators, microRNA (miRNAs), and long noncoding RNAs (lncRNAs) have been identified to be involved in the tumorigenesis, migration, proliferation and invasiveness of pituitary adenomas. There are still no reliable morphologic markers predictive of pituitary adenoma recurrence. Recent scientific research introduced new techniques to enable us to attain new information on the genesis and biologic behavior of pituitary adenomas. Areas covered: This review covers selected, compelling and cumulative information in regards to TACSTD family (EpCAM, TROP2), neuropilin (NRP-1), oncogene-induced senescence (OIS), fascins (FSCN1), invasion-associated genes (CLDN7, CNTNAP2, ITGA6, JAM3, PTPRC and CTNNA1) EZH2, and ENC1 genes and endocan. Expert commentary: Ongoing research provides clinicians, surgeons and researchers with new information not only on diverse pathways in tumorigenesis but also on the clinical aggressive behavior of pituitary adenomas. Newly developed molecular techniques, bioinformatics and new pharmaceutical drug options are helpful tools to widen the perspectives in our understanding of the complex nature of pituitary tumorigenesis. The discovery of new molecular biomarkers can only be accomplished by continuing to investigate pituitary embryogenesis, histogenesis and tumorigenesis.

IDH-mutant glioma specific association of rs55705857 located at 8q24.21 involves MYC deregulation

Science Reports

The single nucleotide polymorphism rs55705857, located in a non-coding but evolutionarily conserved region at 8q24.21, is strongly associated with IDH-mutant glioma development and was suggested to be a causal variant. However, the molecular mechanism underlying this association has remained unknown. With a case control study in 285 gliomas, 316 healthy controls, 380 systemic cancers, 31 other CNS-tumors, and 120 IDH-mutant cartilaginous tumors, we identified that the association was specific to IDH-mutant gliomas. Odds-ratios were 9.25 (5.17–16.52; 95% CI) for IDH-mutated gliomas and 12.85 (5.94–27.83; 95% CI) for IDH-mutated, 1p/19q co-deleted gliomas. Decreasing strength with increasing anaplasia implied a modulatory effect. No somatic mutations were noted at this locus in 114 blood-tumor pairs, nor was there a copy number difference between risk-allele and only-ancestral allele carriers. CCDC26 RNA-expression was rare and not different between the two groups. There were only minor subtype-specific differences in common glioma driver genes. RNA sequencing and LC-MS/MS comparisons pointed to significantly altered MYC-signaling. Baseline enhancer activity of the conserved region specifically on the MYC promoter and its further positive modulation by the SNP risk-allele was shown in vitro. Our findings implicate MYC deregulation as the underlying cause of the observed association.

Left Cerebellary Toxoplasmosis with No Immunodefiency: Case Report.

Toxoplasmosis is caused by an infection with an obligate intracellular parasite, Toxoplasma gondii. The patients are usually immunocompromised. It causes a silent infection in healthy individuals. Trimethoprim sulfamethoxazole is the most common drug used for toxoplasmosis. A fifty year old male applied to our out-patient department with the complaint of right hemiparesis, walking difficulty, ear humming and facial asymmetry on May 2010. Magnetic resonance imaging revealed a left cerebellary mass lesion. He was operated and the pathological findings were consistent with ‘toxoplasmosis’; although we could not demonstrate an immunodeficiency in this patient.

Chordoid Glioma of the Third Ventricle with High Mib-1 Index: A Case Report.

Chordoid glioma is a slowly growing and uncommon neoplasm which involves the third ventricle and is commonly seen in middle age woman. It is a novel entity which was just recently been incorporated to the World Health Organization (WHO) pathological central nervous system (CNS) tumor classification with chordoma like histologic features and glial fibrillary acidic protein immunoreactivity. Hereby we report a case of third ventricular chordoid glioma in a 32 year old man with history of headache and visual disturbances. Magnetic Resonance Imaging (MRI) imaging showed a homogeneously enhancing mass occupying in the third ventricle. The lesion underwent a subcapsular removal through craniotomy. Histologically the tumor had a uniform appearance consisting of clusters and cords of epithelioid cells embedded within a mucinous stroma, containing a lymphoplasmacytic infiltrate. Immunohistochemistry revealed diffuse expression of Glial Fibrillary Acidic Protein (GFAP), vimentin, cytokeratin and S-100 protein reactivity. An uncommonly high MIB-1 index of 10% was detected. There was no nuclear accumulation of p53 protein. Our patient, who rejected radiotherapy and chemotherapy options after surgery (at 52 months), is currently disease-free

Intracerebral Schwannoma: A Case Report.

