Accuracy of salivary glucose assessment in diagnosis of diabetes and prediabestes

Aims

to assess salivary glucose correlation with blood glucose and its accuracy in diagnosis of diabetes and prediabetes.

Materials and methods

A comparative study including 204 adults in 3 groups (104 type 2 diabetics, 50 prediabetics, 50 non-diabetic controls) aging 18–65 years. The participants were interviewed about their socio-demographic, comorbidities, & drug treatment using a predesigned questionnaire. Salivary & blood samples were collected and analyzed.

Results

Mean salivary glucose was observed to be 23.40 ± 12.755 mg/dl in control group, 42.68 ± 20.830 mg/dl in prediabetic group and 59.32 ± 19.147 mg/dl in diabetic group with significant difference between the 3 groups (P value < 0.001). Salivary glucose was significantly correlated to FBS with strong positive association (r = 0.67, P value < 0.001 in control group, r = 0.56, P value < 0.001in diabetic group and r = 0.36, P value 0.01 in pre-diabetic group). Salivary glucose could differentiate non-diabetics from diabetics (AUC: 0.928, P value < 0.001) with sensitivity (94.2%) and specificity (62%) & differentiate non-diabetics from prediabetics (AUC: 0.928, P value < 0.001) with sensitivity (94.2%) and specificity (62%).

Conclusions

Salivary glucose estimation can serve as valid and non-invasive test for screening and diagnosis of diabetes & prediabetes.

Prevalence of intestinal parasitic infections and its relationship with socio–demographics and hygienic habits among male primary schoolchildren in Al–Ahsa, Saudi Arabia

Objective To investigate the prevalence of different intestinal parasitic infections among male primary schoolchildren in Al-Ahsa, Saudi Arabia, and to explore the possible relative socio-economic, environmental and behavioural sanitary correlates. Methods A cross-sectional descriptive study was performed. A total of 1 289 male students aged from 7 to 12 years were selected randomly from 20 urban and 12 rural primary schools by multistage sampling method. Data collection was carried out by self administered questionnaire form to the parents/guardians of students and included inquires about socio-demographics, environmental conditions, and behavioural sanitary habits. Stool analysis was carried out to examine the presence of intestinal parasitic infections. Results Overall prevalence of parasitic infections was 27.2%, more among rural students. Frequently encountered infections included Entamoeba histolytica/dispar (8.2%), Giardia lamblia (6.5%), Entamoeba coli (4.0%) and Enterobius vermicularis (1.6%). Logistic regression revealed that lower maternal educational level and occupational status, low family income, big family size, poor personal hygienic practices and positive history of previous intestinal infections among family members increased the likelihood of infections. Conclusion Although of low magnitude, intestinal parasitic infections still represent a public health concern among male schoolchildren in Al-Ahsa region. Socio-demographic and poor personal hygienic habits are the main predictors for these infections.

Healthcare providers’ knowledge of standard precautions at the primary healthcare level in Saudi Arabia

The aim of this study was to assess knowledge of standard precautions (SPs) and infection control by healthcare workers (HCWs) at the primary healthcare level in Al-Hassa, and to define possible correlates that influence such knowledge. All HCWs at primary healthcare centres in Al-Hassa were targeted for the survey. A self-administered questionnaire was designed and pre-tested. The data gathered covered basic knowledge of components of SPs and infection control including objectives, hand hygiene, personal protective equipment, sharps disposal, environmental sanitation and care of healthcare providers. Attitudes pertaining to implementation of SPs and training needs were also assessed. Knowledge deficits of components of SPs were demonstrated, especially those related to hand hygiene, sharps disposal, management of sharps injuries and environmental cleaning. Misconceptions were identified in the management of HCWs after exposure to patients with communicable diseases and following sharps injuries. Age of providers, speciality and previous on-the-job training did not influence the knowledge of the participants. Female gender, holding a postgraduate degree and >5 years of experience in primary healthcare were positive correlates to the knowledge score. Lack of resources and training opportunities, and excessive workload were the most frequent factors cited by HCWs for not implementing SPs during routine tasks. The level of knowledge of SPs by HCWs in primary healthcare in Al-Hassa is low. Current training and medical and nursing school curricula should therefore be revised.