Schwannomas are encapsulated, benign tumors, which generally originate from the schwann cells on cranial or peripheral nerves. Malignant transformation into neurofibrosarcoma occurs in 1% of these tumors. While schwannomas account for 8% of all central nervous system tumors, intracerebral schwannomas occurring independent from cranial nerves are extremely rare. Their treatment is usually surgical and total excision is generally curative. In this case report, we present a 27-year-old male patient who was admitted to our clinic with focal motor seizures due to a left frontal mass demonstrated on radiological imaging. Surgical treatment was planned and total excision was achieved. Histopathological investigations of the resected mass showed an extremely rare entity of intracerebral schwannoma. Intracerebral schwannomas are further discussed in light of the relevant literature.

However, clinically relevant pituitary tumors presenting with disturbances of hormonal secretion or mass effect are rare and they only represent about 10 % of all surgically resected intracranial neoplasms. Prolactinomas are the most common types of pituitary adenomas. Generally, hormonal expression patterns provided by immunohistochemistry (IHC) studies are correlated with the clinical features and endocrine activity of the patients. Nonetheless, exceptions occur where the immunocytochemical staining is not concordant with the clinical picture. Pituitary adenomas presenting with apoplexy are well known. However, pituitary adenomas causing cerebral stroke and resulting in hemiplegia are unusual. Here, we report an unusual case of prolactinoma with cerebral stroke and sparse prolactin (PRL) expression. A 25-year-old woman complaining of amenorrhea, dysphasia, and left hemiplegia presented with serum PRL level in excess of 4,700 ng/ml. Pre-operational radiology images revealed a giant macroadenoma and a thalamic infarct due to carotid compression. Transcranial surgery was performed. IHC study of the adenoma revealed no hormonal expression other than sparse PRL immunoreactivity. Therefore, a sparsely granulated PRL cell adenoma was diagnosed. The patient is still under follow-up with continuing cabergoline treatment.

This editorial emphasizes recent advances in neonatal and adult stem cells, stem cells of Anterior Pituitary (AP) and stemness markers and biomarkers of pituitary cytogenesis. Therefore concise information for each specific stem cell biomarker is presented in the light of recent cellular and molecular research. Stemness markers have diverse effects either in pituitary embryogenesis and tumor development. This puzzling group consists of several elements, i.e. nestin, Sca1, Sox2+/E- cadherin, S100 proteins, Prop-1, Oct-4, Bmi-1, CD133, beta-catenin, SOX2, Pax6, Notch2, Notch3, Pit1, p57. Each specific marker has an eloquent effect on intimate molecular pathways which take part in pituitary cytogenesis. Therefore their formal roles will be briefly mentioned herein [1,2].

Monostotic fibrous dysplasia involving occipital bone: a case report and review of literature.

Fibrous dysplasia (FD) is a progressive systemic bone tumour of young and it can be seen on cranial bones. FD is divided into three types according to radiological features. The second most common subtype is polyostotic subtype. With this article, we aimed to review and present clinical features, radiological examination, differential diagnosis and treatment management of a case of solitary monostotic fibrous dysplasia of occipital bone. 15 years old female patient admitted to our hospital for a bump and in the back of his head that she noticed 1 month ago. Her physical and neurological examination was normal. On cranial CT examination we detected a bony defect. Her gadolinium enhanced cranial MRI revealed bony defect along with massive gadolinium enhancement in adjacent tissue. On histopathologic examination; PANCK, CD68, CD1a were found negative and CD45, S-100, Vimentine were found positive. Ki-67 was 4,8%. In conclusion, fibrous dysplasia is a progressive bone disease of the young patients. Despite its resemblance to a benign lesion by not being symptomatic it can progress and cause severe bony defects and skin lesions. Total surgical resection is necessary and sufficient for total treatment.

Although the hypothalamus is the master regulator, it is appropriate terming the pituitary gland as the conductor of endocrine orchestra of the human body. The existence of pituitary stem cells in the adult pituitary gland is supported by such findings as postnatal proliferation, differentiation based on environmental alterations, and development of hormoneproducing cells after specific lesions in the pituitary. Stem cell characteristics, including renewal, proliferation abilities, and the presence of stem cells markers, have been demonstrated in adult pituitary cells from mammals. Research into the capacity of “pituitary stem cells” to differentiate in vitro and in vivo will clarify the regulatory mechanisms of these cells. The characteristics of stem cells include self-renewal capacity, lack of specialization, and pluripotency with the ability to differentiate into different cell phenotypes. Adult pituitary cell lineages might have a pituitary stem cell role and the potential participation of these cells in pituitary tumorigenesis. Inevitably numerous questions remain to be resolved, and future challenges are enormous. Therefore, this review provides a comprehensive overview of the motivating recent developments in the pituitary stem cell exploration. The latest expansion in pituitary stem cell research may commence a stimulating period in the pituitary embryology and pathology.

The expression of MMP-11 in benign meningiomas.

The WHO categorises tumours based on their histological properties; however, this classification falls short of understanding their biological behaviours. Recently, there has been an increase in the number of studies on extracellular matrix components with the aim of understanding the biological behaviours of tumours. MMP-11 is a metalloproteinase from the MMP family that has a different expression pattern, mechanism of action, and substrate range compared with the other members of this family. Of the 55 benign meningioma cases examined, 29 were transitional, 16 were meningothelial, and 8 were fibroblastic in nature. In the meningotheliomatous cases, the MMP-11 expression score was high (75%), whereas it was low in the fibrous cases (87.5%). The Ki-67 levels in the cases with high MMP-11 expression scores were significantly higher than those of cases with low MMP-11 expression scores. Although there was no statistically significant correlation between recurrence and the MMP-11 expression score, 5 out of 7 cases with recurrence were found to have high MMP-11 expression scores. According to our study, MMP-11 is expressed in Grade I meningiomas. The level of MMP-11 expression appears to be high in the meningotheliomatous tumours and low in the fibrous subtypes. The tumours with a high level of MMP-11 expression also demonstrated high Ki-67 values.

Modulation of extracellular matrix proteins and hepatate stellate cell activation following gadolinium chloride induced Kuffer cell blockade in an experimental model of liver fibrosis/cirrhosis

Background: Hepatic fibrosis results from an imbalance between fibrogenesis and fibrolysis in the liver. The process of Ito cell activation, which is thought to be the central pathogenic mechanism in liver fibrogenesis/cirrhosis, may involve distinct interactions with Kupffer cells (KCs) mediated by various cytokines and growth factors. The aim of this study was to determine whether targeting KC function using GdCl3, which specifically acts on Kupffer cells, interferes with the manifastation of carbon tetrachloride (CCl4) and dimethylnitrosamine (DMN) induced hepatic fibrosis/cirrhosis. Methods: We assesed the change in distribution of ECM proteins, laminin and fibronectin and the marker of HSC activation, alpha-smooth muscle actin (a-SMA) after liver injury and after KC inactivator, GdCl3-treatment with light microscope immunohistochemistry. Using light microscopy, characteristic changes of fibrosis/cirrhosis were seen in the hepatotoxin-administrated groups. Results: The immunohistochemical profile of anti-laminin was significantly altered in hepototoxintreated groups (p , 0.05) and GdCl3 blocked this effect. The immunoproducts of anti-fibronectin and anti-SMA antibody were not significantly altered in the CCl4-treated group. In contrast, after DMN-induced flbrosis/cirrhosis, laminin, fibronectin and a-SMA staining were significantly increased (p , 0.05) and were present along the sinusoids in cirrhotic liver tissue. However, those ECM proteins and a-SMA staining in the parenchyma and fibrotic nodules decreased but not significantly after GdCl3 treatment. Conclusions: The results suggest that GdCl3 suppressed the activation of lipocytes and their transition from hepatic lipocytes to myofibroblast-like cells in cirrhotic livers in CCl4-treated mice. These results support that treatment with the selective Kupffer cell toxicant GdCl3 prevents stellate cell activation and prevents liver fibrosis/cirrhosis.

Pituitary Ganglioneuroma: Case Report and Literature Review.

We present a case of pituitary ganglioneuroma occurring in a 25 year old woman who presented with headache and hemianopsia. The histologic, immunohistochemical, and ultrastructural features of this tumor support the view that these rare hypothalamic-pituitary tumors are independently functioning neurohormonal units. Immunostaining is strongly positive for neuron-specific enolase (NSE) in all tumor cells and cell processes and for synaptophysin in a few cells and fibers. Few cells closely associated with tumor cells are positive for S100 protein. The cell processes are positive for neurofilament antigen. Ultrastructural analysis documents a spectacular neoplasm of neural derivation. The tumor consists of ganglion cells and areas of neuropil. The most typical tumor cells have relatively lucent spherical nucleus with large nucleolus, and abundant cytoplasm harbors compact sacks of RER membranes studded with ribosomes. Particular emphasis is given to the ultrastructural features which strongly support the functional relationship between the tumor components. It should be kept in mind that unusual and rare ganglionic tumors might show a progressive clinical course and needs surgical intervention as the major part of their treatment whereas ontogenetic and pathogenetic mechanisms taking part still need more work to enlighten pituitary neuronal tumorigenesis.

Diagnostic accuracy of biopsied skin lesions in dermatology and plastic surgery clinics.

Considering the increasing frequency of malignant skin lesions in the last decades, appropriate diagnosis of all kinds of skin lesions has become more important than ever. This study compares the type of skin lesions, seen and treated by dermatology and plastic surgery clinics in Marmara University Hospital, the preferred method of diagnosis, and the correlation between the clinical and pathological diagnosis for each clinic. Patient records, for the years 2000 and 2001, were reviewed retrospectively from the archives of the pathology department. Specimens, 495, from the dermatology clinic and 285 specimens from the plastic surgery clinic were reviewed. In the plastic surgery clinic, excisional biopsy was by far the most utilized method (93.7%). In the dermatology clinic, punch biopsy was the most frequently used method (47.9%). In 317 of the 495 dermatology specimens (64%) and in 175 of the 285 plastic surgery specimens (61.4%), the clinical diagnosis correlated with the pathological diagnosis. Statistical comparison of the overall correct diagnosis ratio between the two clinics did not show a significant difference (p > 0.05). Patient spectrums of the two clinics were different. Benign and malignant neoplastic proliferations were predominant in the patient spectrum of the plastic surgery clinic. Inflammatory and infectious and benign neoplastic proliferations were predominant in the patient spectrum of the dermatology clinic. Biopsy methods preferred were also different, but accuracy in clinical diagnosis was comparable.

Silent Somatotroph Adenoma: A Morphologic, Immunohistochemical and Electron Microscopic Study: A case report

Pituitary adenoma, removed surgically from a 22 old young man with normal serum growth hormone level, and no evidence of acro-megaly by histology, immunohistochemistry and electron micros-copy was studied. Ultrastructurally the tumor cells were shown to correspond to sparsely granulated somatotrophs and immunohisto-chemistry showed no, moderate, or little growth hormone. Clini-cally silent somatotroph adenomas so far are unresolved entities; although electron microscopy shows that they are consisted of sparsely granulated somatotrophs they do not secrete substantial amounts to raise growth hormone and IGF-1 blood levels. Further work is required to clarify the mechanisms accounting for the clini-cal and biochemical silence of these tumors.

Investigation of the Effect of Radiotherapy on the Proliferative Index (Ki-67) in Recurring Diffuse Glial Tumors.

Aim: The aim of this study was to investigate the effect of RT administered to diffuse glial tumors on the proliferation activity and mitotic activity of the tumor. Material-Method: An overall of 49 re-operated recurrent cases (16 cases had received postoperative RT while 33 cases did not receive RT) diagnosed as glial tumor were included in this study. The tumor cohort consisted of diffuse astrocytoma, oligodendroglioma and oligoastrocytomas. Ki-67 proliferating index, mitoses and grades of the primary and recurrent tumors were compared. Result: We found that the grades of recurrent tumors were lower than the grade during the first surgery in cases that had RT compared to those that had not. Similarly, Ki-67 was lower in cases that had received RT. We also noted suppressed mitotic activity in cases that had received RT.

Giant Cell Ependymoma of the Lumbar Spine; Case Report and Review of the Literature.

Giant cell ependymoma (GCE), is a extremely rare subtype of ependymoma. To date 14 cases have been documented in the literature. Here we present a case of GCE occuring in a 32-year-old patient with a 3-month history of progressively increasing pain in low back and right leg. Magnetic Resonance Imaging demonstrated a well circumscribed and enhancing intramedullar mass extending from L1 to L3. Tumor was totally resected. Histological and immunohistochemical findings were consistent with low grade giant cell ependymoma. This case is the first GCE of a lumbar spinal cord. All literature related to GCE was reviewed, and the clinic, radiologic, prognostic and histopathologic characteristics associated with such lesions are discussed, based on the presented case.
